V posledních letech vzrostla v populaci obliba rostlinných přípravků, protože jsou pacienty považovány za čistě přírodní, a proto bezpečné. Rostlinné výtažky jsou také obsaženy v běžně používané kosmetice. Riziko alergických reakcí je zvýšené u osob s atopickým ekzémem, alergickou rýmou a průduškovým astmatem. Nejčastěji dochází k rozvoji alergické kontaktní dermatitidy. Anafylaxe se objevuje výjimečně. Kazuistika pojednává o anafylaxi ve vazbě na lokální aplikaci dužiny z listů aloe pravé u dospívající dívky po předchozí senzibilizaci kosmetickými krémy s obsahem aloe vera.
Medicinal remedies of plant origin became very popular in recent years, and are considered to be „naturally safe“ by many patients. Herbal extracts are commonly used in cosmetics. The risk of allergic reactions to these is increased in patients suffering from allergic rhinoconjunctivitis, bronchial asthma and atopic dermatitis. Allergic contact dermatitis is the most frequently reported side effect, whereas anaphylaxis is considered rare. The case report presents a case of anaphylaxis due to local application of Aloe vera leave pulp in a pubertal girl after previous sensitization to cosmetics containing extracts of Aloe vera.

Alergie na domácí zvířata je spojena s velkým rizikem rozvoje alergické rýmy a astmatu. Senzibilizace alergeny kočky je v populaci velmi rozšířená, dokonce i u lidí, kteří nikdy sami kočku nechovali. Bylo popsáno několik alergenních molekul z různých zdrojů (srst, sliny, moč a produkty kožních žláz), z nichž některé zkříženě reagují s alergeny jiných zvířat. Klinicky nejvýznamnější je alergen Fel d 1, na který rea- guje až u 96 % alergických pacientů. Vzhledem k významu Fel d 1 je diagnostika alergie na kočku jednodušší než u jiných zvířat. Podrobné vyšetření dalších alergenních molekul je přínosné u pacientů s polyvalentní alergií a má význam pro predikci tíže a charakteru alergických potíží. Důležitou součástí terapie je eliminace alergenu kočky, která je však v praxi obtížně proveditelná. Obvykle je doporučována kombinace různých opatření, vedoucích k omezení množství alergenu v prostředí. U některých pacientů byl pozorován přechodný rozvoj tolerance vlastního zvířete při trvalé expozici. Alergenová imunoterapie kvalitním, standardizovaným alergenovým extraktem může v indikovaných případech vést k redukci symptomů a zvýšení kvality života.
Allergy to pets represents a great risk for developing respiratory allergic disease. The sensitizaton to cat is very frequent in the population even in people who had never cat in their homes. Several allergenic molecules have been identified from different sources (dander, saliva, urine and skin glands), some of them having cross-reactivity with other mammalian allergens. The most important cat allergen is Fel d 1 to which up to 96% of allergic patients react. Due to this fact the diagnosis of cat allergy is more simple than in other mammalian. The more precise molecular diagnostic is usefull in pollyallergic patients and in prediction of the disease severity and the risk of asthma developing. Elimination of the cat allergen as an important part of the therapy is difficult to realise. Combination of several metodes of diminution quantitiy of allergen at home is recommended, but none of them is fully effective. In some patients the tolerance of their own animal can develop in the time of high exposure to cat allergens, but it is usually temporary. Allergen immunotherapy with standardised allergenic extract can bring reduction of symptoms and improvement of quality of life even if the contact with cat continues.

Alergické choroby postihujú približne jednu tretinu populácie, pričom obvykle vyžadujú pravidelnú aplikáciu antialergickej liečby, vrátane alergénovej imunoterapie. Alergici a pacienti s chronickými alergickými respiračných chorobami nemajú na základe doposiaľ publikovaných údajov výraznejšie zvýšené riziko závažného ochorenia COVID-19 v porovnaní s bežnou populáciou. Lieky používané v liečbe alergií (s vý-nimkou systémových kortikoidov) nepredstavujú žiadne riziko vo vzťahu k vzniku a komplikovanému priebehu ochorenia COVID-19. Pacienti majú pokračovať vo svojej chronickej antialergickej liečbe (vrátane biologík, imunosupresív či kortikoidov) bez jej prerušovania či znižovania dávky. To platí aj pre alergénovú imunoterapiu, v ktorej má pacient ďalej pokračovať. Ak sa u alergika potvrdí COVID-19, je nevyhnutné kontaktovať ošetrujúceho špecialistu s vysoko individuálnym prehodnotením ďalšej terapeutickej stratégie alergického ochorenia. Cieľom je udržanie dosiahnutej kontroly nad alergickým ochorením.
Allergic diseases affect one third of general population and usually require regular application of anti-allergic treatment of allergen immuno-therapy. Allergic people and patients with chronic allergic respiratory diseases, based on the published studies and case report series, do not yield an increased risk for obtaining COVID-19 compared to the general population. Anti-allergic medicaments /with the exception of systemic corticosteroids) do not represent a risk in relation with COVID-19 and its complicated course. Patients should continue in their chronic anti-allergic treatment (including biologicals, immunosuppressants, and corticosteroids) without the withdrawal or tapering the dose. Patients should continue also in the applied allergen immunotherapy. In case of acquiring COVID-19, it is necessary to contact the caring specialists with the highly-individualized re-assessment of the therapeutic strategy. The aim is to maintain the achieved control over allergic disease.
