Cíl práce: Cílem retrospektivní studie bylo porovnání vybraných dentálních a skeletálních změn u paci:ientů s distookluzí léčených pomocí snímatelného funkčního aparátu Twin Block. Porovnávány byly parametry získané hodnocením kefalometrických rentgenových snímků před zahájením a po šesti měsících léčby aparátem Twin Block. Materiál a metodika: Do studie bylo zařazeno celkem 30 rostoucích pacientů (19 chlapců a 11 dívek) s diagnózou distookluze s protruzí, léčených funkčním aparátem Twin Block na Ortodontickém oddělení Stomatologické kliniky dětí a dospělých 2. LF UKaFNv Motole. U všech pacientů zařazených do studie byly hodnoceny kefalometrické rentgenové snímky zhotovené před zahájením léčby a po 6 měsících léčby funkčním aparátem Twin Block. Bylo hodnoceno celkem 17 parametrů (skeletálních i dentálních). Výsledky: Bylo zjištěno, že došlo ke dvěma statisticky signifikantním změnám dentálních parametrů - zmenšení vzdálenosti U1-APo a zvětšení vzdálenosti L1-APo. Změny u ostatních měřených skeletálních i dentálních parametrů nebyly statisticky signifikantní nebo jejich hodnoty nepřesahovaly chybu měření.
Aims: The retrospective study aimed to compare selected dental and skeletal changes In patients with distoocclusion treated with the removable appliance Twin Block. We compared the parameters that were evaluated in cephalograms made before and after the 6-month treatment with Twin Block appliance. Material and methods: The study included 30 growing patients (19 males, 11 females) with distoocclusion accompanied by protrusion, treated with the removable appliance Twin Block at the Department of Orthodontics. Clinic of Stomatology for Children and Adults In Prague-Motol. The patients' cephalograms taken before treatment and after the 6-month therapy with Twin Block were evaluated. 17 parameters (skeletal and dental) were examined. Results: We recorded two statistically significant modifications of dental parameters - decrease of the distance U1-APo and increase of the distance L1-APo. Other changes of skeletal and dental parameters were not statistically significant, or their values did not exceed the error of measurement.

TWIN BLOCK patří do skupiny funkčních aparátů, indikován pro druhou třídu skeletální. Cílem přednášky je seznámit techniky s doposud u nás v republice neznámým typem aparátu a popsat jeho pracovní laboratorní postup. TWIN BLOCK, jak uz sann název zapadajících bloků v laterárních úsecích na horní i dolní desce. Sešikmením bloků-valů dochází k předsunutí dolní čelisti.Valy mohou být jen pryskyřičné nebo doplněné speciálním šroubem, který slouží k pozvolnějšímu předsouvání dolní čelisti, mandibuly. Přednáška bude doplněna fotografickými ukázkami z pracovní stáže v ortodontické laboratoři v Irsku, která se na tuto technologii specializuje.
Cílem sdělení je seznámit ortodontické laboranty s prvními klinickými výsledkv u oacientů léčenýchTwin Blockem u nás. Jedná se O funkční aparát používaný k léčbě II tř.A/1 odd. nebo 2 odd. Oblíbený je především ve Velké Británii a Irsku. Hlavní výhodou tohoto funkčního aparátu je oddělení horní a dolní base, což umožňuje pacientovi normální komunikaci, ale přitom postranní nákusné valy navádějí neustále dolní čelist anteriorně. Nasazeno bylo 46 TB. Výsledek byl posuzován do 1/2 roku od nasazení. První klinické výsledky v běžné praxi poukazují na dobrou akceptovatelnost tohoto aparátu pacienty a tím i k dosažení slušných léčebných výsledků.

Aim: The generation of vertical forces from fixed orthodontic appliances is important in incisor intrusion, as well as in the creation of anti-tipping moments in connection with space closure. The unique wing slot of the double bracket system (Cannon Ultra*) facilitates the use of a wire with "V" bend in the premolar region and ligated loosely in the incisor region, a so called "free end" force system. The aim of this study was to investigate the biomechanical characteristics of a standard "V" bend wire configuration in a "free end" system, i.e. with one end mounted in an orthodontic tube and Ithe other loosely ligated to a bracket, as well as the biomechanical effects of alteration of "V" bend position and size, wire dimension, method of ligation and the inclusion of a second wire in the system. Material and method: Samples of the standard steel wires (0.016" and 0.018") were mounted in a bench testing machine measuring moments and forces at both ends in a standardised configuration resembling a two premolar extraction clinical case treated by means of the Cannon Ultra • system. The position of the "V" bend (25deg) was originally immediately between the bracket of the premolar and canine and subsequently altered in both directions in 1.0mm intervals. Results: Moments created at the posterior end (1598.80cNmm) resulted in intrusive forces anteriorly (191.30cNmm), which in turn induced moments on the anterior block (1532.80cNmm). The size of the moments and forces was increased in connection with posterior positioning of the "V" bend, increasing wire dimension and "V" bend size, though reduced and stabilised by the inclusion of second, nickel-titanium wire. The double wire system created moments and forces of a size which were felt to be clinically relevant. Conclusion: The "V" bend in a "free end" system based on the Cannon Ultra* double slot bracket can create variable moments and forces which can be used clinically in intrusion and space closing procedures but should be measured and monitored dosely.
BACKGROUND AND AIMS: Right bundle branch block (RBBB) and resulting right ventricular (RV) electromechanical discoordination are thought to play a role in the disease process of subpulmonary RV dysfunction that frequently occur post-repair tetralogy of Fallot (ToF). We sought to describe this disease entity, the role of pulmonary re-valvulation, and the potential added value of RV cardiac resynchronization therapy (RV-CRT). METHODS: Two patients with repaired ToF, complete RBBB, pulmonary regurgitation, and significantly decreased RV function underwent echocardiography, cardiac magnetic resonance, and an invasive study to evaluate the potential for RV-CRT as part of the management strategy. The data were used to personalize the CircAdapt model of the human heart and circulation. Resulting Digital Twins were analysed to quantify the relative effects of RV pressure and volume overload and to predict the effect of RV-CRT. RESULTS: Echocardiography showed components of a classic RV dyssynchrony pattern which could be reversed by RV-CRT during invasive study and resulted in acute improvement in RV systolic function. The Digital Twins confirmed a contribution of electromechanical RV dyssynchrony to RV dysfunction and suggested improvement of RV contraction efficiency after RV-CRT. The one patient who underwent successful permanent RV-CRT as part of the pulmonary re-valvulation procedure carried improvements that were in line with the predictions based on his Digital Twin. CONCLUSION: An integrative diagnostic approach to RV dysfunction, including the construction of Digital Twins may help to identify candidates for RV-CRT as part of the lifetime management of ToF and similar congenital heart lesions.

Současné důkazy svědčí o tom, že kombinace profylaktického podání vazopresorů s aplikací HES před anestezií („preloading") či při zahájení anestezie („coloading") nebo s aplikací krystaloidů při zahájení anestezie („coloading") je nejlepší metodou prevence hypotenze u rodičky po zahájení subarachnoidální anestezie. Podání krystaloidů před anestezií („preloading") je klinicky neúčinné a nemělo by se již používat.
