INTRODUCTION: Aging negatively influences the structure of the human brain including the white matter. The objective of our study was to identify, using fixel-based morphometry, the age induced changes in the pathways connecting several regions of the central auditory system (inferior colliculus, Heschl's gyrus, planum temporale) and the pathways connecting these structures with parts of the limbic system (anterior insula, hippocampus and amygdala). In addition, we were interested in the extent to which the integrity of these pathways is influenced by hearing loss and tinnitus. METHODS: Tractographic data were acquired using a 3 T MRI in 79 volunteers. The participants were categorized into multiple groups in accordance with their age, auditory thresholds and tinnitus status. Fixel-based analysis was utilized to identify alterations in the subsequent three parameters: logarithm of fiber cross-section, fiber density, fiber density and cross-section. Two modes of analysis were used: whole brain analysis and targeted analysis using fixel mask, corresponding to the pathways connecting the aforementioned structures. RESULTS: A significantly negative effect of aging was present for all fixel-based metrics, namely the logarithm of the fiber cross-section, (7 % fixels in whole-brain, 14% fixels in fixel mask), fiber density (5 % fixels in whole-brain, 15% fixels in fixel mask), fiber density and cross section (7 % fixels in whole-brain, 19% fixels in fixel mask). Expressed age-related losses, exceeding 30% fixels, were particularly present in pathways connecting the auditory structures with limbic structures. The effect of hearing loss and/or tinnitus did not reach significance. CONCLUSIONS: Our results show that although an age-related reduction of fibers is present in pathways connecting several auditory regions, the connections of these structures with limbic structures are even more reduced. To what extent this fact influences the symptoms of presbycusis, such as decreased speech comprehension, especially in noise conditions, remains to be elucidated.

OBJECTIVE: Vestibular schwannoma surgery leads to acute unilateral vestibular loss. In some patients, however, the process of post-operatively initiated central compensation proceeds more rapidly than in others. This study aimed to evaluate post-operative vestibular function and correlate it with morphological findings of MRI scans. METHODS: The study included 29 patients who underwent surgery for vestibular schwannoma. Vestibular function was analysed post-operatively by video head impulse test (vHIT). Subjective symptoms were evaluated using validated questionnaires. All patients underwent MRI 3 months post-operatively, and the presence of the facial and vestibulocochlear nerves in the internal auditory canal was assessed. RESULTS: The vestibulo-ocular reflex gain measured by the vHIT correlated positively with audiological findings. Subjective perception of vestibular disorder did not correlate with objectively measured vestibular impairment or with MRI findings. CONCLUSIONS: After the resection of vestibular schwannoma, some patients may still have preserved vestibular function as measured by vHIT. The preserved function does not correlate with subjective symptoms. Patients with partially deteriorated vestibular function showed lower sensitivity to combined stimuli.

Cílem práce je podat přehled o současných možnostech vestibulární rehabilitace u pacientů po operaci vestibulárního schwannomu. Resekce nádoru vede obvykle ke vzniku jednostranné akutní periferní či kombinované vestibulární léze způsobené přerušením větví vestibulárního nervu a případně i poškozením mozečku. Léze se projevuje u pacientů poruchami stability, závratí, oscilopsií a vegetativním doprovodem. Lidský organizmus na tuto situaci reaguje zahájením mechanizmu centrální kompenzace, v níž hraje zásadní úlohu mozeček. Cílem vestibulární rehabilitace je tyto mechanizmy posílit a rekonvalescenci pacientů nejen zrychlit, ale zároveň dosáhnout i lepších funkčních výsledků, neboť ne všichni pacienti jsou schopni lézi dostatečně zkompenzovat. Vestibulární rehabilitace v sobě v současné době zahrnuje kromě specifických vestibulárních cvičení také možnosti využití prehabituace a virtuální reality. Díky prehabituaci, tedy předoperační chemické labyrintektomii pomocí intratympanálně podaného gentamicinu, dochází k časovému oddělení vzniku vestibulární léze od vlastního chirurgického výkonu a tím možnosti dosažení vestibulární kompenzace ještě před samotnou resekcí vestibulárního schwannomu. V poslední dekádě se nástroje pro využití virtuální reality stávají cenově dostupné, a tím pádem i v širší praxi použitelné. Prostřednictvím virtuální reality mohou být pacienti vystavováni scénám, které posilují 3D optokinetickou stimulaci, čímž se rozšiřují možnosti posilování centrálních kompenzačních mechanizmů a tím dochází ke zlepšení kvality života pacientů.
This review aims to summarize the current vestibular rehabilitation options in patients after vestibular schwannoma surgery. Resection of the tumor usually leads to unilateral acute peripheral or combined vestibular loss caused by a surgical lesion to the branches of the vestibular nerve and less frequently also a lesion in the cerebellum. In patients, the vestibular lesion manifests as postural instability, vertigo, oscillopsia and vegetative symptoms. Human organisms react to this state using the process of central compensation with a significant role of the cerebellum. The goal of vestibular rehabilitation is to support this process and thus make recovery faster and more efficient since not all patients are capable of sufficient restoration of vestibular function. Currently, vestibular rehabilitation includes, apart from the specific vestibular exercise, modern techniques using virtual reality space and prehabituation. Due to prehabituation, i.e., preoperative chemical labyrinthectomy with intratympanically installed gentamicin, the timing of the origin of the acute vestibular loss and surgical procedure is separated. Therefore, there is a chance of achieving vestibular compensation before vestibular schwannoma removal. In the last decade, the tools for virtual reality have become less expensive and more available in medical care. Virtual reality is a technique used for generating an environment that can strengthen three-dimensional optokinetic stimulation, subsequently the process of central compensation and finally it may improve patients’ quality of life.

PURPOSE: There is a clinical need for shortened door-to-needle time (DNT) for intravenous thrombolysis, but effective training methods are missing. Simulation training improves teamwork and logistics in numerous fields. Still, it is not clear if simulation improves logistics in stroke. METHODS: To evaluate the efficiency of a simulation training programme, the DNT of participating centres was compared with the rest of stroke centres in the Czech Republic. Patients' data were prospectively collected from the nationally used Safe Implementation of Treatments in Stroke Registry. The outcome was an improvement in DNT in 2018 as compared with 2015 (after and before the simulation training). Scenarios were based on real clinical cases, and simulation courses were conducted in a standardly equipped simulation centre. FINDINGS: Between 2016 and 2017, 10 courses were conducted for stroke teams from 9 of all 45 stroke centres. DNT data were available both in 2015 and 2018 from 41 (91%) stroke centres. The simulation training improved the DNT in 2018 as compared with 2015 by 30 min (95% CI 25.7 to 34.7) and as compared with 20 min (95% CI 15.8 to 24.3) in stroke centres without the simulation training (p=0.01). Any parenchymal haemorrhage occurred in 5.4% and 3.5% of patients treated in centres without and with simulation training (p=0.054), respectively. CONCLUSIONS: DNT was considerably shortened nationally. It was feasible to implement simulation as a nationwide training programme. The simulation was associated with improved DNT; however, other studies should confirm that such an association is causal.
Presbycusis and tinnitus are the two most common hearing related pathologies. Although both of these conditions presumably originate in the inner ear, there are several reports concerning their central components. Interestingly, the onset of presbycusis coincides with the highest occurrence of tinnitus. The aim of this study was to identify age, hearing loss, and tinnitus related functional changes, within the auditory system and its associated structures. Seventy-eight participants were selected for the study based on their age, hearing, and tinnitus, and they were divided into six groups: young controls (Y-NH-NT), subjects with mild presbycusis (O-NH-NT) or expressed presbycusis (O-HL-NT), young subjects with tinnitus (Y-NH-T), subjects with mild presbycusis and tinnitus (O-NH-T), and subjects with expressed presbycusis and tinnitus (O-HL-T). An MRI functional study was performed with a 3T MRI system, using an event related design (different types of acoustic and visual stimulations and their combinations). The amount of activation of the auditory cortices (ACs) was dependent on the complexity of the stimuli; higher complexity resulted in a larger area of the activated cortex. Auditory stimulation produced a slightly greater activation in the elderly, with a negative effect of hearing loss (lower activation). The congruent audiovisual stimulation led to an increased activity within the default mode network, whereas incongruent stimulation led to increased activation of the visual cortex. The presence of tinnitus increased activation of the AC, specifically in the aged population, with a slight prevalence in the left AC. The occurrence of tinnitus was accompanied by increased activity within the insula and hippocampus bilaterally. Overall, we can conclude that expressed presbycusis leads to a lower activation of the AC, compared to the elderly with normal hearing; aging itself leads to increased activity in the right AC. The complexity of acoustic stimuli plays a major role in the activation of the AC, its support by visual stimulation leads to minimal changes within the AC. Tinnitus causes changes in the activity of the limbic system, as well as in the auditory AC, where it is bound to the left hemisphere.

Úvod: Vysokofrekvenční tónová audiometrie (VFA) nepatří k rutinnímu audiologickému vyšetření. Na ORL klinice FN v Motole je tato metoda využívána často, zejména v případě pacientů sledovaných pro tumor mostomozečkového koutu (MMK), avšak jednoznačná indikační kritéria nejsou stanovena. Zpětně jsme procházeli audiometrické záznamy s cílem charakterizovat skupinu osob, u které měla metoda diagnostický přínos. Materiál a metodika: Retrospektivně jsme analyzovali 1 515 audiogramů provedených audiometrem MADSEN Astera firmy GN Otometrics během let 2011–2018 u 773 dospělých osob ve věku 16–80 let (47,79 ± 13,54) a anamnézu těchto pacientů. U všech byla provedena VFA zahrnující běžně vyšetřované frekvence do 8 kHz a frekvence nad 8 kHz: 9 kHz, 10 kHz, 11,2 kHz, 12,5 kHz, 14 kHz a 16 kHz. Pacienti podstoupili VFA v rámci vyšetření z různých indikací – pro subjektivní hypakuzi/hyperakuzi, tinitus, vestibulopatii, po akutraumatu či byli odesláni na naše pracoviště již s diagnostikovaným tumorem MMK. Výsledky: U 52 osob (6,73 %) se objevily patologické hodnoty nebo klinicky významná asymetrie pouze v oblasti nad 8 kHz. Tuto skupinu tvořili z 48,08 % pacienti s vestibulárním schwannomem. Dalších 11,53 % byli pacienti s konečnou diagnózou jiných tumorů a vaskulárních malformací otologické oblasti, 3,85 % pacienti postižení vestibulopatií a 23,08 % vyšetřených dále nevyvinulo významně patologický stav, jednalo se většinou o pacienty s prostou hypakuzí a tinitem. Závěr: V klinické praxi se objevují případy pacientů se sluchovou vadou zjistitelnou pouze při provedení VFA. VFA zpřesňuje diagnostiku sluchových poruch a může vést k odhalení i potenciálně život ohrožujících stavů, jako jsou nádory a cévní malformace otologické oblasti.
Introduction: High-frequency audiometry is not usually carried out during routine hearing examinations. It is a frequently used method at the ENT Clinic of the University Hospital Motol (especially in patients being treated for tumours of the pontocerebellar angle), although the exact indicative criteria for this have not been determined. We aimed to characterize a group of patients, using a method which improved the accuracy of the diagnostics. Material and methodology: We analysed 1,515 audiograms retrospectively. These audiograms were performed by using audiometer MADSEN Astera of GN Otometrics during 2011–2018 in 773 adults, aged 16–80 (47.79 ±13.54). They all underwent high-frequency audiometry comprising of all commonly examined frequencies up to 8 kHz, and also at higher frequencies of – 9 kHz, 10 kHz, 11.2 kHz, 12.5 kHz, 14 kHz a 16 kHz. The indication was very diverse – subjective hypacusis/hyperacusis, tinnitus, vestibulopathy, acutrauma, or an already known diagnosis of a pontocerebellar tumour. Results: We identified a clinically significant asymmetry or pathological elevation of the hearing threshold, which was only detectable at frequencies above 8 kHz in 52 patients (6.73%). This group consisted of patients with vestibular schwannoma (48.08%), other tumours and vascular malformations of the temporal region (11.53%), vestibulopathy (3.85%), and patients that had never developed a serious pathology (23.08%). They were mostly patients with straightforward hypacusis or tinnitus. Conclusion: In clinical practice, there are cases of patients with a hearing defect that can only be detected when HFA is performed. HFA clarifies the diagnosis of hearing disorders and can lead to the detection of potentially life-threatening conditions such as tumours and vascular malformations of the otological area.

Úvod: Zachování sluchu při operaci vestibulárního schwannomu je stále výzvou i pro zkušené chirurgy. Sledování evokovaných kmenových odpovědí (brainstem evoked response audiometry; BERA) patří mezi standardní metody peroperační monitorace sluchu, nicméně v posledních desetiletích již byl zcela vyčerpán potenciál této techniky k dalšímu zlepšování výsledků zachování sluchu. Zatím málo rozšířenou metodou monitorace sluchu je sledování přímých odpovědí ze sluchového nervu (cochlear nerve action potential; CNAP). Přímá monitorace informuje operatéra téměř okamžitě o stavu vnitřního ucha a sluchového nervu, a umožňuje tak rychle reagovat změnou operační techniky. Soubor a metodika: V období únor–květen 2021 jsme použili monitoraci přímých odpovědí ze sluchového nervu u čtyř pacientů. K monitoraci byla použita monitorační jednotka AVALANCHE®XT (Dr. Langer Medical GmbH, Waldkirch, Německo) umožňující souběžnou monitoraci BERA, CNAP a lícního nervu. U tří pacientů bylo primárním cílem zachovat sluch. U čtvrtého pacienta s objemným nádorem bylo cílem trasovat sluchový nerv. Výsledky: U dvou pacientů byl pooperačně zachován sluch. V prvním případě byly na konci operace výbavné evokované potenciály i přímé odpovědi sluchového nervu. V druhém případě byly navzdory pozitivní přímé monitoraci sluchového nervu evokované potenciály již nevýbavné. U třetího pacienta nebyl sluch zachován navzdory zachovaným evokovaným odpovědím a pozitivní přímé monitoraci na konci výkonu. U čtvrtého pacienta s nádorem velikosti IV dle Koose a neexistujícími evokovanými odpověďmi od počátku výkonu se podařilo detekovat přímou odpověď ze sluchového nervu. Sluchový nerv musel být poté přerušen z důvodu jeho infiltrace tumorem. Závěr: Přímá monitorace sluchového nervu přináší operatérovi informaci o stavu sluchu v průběhu operace vestibulárního schwannomu v reálném čase. Tato metoda by mohla být též citlivější v predikci pooperačního zachování sluchu ve srovnání s evokovanými potenciály. Současně využití monitorační sondy umožňuje trasovat sluchový nerv i v případě větších nádorů, u kterých je dosud úspěšnost zachování sluchu velmi nízká.
Introduction: Hearing preservation during vestibular schwannoma surgery is still a challenge even for experienced surgeons. Monitoring of evoked brainstem potentials (brainstem evoked response audiometry; BERA) is one of the standard methods of perioperative hearing evaluation; however, in recent decades, its potential to further improve hearing preservation outcomes has been fully exhausted. Monitoring of direct responses from the cochlear nerve (cochlear nerve action potential; CNAP) is not a widespread technique so far. Direct monitoring informs surgeons almost immediately about the condition of the inner ear and auditory nerve, allowing them to react quickly by changing surgical techniques. Materials and methods: During the February–May 2021 period, we used direct monitoring in 4 patients. An AVALANCHE®XT monitoring unit (Dr. Langer Medical GmbH, Waldkirch, Germany) was used for monitoring, allowing simultaneous monitoring of BERA, CNAP, and the facial nerve. In three patients, the primary goal was to preserve hearing. In the fourth patient with a large tumor, the goal was to track the auditory nerve. Results: Hearing has been preserved in two patients postoperatively. In the first case, both evoked potentials and direct auditory nerve responses were detected at the end of the surgery. In the second case, despite the positive direct responses, the evoked potentials were already not elicited. In the third patient, hearing was not preserved despite elicited evoked responses and direct responses at the end of the procedure. In the fourth patient with a Koose size IV tumor and non-elicited evoked responses from the beginning of the procedure, a direct response from the auditory nerve was detected. The auditory nerve then had to be discontinued due to its infiltration by the tumor. Conclusion: Direct monitoring of the cochlear nerve provides the surgeon with real-time information about the state of hearing during vestibular schwannoma surgery. This method could also be more sensitive in predicting postoperative hearing preservation compared to evoked potentials. Simultaneously with the use of a monitoring probe, it makes it possible to trace the auditory nerve even in the case of larger tumors, in which the success rate in hearing preservation is still very low.

Cíl: Ménièrova choroba je jednou z nejčastějších paroxyzmálních poruch periferního vestibulárního systému. Léčba je především farmakologická, rozdílná v období mezi atakami a v době akutních obtíží. Při selhání farmakologické léčby bývá využívána chemická labyrintektomie nebo chirurgická léčba. Cílem této studie bylo zjistit úspěšnost prováděné chirurgické léčby porovnáním objektivních a subjektivních metod. Soubor a metodika: V souboru pacientů operovaných pro farmakorezistentní Méni?rovu chorobu jsme hodnotili předoperační a pooperační výsledky testu video-Head Impulse a kalorizace s videookulografickým záznamem. K subjektivnímu hodnocení obtíží byl využit dotazník Dizziness Handicap Inventory. Výsledky: Pooperačně dochází ke statisticky významné kalorické hyporeflexii až areflexii operovaného ucha (p < 0,05), zatímco při vyšetření testem video-Head Impulse nebyl ani v jednom ze sledovaných parametrů pozorován signifikantní rozdíl před a po operaci (p > 0,05). Z dotazníků bylo zjištěno, že pacienti po operaci měli lepší kvalitu života z pohledu závrativých obtíží, signifikantní rozdíl jsme pozorovali ve všech hodnocených parametrech (p < 0,05). Závěr: Využitím chirurgické léčby u Ménièrovy choroby je možné ovlivnit vestibulární ataky s výrazným zlepšením kvality života pacientů. Dle výsledků studie se osvědčilo hodnotit pooperační stav kombinací kalorizace a dotazníkových metod.
Aim: Ménière's disease is one of the most common paroxysmal disorders of the peripheral vestibular system. Treatment is mainly pharmacological. Treatment differs for acute attacks and the period between the attacks. If pharmacological treatment fails, chemical labyrinthectomy or surgical treatment is being used. The aim of this study was to determine the success of surgical treatment by comparing objective and subjective methods. Patients and methods: In a group of patients operated on for drug-resistant Méni?re‘s disease, we evaluated the preoperative and postoperative results of the video-Head Impulse Test and caloric test with video oculographic recording. The Dizziness Handicap Inventory questionnaire was used for the subjective evaluation of symptoms. Results: Postoperatively, there is a statistically significant caloric hyporefl exia or areflexia in the operated ear (P < 0.05), while for the video-Head Impulse Test there was no significant difference observed in any of the monitored parameters before and after surgery (P > 0.05). After surgery, patients had a better quality of life from the perspective of dizziness according to the questionnaire survey. A significant difference in all evaluated parameters (P < 0.05) was observed. Conclusion: Using surgical treatment for Ménière's disease, it is possible to influence vestibular attacks with a significant improvement in the quality of life of patients. According to the results of the study, it seems to be useful to evaluate the postoperative condition by a combination of the caloric test and questionnaire methods.
