Specifická alergenová imunoterapie je unikátní kauzální personalizovaný přístup k léčbě alergií. Jednou se základních podmínek úspěšné terapie je adekvátní dávkování. Mimo studie v rámci požadavků evidence-based medicine (EBM) osvědčujících efektivitu léčivých produktů pro určitou konkrétní udržovací (denní) dávku, najdeme i studie srovnávající účinnost různých dávek („vícedávkové“ studie). Z nálezů obou typů studií se odvozují doporučené udržovací (denní) dávky pro léčbu jednotlivých druhů inhalačních alergií, respektive vzhledem k jedinečnému a nepřenositelnému charakteru extraktových terapeutik různých výrobců pro konkrétní léčivé produkty.
Specific allergen immunotherapy is a unique causal personalized approach to allergy treatment. One of the basic conditions of successful treatment is adequate dosage. In addition to studies within the framework of EBM requirements certifying the effectiveness of medical products for a specific maintenance (daily) dose, we also find studies comparing the effectiveness of different doses (dose-response studies). Recommended maintenance (daily) doses for the treatment of individual types of inhalation allergies are derived from the findings of both types of studies, respectively due to the unique and non-transferable nature of extract therapeutics from different manufacturers for specific therapeutic products

Allergen specific immunotherapy (AIT) is the only causal therapeutic option for allergic airway diseases including asthma and allergic rhinitis. AIT has been shown to restore the allergen immune tolerance, can modify both the early and late-onset allergen-specific airway hyperreactivity, helps to achieve disease control/remission and prevents new sensitisations. Recent real life data on long-term effectiveness of house dust mite (HDM) AIT in a large group of patients with HDM-driven asthma further underscored its unique therapeutic potential as well as confirmed previous data with pollen AIT. More widespread use of this causal treatment in select patient populations should further move this promising therapeutic field. In this mini-review, we discuss updates on new insights based on real world patient data.

Bile acid profiles are altered in obese individuals with asthma. Thus, we sought to better understand how obesity-related systemic changes contribute to lung pathophysiology. We also test the therapeutic potential of nitro-oleic acid (NO2-OA), a regulator of metabolic and inflammatory signaling pathways, to mitigate allergen and obesity-induced lung function decline in a murine model of asthma. Bile acids were measured in the plasma of healthy subjects and individuals with asthma and serum and lung tissue of mice with and without allergic airway disease (AAD). Lung function, indices of inflammation and hepatic bile acid enzyme expression were measured in obese mice with house dust mite-induced AAD treated with vehicle or NO2-OA. Serum levels of glycocholic acid and glycoursodeoxycholic acid clinically correlate with body mass index and airway hyperreactivity whereas murine levels of β-muricholic acid and tauro-β-muricholic acid were significantly increased and positively correlated with impaired lung function in obese mice with AAD. NO2-OA reduced murine bile acid levels by modulating hepatic expression of bile acid synthesis enzymes, with a concomitant reduction in small airway resistance and tissue elastance. Bile acids correlate to body mass index and lung function decline and the signaling actions of nitroalkenes can limit AAD by modulating bile acid metabolism, revealing a potential pharmacologic approach to improving the current standard of care.

House dust mites inhabit bed mattresses contaminating them with allergens. A strong temperature/moisture gradient exists in mattresses when it is used by humans daily. Here, we studied migration patterns of the mite Dermatophagoides farinae in continuous and time-discontinuous temperature gradients consisting of five sectors with 19-23, 23-28, 28-32, 32-36 and 36-41 °C, containing dye-labeled diets as an indicator of mite presence and feeding. The mites migrated through the sectors and fed on the labeled diets or stayed unfed. The numbers of mites with the same coloration in their guts and the numbers of unfed mites in the sectors were recorded. Unfed mites provided information on short-term temperature preferences. Apart from a control trial, two experiments were performed: (i) a constant 19-41 °C gradient for 24 h, and (ii) alternating cycles of the same temperature gradient (19-41 °C, 8 h) and room temperature (16 h) for 5 days to model the typical daily occupancy of bed by humans. In both experiments, fed mites preferred a sector with 32-36 °C, suggesting that in mattresses, house dust mites prefer to stay as close as possible to the resting human, thus maximizing allergen exposure. However, the number of unfed mites decreased with increased temperatures in the gradient. Experiment (ii) showed that the fed mites remained at the same optimal distance from the heat source, suggesting that they stay at the upper surface of the regularly used mattress, even when human was temporarily absent during the day. Unfed mites apparently hide deeper in mattresses as suggested by their avoidance of increased temperatures.
The molecular basis for the propensity of a small number of environmental proteins to provoke allergic responses is largely unknown. Herein, we report that mite group 13 allergens of the fatty acid-binding protein (FABP) family are sensed by an evolutionarily conserved acute-phase protein, serum amyloid A1 (SAA1), that promotes pulmonary type 2 immunity. Mechanistically, SAA1 interacted directly with allergenic mite FABPs (Der p 13 and Blo t 13). The interaction between mite FABPs and SAA1 activated the SAA1-binding receptor, formyl peptide receptor 2 (FPR2), which drove the epithelial release of the type-2-promoting cytokine interleukin (IL)-33 in a SAA1-dependent manner. Importantly, the SAA1-FPR2-IL-33 axis was upregulated in nasal epithelial cells from patients with chronic rhinosinusitis. These findings identify an unrecognized role for SAA1 as a soluble pattern recognition receptor for conserved FABPs found in common mite allergens that initiate type 2 immunity at mucosal surfaces.

Roztočová alergie je ve střední Evropě nejčastější a nejzávažnější formou nesezonní inhalační alergie. V předešlé části jsme shrnuli aktuální teoretické poznatky o této nejčastější celoroční alergii. Přiblížili jsme alergenovou imunoterapii (AIT) jako jedinou kauzální léčbu s potenciálem změnit přirozený průběh nemoci. V této části představujeme vlastní zkušenosti s provokačními testy a se sublingvální roztočovou tabletou. O účinnosti AIT rozhoduje kumulativní dávka alergenu, a tak u pacientů, kteří dostatečně neadherují ke každodennímu užívaní a/nebo léčbu předčasně ukončí, nemůžeme počítat s její účinností.
House dust mite (HDM) allergy is in Middle Europe most common and significant form of non-seasonal inhaled allergy. In the previous part we summarised actual knowledge about this most common perennial allergy. Allergen immunotherapy (AIT) as the only causal treatment with the potential to change the disease development was introduced. In this part we present our own experience with provocation tests and with sublingual (SLIT) HDM tablet. We demonstrate possible difficulties which must be calculated to ensure the adherence to SLIT. Sufficient cumulative dose must be taken to assure effect of this type of treatment

Roztočová alergie je ve střední Evropě nejčastější a nejzávažnější formou nesezónní inhalační alergie. Diagnostika přecitlivělosti na roztoče vychází ze syntézy důsledné alergologické anamnézy, prick testů a vyšetření specifických IgE (sIgE) protilátek, pro ověření kauzality jsou vhodné provo-kační testy. Po realizaci kožního testu s alergenovými extrakty při vyšetření sIgE upřednostňujeme molekulovou (komponentovou) diagnostiku. I když současná diagnostika roztočové alergie není zcela optimální – extraktová diagnostika má svá omezení a pro komponentovou diagnostiku některé z hlavních roztočových alergenů chybí – většinu roztočových pacientů můžeme diagnostikovat správně a také léčebně podchytit. Alerge-nová imunoterapie je jediná léčba modifikující vývoj respirační alergie směrem k toleranci alergenu. V současnosti máme k dispozici kvalitní sub-lingvální alergenovou imunoterapii (SLIT), kterou bychom se měli snažit poskytnout co největšímu počtu pacientů, včetně roztočových astmatiků
House dust mite (HDM) allergy is in Middle Europe most common and significant form of non-seasonal inhaled allergy. Diagnosis of hyper-sensitivity to HDM is arising from synthesis of precise anamnesis, prick test with allergen extracts and specific IgE, to prove causality provo-cation tests are advisable. After performing skin prick tests with allergen extracts we prefer molecular (component resolved, CRD) specific IgE diagnostics. Despite actual level of HDM allergy diagnosing is not optimal – many molecules for CRD are missing in routine praxis – most of patients with HDM allergy can be diagnosed and treated adequately. Allergen immunotherapy is the only therapeutic approach influencing natural course of allergic disease. Actually, we have available good-quality products for sublingual allergen immunotherapy (SLIT) with HDM extracts, which should be offered to all suitable patients, including those with asthma.
