BACKGROUND AND AIMS: Right bundle branch block (RBBB) and resulting right ventricular (RV) electromechanical discoordination are thought to play a role in the disease process of subpulmonary RV dysfunction that frequently occur post-repair tetralogy of Fallot (ToF). We sought to describe this disease entity, the role of pulmonary re-valvulation, and the potential added value of RV cardiac resynchronization therapy (RV-CRT). METHODS: Two patients with repaired ToF, complete RBBB, pulmonary regurgitation, and significantly decreased RV function underwent echocardiography, cardiac magnetic resonance, and an invasive study to evaluate the potential for RV-CRT as part of the management strategy. The data were used to personalize the CircAdapt model of the human heart and circulation. Resulting Digital Twins were analysed to quantify the relative effects of RV pressure and volume overload and to predict the effect of RV-CRT. RESULTS: Echocardiography showed components of a classic RV dyssynchrony pattern which could be reversed by RV-CRT during invasive study and resulted in acute improvement in RV systolic function. The Digital Twins confirmed a contribution of electromechanical RV dyssynchrony to RV dysfunction and suggested improvement of RV contraction efficiency after RV-CRT. The one patient who underwent successful permanent RV-CRT as part of the pulmonary re-valvulation procedure carried improvements that were in line with the predictions based on his Digital Twin. CONCLUSION: An integrative diagnostic approach to RV dysfunction, including the construction of Digital Twins may help to identify candidates for RV-CRT as part of the lifetime management of ToF and similar congenital heart lesions.

Transkatétrová náhrada aortální chlopně (TAVI) se s nárůstem klinických zkušeností a technologickým vývojem stala vhodnou alternativou chirurgické náhrady chlopně (SAVR) u chirurgem neakceptovaných pacientů, ale i preferovanou metodou pro méně rizikové pacienty zejména ve vyšších věkových kategoriích. V porovnání se SAVR je však zatížena vyšším rizikem iatrogenních převodních poruch. Tyto poruchy vznikají převážně periprocedurálně či v časném pooperačním období a část z nich je plně reverzibilní. Naše kazuistika popisuje případ pacientky po TAVI, u které byla dokumentována nově vzniklá převodní porucha včetně proběhlých synkop. Cílem této kazuistiky je poukázat na schopnost reparace převodního systému po jeho mechanické iritaci související s provedeným TAVI. ©
Transcatheter aortic valve replacement (TAVI) has become a suitable alternative to surgical aortic valve replacement (SAVR) for patients not accepted by surgeons, but also a preferred method for lower-risk pa- tients, especially in older age groups. However, compared to SAVR, it is burdened with a higher risk of iatrogenic heart rhythm disturbances. These disturbances occur mostly periprocedurally or in the early postoperative period and some of them are fully reversible. Our case report describes the case of a patient after TAVI in whom a new-onset cardiac rhythm disturbance including ongoing syncope was documented. The aim of this case report is to highlight the ability of the conduction system to repair after its mechanical irritation related to the performed TAVI.

The field of cardiac pacing has changed rapidly in the last several years. Since the initial description of His bundle pacing targeting the conduction system, recent advances in pacing the left bundle branch and its fascicles have evolved. The field and investigators' knowledge of conduction system pacing including relevant anatomy and physiology has advanced significantly. The aim of this review is to provide a comprehensive update on recent advances in conduction system pacing.

AIMS: The field of conduction system pacing (CSP) is evolving, and our aim was to obtain a contemporary picture of European CSP practice. METHODS AND RESULTS: A survey was devised by a European CSP Expert Group and sent electronically to cardiologists utilizing CSP. A total of 284 physicians were invited to contribute of which 171 physicians (60.2%; 85% electrophysiologists) responded. Most (77%) had experience with both His-bundle pacing (HBP) and left bundle branch area pacing (LBBAP). Pacing indications ranked highest for CSP were atrioventricular block (irrespective of left ventricular ejection fraction) and when coronary sinus lead implantation failed. For patients with left bundle branch block (LBBB) and heart failure (HF), conventional biventricular pacing remained first-line treatment. For most indications, operators preferred LBBAP over HBP as a first-line approach. When HBP was attempted as an initial approach, reasons reported for transitioning to utilizing LBBAP were: (i) high threshold (reported as >2 V at 1 ms), (ii) failure to reverse bundle branch block, or (iii) > 30 min attempting to implant at His-bundle sites. Backup right ventricular lead use for HBP was low (median 20%) and predominated in pace-and-ablate scenarios. Twelve-lead electrocardiogram assessment was deemed highly important during follow-up. This, coupled with limitations from current capture management algorithms, limits remote monitoring for CSP patients. CONCLUSIONS: This survey provides a snapshot of CSP implementation in Europe. Currently, CSP is predominantly used for bradycardia indications. For HF patients with LBBB, most operators reserve CSP for biventricular implant failures. Left bundle branch area pacing ostensibly has practical advantages over HBP and is therefore preferred by many operators. Practical limitations remain, and large randomized clinical trial data are currently lacking.

Úvod: Náhrada aortální chlopně chirurgickou metodou (transcatheter aortic valve replacement, TAVR) je stále prevalentnější metodou náhrady aortální chlopně, zejména u pacientů s vyšším operačním rizikem. I přes velké pokroky ve vývoji chlopní se však incidence poruch rytmu vyžadujících trvalou kardiostimulaci po TAVR výrazně nesnižuje. Existuje množství rizikových faktorů, pomocí nichž lze predikovat riziko nutnosti kardiostimulace po náhradě chlopně. Cíl: Přinést data o prevalenci komplikace v podobě nutnosti kardiostimulace u pacientů po TAVR z jednoho českého centra, kde probíhají implantace TAVR již déle než dekádu. Metodika: Byla analyzována skupina pacientů, kteří podstoupili TAVR v našem centru v období mezi srpnem 2009 a únorem 2021. Ze vzorku byli vyjmuti pacienti, kteří již mají naimplantovaný kardiostimulátor či defibrilátor. V souboru 717 pacientů průměrný věk dosahoval 78,8 roku, 52 % tvořily ženy, průměrný index tělesné hmotnosti (BMI) je 29,6. V tomto období byly implantovány chlopně od různých výrobců od chlopní první generace až po protézy novějších generací. Výsledky: Trvalá kardiostimulace byla nutná u 162 pacientů (22,6 %), kterým bylo implantováno srdeční implantabilní elektronické zařízení (cardiac implantable electronic device, CIED) v průměru 3,9 dnu od TAVR. Z naimplantovaných pacientů bylo signifikantně více mužů (56,2 %) v porovnání s pacienty bez CIED (45,4 %); p = 0,016. Rovněž byl zaznamenán významný rozdíl mezi výškou, hmotností a BSA mezi těmito skupinami. Pacienti s preexistující blokádou pravého Tawarova raménka (right bundle branch block, RBBB) byli četnější mezi pacienty vyžadujícími kardiostimulaci v porovnání s nestimulovaným souborem (p < 0,0001), tento parametr rovněž vyšel jako jediný nezávislý prediktor implantace CIED po TAVR (poměr šancí [OR] 3,8, 95% interval spolehlivosti [CI] 2,0-7,0; p < 0,0001). Jednotlivé typy protéz se výrazně lišily v procentuálním zastoupení pacientů s nutností kardiostimulace. Nebyl nalezen signifikantní rozdíl mezi jednotlivými velikostmi protéz, stejně jako u pacientů s predilatací či postdilatací, v zastoupení pacientů s CIED a bez nich. Závěr: Naše data jsou ve shodě s jinými studiemi zabývajícími se tímto tématem a podporují přítomnost RBBB jako jeden z rizikových faktorů pro nutnost kardiostimulace po TAVR. Mezi stimulovanými pacienty bylo rovněž četnější mužské pohlaví a pacienti s větším tělesným povrchem (BSA). Náš vzorek je však nehomogenní a další subanalýzy v rámci jednotlivých designů chlopní ve větším zastoupení by byly potřebné k vyhodnocení dalších rizikových faktorů.
Introduction: Transcatheter aortic valve replacement (TAVR) is becoming more prevalent option of aortic valve replacement, predominantly amongst patients with a higher surgical risk. Despite a rapid development of TAVR prostheses, incidence of conduction abnormalities requiring cardiac pacing as a result of TAVR is not significantly decreasing. There are numerous risk factors which can help to predict the risk for cardiac pacing after TAVR. Aim: To present our data of a prevalence of this complication from one Czech center where TAVR prostheses have been implanted for over a decade. Methods: A group of patients who underwent TAVR between August 2009 and February 2021 in our center were analyzed. Patients with already implanted pacemaker or defi brillator were excluded from this group, leaving 717 patients for further analysis. The mean age was 78.8 years, 52% were females, the mean BMI was 29.6. Various different prostheses of different manufacturers and generations were implanted. Results: Permanent pacing was required in 162 patients (22.6%), in whom CIED (cardiac implantable electronic device) was implanted in average 3.9 days from TAVR. There was a signifi cantly higher amount of men in the paced group (56.2%) compared to the group without CIED (45.4%); p = 0.016. Moreover, there was a signifi cant difference between height, weight, and BSA among these groups. Patients with preexisting right bundle branch block (RBBB) were more prevalent in the paced group compared to non-paced group (p <0.0001); this parameter was the only independent predictor of the implantation of CIED after TAVR (odds ratio 3.8, 95% CI 2.0–7.0; p <0.0001). There were marked differences between the amounts of implanted prostheses of various designs. No signifi cant difference was found in the size of prostheses, predilatation or postdilatation between the two groups of patients. Conclusion: Our data are in concordance with other published studies, supporting the RBBB being one of the risk factors for pacing after TAVR. Males and patients with higher BSA were more prevalent in the paced group. Our sample is, however, rather inhomogeneous and further analysis of different designs in higher numbers would be necessary to identify other risk factors of this prevalent complication

V posledních letech se do klinické praxe dostávají nové metody trvalé kardiostimulace, které se v literatuře nazývají fyziologickou stimulací či stimulací Hisova-Purkyňova převodního systému. Patří mezi ně stimulace Hisova svazku, přímá stimulace levého Tawarova raménka a stimulace myokardu levokomorového septa. Tyto stimulační techniky jsou spojeny se změnou přístupu k trvalé kardiostimulaci, jak ji známe v posledních desetiletích. Na rozdíl od myokardiální stimulace pravé komory srdeční je u nich stimulační elektroda fixována přímo do oblasti výskytu buněk převodního systému (Hisova svazku či levého Tawarova raménka), či do jejich těsné blízkosti. Dalším významným rozdílem je nutnost použití speciálního vybavení k dosažení optimálního místa stimulace a obvykle i využití speciálních manévrů k potvrzení uchvácení buněk převodního systému. Narůstající evidence o bezpečnosti a účelnosti těchto metod se odrazila v doporučených postupech některých národních společností pro kardiostimulaci. Fyziologická stimulace (především stimulace Hisova svazku) je ve specifických indikacích doporučována jako alternativa pro pravokomorovou nebo biventrikulární stimulaci. Tento přehledový článek popisuje jednotlivé typy fyziologické stimulace, techniku jejich provádění, způsoby potvrzení typu komorové aktivace a výhody či nevýhody jejich použití u pacientů s indikací k implantaci kardiostimulátoru pro bradykardii.
New methods of cardiac pacing are being adopted in clinical practice in recent years. They are called physiological pacing or conduction system pacing and include His bundle pacing, direct left bundle branch pacing, and left ventricular septal pacing. These methods differ significantly from a standard pacemaker implant procedure. The lead is fixed directly into or adjacent to the tissue of the conduction system of the heart as opposed to lead fixation in myocardial tissue of the right ventricle. Specialized tools and maneuvers for confirming conduction system capture are necessary during the implant procedure. Growing evidence on the safety and feasibility of these methods is reflected in latest guidelines on cardiac pacing of several national societies. They recommend physiological pacing (mainly His bundle pacing) in some specific indications as an alternative to right ventricular myocardial or biventricular pacing. This review describes types of physiological pacing, implant technique, methods of confirming conduction system capture, and its advantages and disadvantages in patients with bradycardia and indication for pacemaker implantation.

BACKGROUND: Cardiac resynchronization therapy (CRT) using biventricular pacing (BVP) is effective in patients with heart failure, left bundle branch block (LBBB), and reduced left ventricular function. Left bundle branch area pacing (LBBAP) has been reported as an alternative option for CRT. OBJECTIVE: The purpose of this study was to assess the feasibility and outcomes of LBBAP in patients who failed conventional BVP because of coronary venous (CV) lead complications or who were nonresponders to BVP. METHODS: At 16 international centers, LBBAP was attempted in patients with conventional CRT indication who failed BVP because of CV lead complications or lack of therapeutic response to BVP. Heart failure hospitalization (HFH) and death, echocardiographic outcomes, procedural data, pacing parameters, and lead complications including CV lead failure are reported. RESULTS: LBBAP was successfully performed in 200 patients (CV lead failures 156; nonresponders 44) (age 68 ± 11 years; female 35%; LBBB 55%; right ventricular pacing 23%; ischemic cardiomyopathy 28%; nonischemic cardiomyopathy 63%; left ventricular ejection fraction [LVEF] ≤35% in 80%). Procedural duration was 119.5 ± 59.6 minutes, and fluoroscopy duration was 25.7 ± 18.5 minutes. LBBAP threshold and R-wave amplitudes were 0.68 ± 0.35 V @ 0.45 ms and 10.4 ± 5 mV at implant, respectively, and remained stable during mean follow-up of 12 ± 10.1 months. LBBAP resulted in significant QRS narrowing from 170 ± 28 ms to 139 ± 25 ms (P <.001) with V6 R-wave peak times of 85 ± 17 ms. LVEF improved from 29% ± 10% at baseline to 40% ± 12% (P <.001) during follow-up. The risk of death or HFH was lower in those with CV lead failure than in nonresponders (hazard ratio 0.357; 95% confidence interval 0.168-0.756; P = .007) CONCLUSION: LBBAP is a viable alternative to CRT in patients who failed conventional BVP due to CV lead failure or who were nonresponders.

AIMS: In arrhythmogenic cardiomyopathy (ACM), sustained ventricular tachycardia (VT) typically displays a left bundle branch block (LBBB) morphology while a right bundle branch block (RBBB) morphology is rare. The present study assesses the VT morphology in ACM patients with sustained VT and their clinical and genetic characteristics. METHODS AND RESULTS: Twenty-six centres from 11 European countries provided information on 954 ACM patients who had ≥1 episode of sustained VT spontaneously documented during patients' clinical course. Arrhythmogenic cardiomyopathy was defined according to the 2010 Task Force Criteria, and VT morphology according to the QRS pattern in V1. Overall, 882 (92.5%) patients displayed LBBB-VT alone and 72 (7.5%) RBBB-VT [alone in 42 (4.4%) or in combination with LBBB-VT in 30 (3.1%)]. Male sex prevalence was 79.3%, 88.1%, and 56.7% in the LBBB-VT, RBBB-VT, and LBBB + RBBB-VT groups, respectively (P = 0.007). First RBBB-VT occurred 5 years after the first LBBB-VT (46.5 ± 14.4 vs 41.1 ± 15.8 years, P = 0.011). An implanted cardioverter-defibrillator was more frequently implanted in the RBBB-VT (92.9%) and the LBBB + RBBB-VT groups (90%) than in the LBBB-VT group (68.1%) (P < 0.001). Mutations in PKP2 predominated in the LBBB-VT (65.2%) and the LBBB + RBBB-VT (41.7%) groups while DSP mutations predominated in the RBBB-VT group (45.5%). By multivariable analysis, female sex was associated with LBBB + RBBB-VT (P = 0.011) while DSP mutations were associated with RBBB-VT (P < 0.001). After a median follow-up of 103 (51-185) months, death occurred in 106 (11.1%) patients with no intergroup difference (P = 0.176). CONCLUSION: RBBB-VT accounts for a significant proportion of sustained VTs in ACM. Sex and type of pathogenic mutations were associated with VT type, female sex with LBBB + RBBB-VT, and DSP mutation with RBBB-VT.
