AIMS: Data on the hybrid atrial fibrillation (AF) treatment are lacking in patients with structural heart disease undergoing concomitant CryoMaze procedures. The aim was to assess whether the timely pre-emptive catheter ablation would achieve higher freedom from AF or atrial tachycardia (AT) and be associated with better clinical outcomes than surgical ablation alone. METHODS AND RESULTS: The trial investigated patients with non-paroxysmal AF undergoing coronary artery bypass grafting and/or valve repair/replacement with mandatory concomitant CryoMaze procedure who were randomly assigned to undergo either radiofrequency catheter ablation [Hybrid Group (HG)] or no further treatment (Surgery Group). The primary efficacy endpoint was the first recurrence of AF/AT without class I or III antiarrhythmic drugs as assessed by implantable cardiac monitors. The primary clinical endpoint was a composite of hospitalization for arrhythmia recurrence, worsening of heart failure, cardioembolic event, or major bleeding. We analysed 113 and 116 patients in the Hybrid and Surgery Groups, respectively, with a median follow-up of 715 (IQR: 528-1072) days. The primary efficacy endpoint was significantly reduced in the HG [41.1% vs. 67.4%, hazard ratio (HR) = 0.38, 95% confidence interval (CI): 0.26-0.57, P < 0.001] as well as the primary clinical endpoint (19.9% vs. 40.1%, HR = 0.51, 95% CI: 0.29-0.86, P = 0.012). The trial groups did not differ in all-cause mortality (10.6% vs. 8.6%, HR = 1.17, 95%CI: 0.51-2.71, P = 0.71). The major complications of catheter ablation were infrequent (1.9%). CONCLUSION: Pre-emptively performed catheter ablation after the CryoMaze procedure was safe and associated with higher freedom from AF/AT and improved clinical outcomes.

AIMS: Interventional cardiology procedures may expose patients and staff to considerable radiation doses. We aimed to assess whether exposure to ionizing radiation during catheter ablation of supraventricular tachycardia (SVT) can be completely avoided. METHODS AND RESULTS: In this prospective randomized study, patients with SVT (atrioventricular re-entrant tachycardia n = 94, typical atrial flutter n = 29) were randomly assigned in a 1:1 ratio to catheter ablation with conventional fluoroscopic guidance (CF group) or with the EnSite Precision mapping system [zerofluoro (ZF) group]. Acute procedural parameters, increased stochastic risk of cancer incidence and 6-month follow-up data were assessed. Between May 2019 and August 2020, 123 patients were enrolled. Clinical parameters were comparable. Median procedural time was 60.0 and 58.0 min, median fluoroscopy time and estimated median effective dose were 240 s vs. 0 and 0.38 mSv vs. 0 and arrhythmia recurrence was 5% and 7.9% in the CF and ZF groups, respectively. The acute success rate was 98.4% in both groups. No procedure-related complications were reported. At an average age of 55.5 years and median radiation exposure of 0.38 mSv, the estimate of increased incidence was approximately 1 in 14 084. The estimated mortality rate was 1 per 17 857 exposed persons. CONCLUSIONS: The procedural safety and efficacy of the zero-fluoroscopic approach are similar to those of conventional fluoroscopy-based ablation for atrioventricular nodal re-entrant tachycardia and atrial flutter. Under the assumption of low radiation dose, the excessive lifetime risk of malignancy in the CF group due to electrophysiology procedure is reasonably small, whilst totally reduced in zero fluoroscopy procedures.

OBJECTIVES: This study sought to comprehensively determine the procedural safety and midterm efficacy of hybrid ablations. BACKGROUND: Hybrid ablation of atrial fibrillation (AF) (thoracoscopic ablation followed by catheter ablation) has been used for patients with nonparoxysmal AF; however, accurate data regarding efficacy and safety are still limited. METHODS: Patients with nonparoxysmal AF underwent thoracoscopic, off-pump ablation using the COBRA Fusion radiofrequency system (Estech) followed by a catheter ablation 3 months afterward. The safety of the procedure was assessed using sequential brain magnetic resonance and neuropsychological examinations at baseline (1 day before), postoperatively (2-4 days for brain magnetic resonance imaging or 1 month for neuropsychological examination), and at 9 months after the surgical procedure. Implantable loop recorders were used to detect arrhythmia recurrence. Arrhythmia-free survival (the primary efficacy endpoint) was defined as no episodes of AF or atrial tachycardia while off antiarrhythmic drugs, redo ablations or cardioversions. RESULTS: Fifty-nine patients (age: 62.5 ± 10.5 years) were enrolled, 37 (62.7%) were men, and the mean follow-up was 30.3 ± 10.8 months. Thoracoscopic ablation was successfully performed in 55 (93.2%) patients. On baseline magnetic resonance imaging, chronic ischemic brain lesions were present in 60.0% of patients. New ischemic lesions on postoperative magnetic resonance imaging were present in 44.4%. Major postoperative cognitive dysfunction was present in 27.0% and 17.6% at 1 and 9 months postoperatively, respectively. The probability of arrhythmia-free survival was 54.0% (95% CI: 41.3-66.8) at 1 year and 43.8% (95% CI: 30.7-57.0) at 2 years. CONCLUSIONS: The thoracoscopic ablation is associated with a high risk of silent cerebral ischemia. The midterm efficacy of hybrid ablations is moderate.

Cíl: Cílem sdělení je prezentace dosavadních výsledků a zkušeností s léčbou supraventrikulárních tachyarytmií katetrizační radiofrekvenční ablací (KA). Pacienti a metodika: Do souboru byli zařazeni všichni nemocní (n = 1 485), u kterých jsme v letech 1996–2009 provedli celkem 1 627 KA pro supraventrikulární tachyarytmii. Jednalo se o nemocné s paroxyzmální atrioventrikulární nodální reentry tachykardií (AVNRT): n = 772, 484 žen, průměrný věk 50,3 ± 16,4 let; atrioventrikulární reentry tachykardií na podkladě přítomnosti akcesorní síňokomorové spojky (AVRT): n = 312, 145 žen, průměrný věk 40,1 ± 14,9 let; flutterem síní I. typu (FlS): n = 391, 96 žen, průměrný věk 61,6 ± 11 let; a se síňovou tachykardií (ST): n = 64, 35 žen, makroreentry ST v 31 případech, ektopická ST v 33 případech, průměrný věk 53,6 ± 14,2 let. U 54 nemocných byla provedena KA u více než jednoho typu tachyarytmie. Výsledky: Bezprostřední úspěšnost katetrizační ablace byla 98,7 % u AVNRT, 94,6 % u AVRT, 97,7 % u FlS a 81,3 % u ST. U 22 nemocných (1,4 %) došlo v souvislosti s výkonem ke vzniku závažné komplikace. Počet reablací pro recidivu arytmie se pohyboval mezi 1,8 a 12,5 %. V 88 případech byl proveden opakovaný výkon, po kterém byla dosažena celková úspěšnost mezi 89 a 99 % v závislosti na typu tachyarytmie. V průběhu dlouhodobého ambulantního sledování po KA (1 014 nemocných; průměrná délka sledování 73 ± 38 měsíců) zemřelo 23 nemocných, nejčastější příčinou úmrtí bylo maligní onemocnění (v 9 případech). Závěr: Naše dlouhodobé zkušenosti s katetrizační radiofrekvenční ablací v léčbě nemocných se supraventrikulárními tachyarytmiemi potvrdily vysokou úspěšnost a bezpečnost této terapeutické metody.
Aim: The aim of the article is presentation of our results and experiences with radiofrequency catheter ablation (CA) in the therapy of supraventricular tachyarrhythmias. Patients and methods: From 1996 to 2009, 1 485 patients underwent CA (total procedure number – 1 627). The group consist of 772 patients with paroxysmal atrioventricular nodal reentry tachycardia (AVNRT): 484 women, mean age 50.3 ± 16.4 years; 312 patients with atrioventricular reentry tachycardia (AVRT): 145 women, mean age 40.1 ± 14.9 years; 391 patients with typical atrial flutter (AF): 96 women, mean age 61.6 ± 11 years; and 64 patients with atrial tachycardia (AT): 35 women, mean age 53.6 ± 14.2 years, focal AT 33, macroreentrant AT 31. CA was performed for more than one type of arrhythmia in 54 patients. Results: Acute ablation success was achieved in 98.7% of patients with AVNRT, 94.6% of patients with AVRT, 97.7% of patients with AF, and 81.3% of patients with AT. Serious procedure complications occurred in 22 patients (1.4%). The recurrence rate was 1.8–12.5%. 88 patients underwent successful reablation procedure. Long-term ablation success was achieved in 89–99 % of patients depending on the different type of arrhythmia. During the long-term follow-up (mean 73 ± 38 months) died 23 patients, the most common cause of death was malignancy (9 patients). Conclusion: Our long-term experience and good results documented high success rate and safety of radiofrequency catheter ablation in the therapy of patients with supraventricular tachyarrhythmias.

The supraventricular arrhythmias include a wide spectrum of disorders including, in descending order of frequency, atrial fibrillation, atrial flutter, atrioventricular nodal reentry, Wolff-Parkinson-White syndrome, and atrial tachycardia. While not life-threatening in most cases, they may cause important symptoms, such as palpitations, chest discomfort, breathlessness, anxiety, and syncope, which significantly impair quality of life. Medical therapy has variable efficacy, and most patients are not rendered free of symptoms. Research over the past several decades has revealed fundamental mechanisms involved in the initiation and maintenance of all of these arrhythmias. Knowledge of mechanisms has in turn led to highly effective surgical and catheter-based treatments. Atrial fibrillation remains a therapeutic challenge but is also yielding to investigation and therapeutic efforts. The supraventricular arrhythmias and their treatment are described in this report.
