The impact of bacterial pneumonia on patients with COVID-19 infection remains unclear. This prospective observational monocentric cohort study aims to determine the incidence of bacterial community- and hospital-acquired pneumonia (CAP and HAP) and its effect on mortality in critically ill COVID-19 patients admitted to the intensive care unit (ICU) at University Hospital Olomouc between 1 November 2020 and 31 December 2022. The secondary objectives of this study include identifying the bacterial etiology of CAP and HAP and exploring the capabilities of diagnostic tools, with a focus on inflammatory biomarkers. Data were collected from the electronic information hospital system, encompassing biomarkers, microbiological findings, and daily visit records, and subsequently evaluated by ICU physicians and clinical microbiologists. Out of 171 patients suffering from critical COVID-19, 46 (27%) had CAP, while 78 (46%) developed HAP. Critically ill COVID-19 patients who experienced bacterial CAP and HAP exhibited higher mortality compared to COVID-19 patients without any bacterial infection, with rates of 38% and 56% versus 11%, respectively. In CAP, the most frequent causative agents were chlamydophila and mycoplasma; Enterobacterales, which were multidrug-resistant in 71% of cases; Gram-negative non-fermenting rods; and Staphylococcus aureus. Notably, no strains of Streptococcus pneumoniae were detected, and only a single strain each of Haemophilus influenzae and Moraxella catarrhalis was isolated. The most frequent etiologic agents causing HAP were Enterobacterales and Gram-negative non-fermenting rods. Based on the presented results, commonly used biochemical markers demonstrated poor predictive and diagnostic accuracy. To confirm the diagnosis of bacterial CAP in our patient cohort, it was necessary to assess the initial values of inflammatory markers (particularly procalcitonin), consider clinical signs indicative of bacterial infection, and/or rely on positive microbiological findings. For HAP diagnostics, it was appropriate to conduct regular detailed clinical examinations (with a focus on evaluating respiratory functions) and closely monitor the dynamics of inflammatory markers (preferably Interleukin-6).

Úvod: Srdeční zástava (SZ) s následnou cerebrální hypoxií vede k multifaktoriálnímu poškození mozku. Kognitivní deficit a vyšší míra depresivních projevů jsou nejčastěji popisovanými psychickými důsledky SZ. Cílem předkládané studie je charakterizovat psychosociální důsledky SZ. Metody: Průzkumného šetření se účastnilo 113 osob. K 62 pacientům po SZ bylo dle demografických charakteristik a premorbidní inteligenční úrovně přiřazeno 51 zdravých jedinců. Účastníkům byly předloženy testy kognitivní výkonnosti (Montreal Cognitive Assessment, MoCA), depresivních (Beck Depression Inventory-II, BDI-II) a úzkostných projevů (State-Trait Anxiety Inventory, STAI) a sociálních faktorů krize středního věku (Škála faktorů krize středního věku, ŠKSV). Výsledky: Analýza ukázala, že pacienti po SZ vykazují nižší kognitivní výkonnost (p = 0,016) a vyšší míru symptomů aktuální úzkosti (p = 0,023). V úrovni depresivních symptomů (p = 0,435) a v dlouhodobé úzkostnosti (p = 0,542) se pacienti po SZ nelišili od kontrolního souboru. Ex-post facto analýza založená na logistické regresi poukazuje na to, že nejsilnějším psychologickým prediktorem SZ je pohlaví (OR = 4,45) a míra aktuálně prožívané úzkosti (OR = 0,50). Analýza diskriminační funkce ukazuje, že predikce vzniku SZ je závislá na věku, kognitivní výkonnosti a na míře aktuální úzkosti (λ = 0,81, p = 0,028). Závěr: Výsledky studie ukazují, že SZ má významný vliv na rozvoj nežádoucích kognitivních a neuropsychiatrických projevů. Zařazení psychosociální péče a neuropsychiatrické léčby do komplexní péče o pacienty po SZ je žádoucí.
Background: Cardiac arrest (CA) leads to cerebral hypoxia resulting in multifactorial brain injury. Cognitive impairment and a higher degree of depressive symptoms are the most frequently described mental health problems after CA. The aim of the present study is to characterize psychosocial sequelae of CA. Methods: The study population included 113 subjects. 62 patients after CA were matched to 51 healthy controls according to demographic characteristics and premorbid intelligence level. Cognitive test (MoCA), inventories of depressive (BDI-II) and anxiety symptoms (STAI) and midlife crisis scale (MCS) were administrated to study participants. Results: The analysis showed that CA patients have a decreased level of cognitive performance (p = 0.016) and a higher degree of state anxiety symptoms (p = 0.023). There was no significant difference between CA patients and control subjects in the degree of depressive (p = 0.435) and trait anxiety symptoms (p = 0.542). Ex-post facto analysis based on logistic regression indicated that the strongest predictors of being classified as having had a cardiac arrest was male gender and state anxiety (OR = 4.45 and 0.50). Discriminant function analysis showed that group prediction was sensitive to age, cognitive performance, and state anxiety (λ = 0.81, p = 0.028). Conclusions: Our results show that CA has significant cognitive and neuropsychiatric sequelae. The integration of psychosocial care and neuropsychiatric treatment into the complex medical care of CA patients seems to be justified.

Cíl studie: Zjistit u kriticky nemocných pacientů s normálními hodnotami arteriálního pH, pCO2, BE a HCO3- přítomnost poruchy acidobazické rovnováhy hodnocené podle Stewartova-Fenclova principu. Porovnat hodnoty SIDapp, SIDef, SIG a AGkor u pacientů, kteří zemřeli do 3 měsíců od přijetí na resuscitační lůžko, a pacientů, kteří přežili déle než 3 měsíce od přijetí na resuscitační lůžko. Typ studie: Monocentrická retrospektivní observační studie. Typ pracoviště: Resuscitační oddělení krajské nemocnice. Materiál a metoda: Kritéria (normální hodnoty arteriálního pH, pCO2, BE, HCO3- a zároveň odběr všech parametrů nutných k vypočítání SID a SIG) splnilo 112 pacientů, 39 z nich zemřelo do 3 měsíců od přijetí, 50 přežilo déle než 3 měsíce a 23 bylo z další části studie vyřazeno. U těchto pacientů bylo spočítáno SIDapp, SIDef, SIG a AGkor a zjištěn rozdíl mezi danými dvěma skupinami. Výsledky: Celkem 110 pacientů mělo abnormální hodnoty SIDef (98,21 %), 107 pacientů mělo abnormální hodnoty SIDapp (95,54 %), 52 pacientů mělo hodnotu SIG nad 5 mmol/l (46,43 %) a 4 z nich SIG nad 10 mmol/l (3,57 %). Mezi zemřelými a přeživšími byl nalezen statisticky signifikantní rozdíl mezi hodnotami SIDapp (p < 0,01), SIG (p < 0,005) a AGkor (p < 0,005). Nebyl nalezen statisticky signifikantní rozdíl mezi hodnotami SIDef. Závěr: V této studii bylo potvrzeno, že většina pacientů přijímaných do nemocnice v akutním stavu, kteří mají normální parametry arteriálního pH, pCO2, BE a HCO3-, mají poruchu acidobazické rovnováhy hodnocené podle Stewarta-Fencla. Dále bylo prokázáno, že existuje signifikantní rozdíl v hodnotách SIDapp, SIG a AGkor mezi danými dvěma skupinami. Objective: To identify acid-base disturbances according to the Stewart-Fencl approach in critically ill patients with normal arterial pH, pCO2, HCO3- and BE. To compare the SIDapp, SIDef, SIG and AGkor values in the patients who were vs. were not alive at three months from ITU admission. Design: Single-centre, retrospective, observational study. Setting: Anaesthesiology and Resuscitation Department of a Regional Hospital. Materials and methods: The criteria (normal arterial pH, pCO2, BE, HCO3-) were met by 112 critically ill patients. In these patients, the parameters needed to calculate the SID and SIG were sampled. Thirty nine of the patients died within three months of admission, 50 patients survived longer than 3 months and 23 patients were eliminated from the subsequent part of the study. SIDapp, SIDef, SIG and AGkor values were calculated in all these patients. Results: A total of 110 patients had abnormal SIDef values (98.21%), 107 patients had abnormal SIDapp values (95.54%), 52 patients had SIG values above 5 mmol/l (46.43%) and 4 of them over 10 mmol/l (3.57%). There was a statistically significant difference between the non-survivors and survivors in the SIDapp (p < 0.01), SIG (p < 0.005) and AGkor (p < 0.005) values. There was no statistically significant difference between the SIDef values. Conclusion: This study confirmed that the majority of critically ill patients admitted to the ITU with normal parameters of arterial pH, pCO2, HCO3- and BE have acid-base disturbances according to the Stewart-Fencl approach. We found a significant difference in SIDapp, SIG and AGkor values between the survivors and the non-survivors.

During an outbreak of methanol poisonings in the Czech Republic in 2012, we were able to study methanol and formate elimination half-lives during intermittent hemodialysis (IHD) and continuous veno-venous hemodialysis/hemodiafiltration (CVVHD/HDF) and the relative impact of dialysate and blood flow rates on elimination. Data were obtained from 11 IHD and 13 CVVHD/HDF patients. Serum methanol and formate concentrations were measured by gas chromatography and an enzymatic method. The groups were relatively comparable, but the CVVHD/HDF group was significantly more acidotic (mean pH 6.9 vs. 7.1 IHD). The mean elimination half-life of methanol was 3.7 and formate 1.6 h with IHD, versus 8.1 and 3.6 h, respectively, with CVVHD/HDF (both significant). The 54% greater reduction in methanol and 56% reduction in formate elimination half-life during IHD resulted from the higher blood and dialysate flow rates. Increased blood and dialysate flow on the CVVHD/HDF also increased elimination significantly. Thus, IHD is superior to CVVHD/HDF for more rapid methanol and formate elimination, and if CVVHD/HDF is the only treatment available then elimination is greater with greater blood and dialysate flow rates.

Úvod: Těžká sepse je stále spojena s významnou morbiditou a mortalitou, která se nicméně liší, stejně jako její management, v závislosti na regionu. Jaká je situace v ČR a jaký mají charakter pacienti s těžkou sepsí v současné době, není známo. Cílem projektu je popsat procesy péče, výsledek léčby a charakteristiku pacientů s těžkou sepsí přijímaných na pracoviště intenzivní péče v ČR. Metodika: Jedná se o multicentrický a observační projekt s retrospektivním zařazováním pacientů, kteří splní kritéria těžké sepse před nebo do 24 hod od přijetí na vybrané jednotky intenzivní péče (EPOSS‑ICU). Výsledky: Analyzováno bylo 394 pacientů. Medián věku při přijetí byl 66 (56–76) let, převažovali muži (58,9 %) a medián APACHE II skóre při příjmu byl 25 (19–32). Příjímaní pacienti byli převážně interního charakteru (56,9 %) a nejvíce příjmů bylo sekundárních z jiných ICU (53,6 %). Splnění kritérií těžké sepse bylo nejčastěji v období (? 4 hod) od příjmu na EPOSS‑ICU (77,6 %). Medián celkového příjmu tekutin během prvních 24 hod byl 6 680 (4 840–9 450) ml. Většina nemocných vyžadovala umělou plicní ventilaci (58,4 %). Splnění tzv. resuscitačních balíčků pro těžkou sepsi bylo v našem souboru velmi dobré a bylo spojeno s nižší mortalitou pacientů. Nejčastěji byla délka pobytu na EPOSS‑ICU 7 (3–15) dnů a medián celkové doby hospitalizace v nemocnici byl 13 (8–28) dnů. Celková nemocniční mortalita souboru byla 35,8 %. Závěr: Představujeme projekt, který ve své první fázi získal cenná a mezinárodně srovnatelná data o pacientech s těžkou sepsí přijímaných na zapojená pracoviště intenzivní péče v ČR.
Introduction: Severe sepsis is still associated with significant morbidity and mortality, which is however different, as well as its management, depending on the region. What is the situation in the Czech Republic and what is the character of patients with severe sepsis is currently not known. The aim of the project is to describe the processes of care, outcome and characteristics of patients with severe sepsis admitted to the intensive care department of the Czech Republic. Methods: This is a multicentre and observational project with retrospective enrollment of patients who meet the criteria for severe sepsis before or within 24 hours after admission to selected intensive care units (ICU‑EPOSS). Results: 394 patients were analyzed. Median age at admission was 66 (56–76) years, males predominated (58.9%) and the median APACHE II score on admission was 25 (19–32). Patients were predominantly medical (56.9%) and most were secondary admitted from other ICU (53.6%). Meeting the criteria of severe sepsis was most frequently within the period (? 4 hours) of admission the EPOSS‑ICU (77.6%). Median total fluid intake during the first 24 hours was 6,680 (4,840–9,450) ml. Most patients required mechanical ventilation (58.4%). Compliance with the resuscitation bundle of severe sepsis in our group was very good and was associated with lower mortality of patients. Most frequently, the EPOSS‑ICU length of stay (LOS) was 7 (3–15) days and median hospital LOS was 13 (8–28) days. Hospital mortality in our cohort was 35.8%. Conclusion: Introducing the project, which in its first stage obtained valuable and internationally comparable data about patients with severe sepsis admitted to the involved ICU in the Czech Republic.

Transcranial color-coded sonography (TCCS) monitoring of severe brain injury patients may reveal various pathological hemodynamic changes. According to changes in flow velocities in basal brain arteries, the presence of brain hyperemia, vasospasms, and oligemia can be detected. The study included a group of 20 patients with severe brain injury. TCCS measured flow velocities and ICP values were monitored on a daily basis in the course of a week after injury. In nearly 50 % of patients significant hemodynamic changes occurred. The most frequent pathological finding was hyperemia (31.8 %), followed by vasospasm (10.9 %) and oligemia (9.1 %). In 42.7 % of patients increased flow velocities were registered and only 9.1 % of records were within the normal range of values. The most substantial elevation in time-averaged mean velocity occurred from the second to the sixth day after injury. In a subgroup of patients with raised intracranial pressure 41.6 % of flow velocity (FV) measuring met the TCCS criteria for hyperemia compared with 26 % in a subgroup of patients without intracranial pressure (ICP) elevation. The study showed that hemodynamic changes after severe brain injury are relatively common findings and that TCCS is a useful bed-side tool for the monitoring of intracranial hemodynamic changes.
