Aim:To address the wishes of patients with cardiovascular disease in relation to their end-of-life stage –an issue that has long been regarded taboo. Design:Cross-sectional study.Methods:After an initial interview, 127 patients completed an anonymous questionnaire:86 patients (67.7%) hospitalized in a cardiology department, and 41 patients (32.3%) on follow-up at an outpatient clinic. The mean age was 67 ± 13 years; 60.6% were male; 51.7% were NYHA class III. Seventeen patients (13.4%) had an implantable cardioverter-defibrillator. Results:The patients most concerned about their future were those aged 65–79 years (p = 0.044); patients who felt they were not receiving sufficient information (p = 0.008); hospitalized patients (p<0.001); or those with a known diagnosis of less than one month, or over one year (p = 0.038). Seventy-one patients (57.7%) would prefer to die at home, although this would mean receiving less health care than in hospital. Only 19 patients (15.4%) expressed a preference for extension of life over quality of life. Twenty-nine patients (23.6%) would contemplate having their cardioverter-defibrillator deactivated. 90.2% subjects considered it important forphysicians to be interested in the views of patients regarding these issues. Conclusion:Despite the contrast with practice in some countries, most patients with cardiovascular disease would appreciate physicians being interested in their attitudes to the issue of end of life. An adequate level of information would help reduce patient concerns about their futures.

Úvod: Incidence akutního krvácení do horní části gastrointestinálního traktu (GIT) se udává 85–108/100 000 obyvatel za rok, přičemž nonvariceální krvácení odpovídá za 80–90 %. Antiagregační a antikoagulační terapie patří mezi významné rizikové faktory pro krvácení do horní části GIT. Cíl práce: Zjistit výskyt krvácení do horní části GIT u běžné populace pacientů v podmínkách krajské nemocnice, srovnání podílu antiagregancií, antikoagulancií a dalších rizikových léků na dané problematice. Soubor a metodika: Retrospektivní analýza pacientů, kteří v roce 2013 (leden–červen) podstoupili za hospitalizace gastroskopii pro akutní krvácení do horní části GIT nebo pro anémii (Hb < 100 g/l) s verifikovaným zdrojem ztrát v horní části GIT. Výsledky: Soubor tvořilo 111 pacientů s průměrným věkem 69 ? 15 let, mužů bylo 60 %. Nonvariceální krvácení tvořilo 90 %. Žádný z pacientů s variceálním krvácením (10 % pacientů) neužíval antiagregační či antikoagulační terapii. Pacientů s nonvariceálním krvácením do horní části GIT bylo 100 s průměrným věkem 70 ? 15 let, 61 % mužů. Příznaky akutního krvácení (hemateméza, meléna) mělo 73 % těchto pacientů. Nejčastější příčinou krvácení byla vředová choroba gastroduodena, a to v 54 % případů. Ze všech pacientů s nonvariceálním krvácením do GIT užívalo antiagregancia 32 %, antikoagulancia 19 %, dalších 10 % pacientů užívalo nesteroidní antirevmatika, selektivní inhibitory zpětného vychytávání serotoninu nebo kortikoidy. 30denní mortalita pacientů s nonvariceálním krvácením byla 11 % a roční mortalita 23 %. Nebyl zjištěn významný rozdíl v mortalitě, nutnosti aplikace krevních transfuzí nebo chirurgické intervence u pacientů na antitrombotické terapii a bez ní. 25 % (8 pacientů) s terapií kyselinou acetylsalicylovou (ASA) nebylo k této léčbě indikováno dle guidelines. Závěr: V souboru pacientů ošetřených gastroskopií pro symptomatické nonvariceální krvácení do GIT nebo anémií (Hb < 100 g/l) je signifikantně větší podíl léčených antiagregancii než antikoagulancii. To může reflektovat větší zastoupení této léčby v populaci, ale také její vyšší riziko pro vznik GIT krvácení. V souvislosti s tím vyznívá varovně fakt, že stále existuje nezanedbatelné procento pacientů, kteří užívají kyselinu acetylsalicylovou i přesto, že nesplňují indikaci k této terapii.
Background: The incidence of acute upper gastrointestinal bleeding is about 85–108/100,000 inhabitants per year, nonvariceal bleeding accounts for 80–90 %. Antiplatelet and anticoagulation treatment are the significant risk factors for upper gastrointestinal bleeding. Objectives: To evaluate the occurrence of upper gastrointestinal bleeding in the general community of patients in a county hospital. And to compare the role played by antiplatelet and anticoagulation drugs and other risk medication. Design and methods: Retrospective analysis of patients over 18 years of age who underwent endoscopy for acute upper gastrointestinal bleeding or anaemia (haemoglobin < 100 g/l) with proved source of blood losses in upper gastrointestinal tract during a hospital stay in 2013 (from January to June). Results: We included 111 patients of average age 69 ? 15 years, men 60 %. Nonvariceal bleeding accounted for 90 % of the cases. None of the patients with variceal bleeding (10 % of patients) took antiplatelet or anticoagulation therapy. There were 100 patients with nonvariceal bleeding of average age 70 ? 15, 61 % men. With the symptoms of acute bleeding (hematemesis, melena) presented in 73 % of patients. The most frequent cause of bleeding was gastric and duodenal ulcer (54 %). 32 % of patients with nonvariceal bleeding had antiplatelets, 19 % anticoagulants and 10 % used nonsteroidal anti-inflammatory drugs, selective serotonin reuptake inhibitors or corticosteroids. 30-days mortality of patients with nonvariceal bleeding was 11 %, annual mortality was 23 %. There was no significant difference in mortality, blood transfusion requirements or surgical intervention between the patients with antithrombotic agents and without them. 25 % of patients (8 patients) using acetylsalicylic acid did not fulfil the indication for this treatment. Conclusion: Among the patients examined by endoscopy for symptomatic nonvariceal bleeding and/or anaemia (haemoglobin < 100 g/l) significantly higher portions of patients are taking antiplatelet rather than anticoagulation therapy. This may be caused by greater use of these drugs in the population, but on the other hand it may reflect an association with greater risk of gastrointestinal bleeding. With regard to that, it is alarming, that there still exists a nonnegligible percentage of patients taking acetylsalicylic acid even though they do not meet the indication for the prescription according to the guidelines.

Úvod: Těžká sepse je stále spojena s významnou morbiditou a mortalitou, která se nicméně liší, stejně jako její management, v závislosti na regionu. Jaká je situace v ČR a jaký mají charakter pacienti s těžkou sepsí v současné době, není známo. Cílem projektu je popsat procesy péče, výsledek léčby a charakteristiku pacientů s těžkou sepsí přijímaných na pracoviště intenzivní péče v ČR. Metodika: Jedná se o multicentrický a observační projekt s retrospektivním zařazováním pacientů, kteří splní kritéria těžké sepse před nebo do 24 hod od přijetí na vybrané jednotky intenzivní péče (EPOSS‑ICU). Výsledky: Analyzováno bylo 394 pacientů. Medián věku při přijetí byl 66 (56–76) let, převažovali muži (58,9 %) a medián APACHE II skóre při příjmu byl 25 (19–32). Příjímaní pacienti byli převážně interního charakteru (56,9 %) a nejvíce příjmů bylo sekundárních z jiných ICU (53,6 %). Splnění kritérií těžké sepse bylo nejčastěji v období (? 4 hod) od příjmu na EPOSS‑ICU (77,6 %). Medián celkového příjmu tekutin během prvních 24 hod byl 6 680 (4 840–9 450) ml. Většina nemocných vyžadovala umělou plicní ventilaci (58,4 %). Splnění tzv. resuscitačních balíčků pro těžkou sepsi bylo v našem souboru velmi dobré a bylo spojeno s nižší mortalitou pacientů. Nejčastěji byla délka pobytu na EPOSS‑ICU 7 (3–15) dnů a medián celkové doby hospitalizace v nemocnici byl 13 (8–28) dnů. Celková nemocniční mortalita souboru byla 35,8 %. Závěr: Představujeme projekt, který ve své první fázi získal cenná a mezinárodně srovnatelná data o pacientech s těžkou sepsí přijímaných na zapojená pracoviště intenzivní péče v ČR.
Introduction: Severe sepsis is still associated with significant morbidity and mortality, which is however different, as well as its management, depending on the region. What is the situation in the Czech Republic and what is the character of patients with severe sepsis is currently not known. The aim of the project is to describe the processes of care, outcome and characteristics of patients with severe sepsis admitted to the intensive care department of the Czech Republic. Methods: This is a multicentre and observational project with retrospective enrollment of patients who meet the criteria for severe sepsis before or within 24 hours after admission to selected intensive care units (ICU‑EPOSS). Results: 394 patients were analyzed. Median age at admission was 66 (56–76) years, males predominated (58.9%) and the median APACHE II score on admission was 25 (19–32). Patients were predominantly medical (56.9%) and most were secondary admitted from other ICU (53.6%). Meeting the criteria of severe sepsis was most frequently within the period (? 4 hours) of admission the EPOSS‑ICU (77.6%). Median total fluid intake during the first 24 hours was 6,680 (4,840–9,450) ml. Most patients required mechanical ventilation (58.4%). Compliance with the resuscitation bundle of severe sepsis in our group was very good and was associated with lower mortality of patients. Most frequently, the EPOSS‑ICU length of stay (LOS) was 7 (3–15) days and median hospital LOS was 13 (8–28) days. Hospital mortality in our cohort was 35.8%. Conclusion: Introducing the project, which in its first stage obtained valuable and internationally comparable data about patients with severe sepsis admitted to the involved ICU in the Czech Republic.
