Zlomeniny páteře patří mezi častá poranění provázející všechny věkové skupiny. Nestabilní zlomeniny jsou léčeny chirurgicky se snahou o obnovení základních funkcí páteře. Stabilní fraktury se léčí ve většině případů konzervativně nebo pomocí minimálně invazivních technik. Bolest je přítomna u většiny nemocných a může být limitujícím faktorem pro časný návrat k běžným denním aktivitám. Mezi minimálně invazivní metody umožňující ovlivnění bolesti patří analgetické blokády, kyslíko-ozonová terapie (KOT), radiofrekvenční ablace (RFA) či augmentace skeletu polymetylmetakrylátovým (PMMA) cementem. Práce pak předkládá současný pohled medicíny založené na důkazech (EBM) na jednotlivé metody léčby bolesti, přičemž jsou probrány základní principy a aspekty jednotlivých metod. Na pracovišti autorů bylo provedeno za posledních 8 let více než 8800 zákroků léčby bolesti zad s CT navigací a 253 augmentačních zákroků. Nebyla zaznamenána klinicky významná komplikace.
Vertebral fracture is a common trauma can occur at any age group. Unstable fractures are treated surgically to restore the basic spinal functionality. In most cases, stable fractures are treated conservatively or by minimally invasive techniques. Pain is the most common symptom and can be a limiting factor for an early return to ones daily activities. Among the minimally invasive procedures for pain management are analgesis blockades, oxygenozone therapy, radiofrequency ablation or augmentation. This paper presents the current view of evidence-based medicine (EBM) on individual methods of pain treatment of which basic principles and aspects are discussed. Furthermore, the practical experiences of the author‘s workplace are presented. Over the past 8 years, more than 8,800 back pain treatments under CT guidance and 253 augmentation procedures have been performed at the authors‘ workplace. No clinically significant complication was observed.

PURPOSE: The most common injuries to the upper cervical spine are fractures of the dens axis. Therefore, the purpose of our study was to answer three questions, namely (1) whether the size of the dens is adequate at all levels to accommodate two screws, (2) what the angle of the posterior tilt of the dens is in a healthy individual and (3) compare the measured variables between the sexes. METHODS: The cohort comprised 50 males and 50 females CT examination of the craniocervical junction. We measured the five diameters of the dens and posterior dens angulation angle (PDAA) and screw insertion angle (SIA). The same dimensions were measured in a control group, consisting of 40 non-pathological second cervical vertebrae specimens. RESULTS: On CT scans, the mean PDAA was 162.7 degrees in males and 160.26 degrees in females; the mean SIA was 62.0 degrees in males and 60.2 degrees in females. On specimens, the mean PDAA was 169.47 degrees in males and 166.95 degrees in females; the mean SIA was 65.42 degrees in males and 64.47 degrees in females. All obtained values were higher in males; regardless of their measuring on either CT scans or specimens, differences between males and females were statistically significant (p < 0.05) in a, c, d and e values. CONCLUSIONS: The values of our measurements correlate with the dimensions identified previously in other studies. Based on our clinical experience and measurements, we presume that two 3.5-mm screws can be inserted into the dens of all adult patients, except for those with pronounced anatomical anomalies. Posterior dens angulation angle is slightly larger than we expected. The dens is significantly larger in males almost in all measurement. The 2-year, randomized, double-blind, active-controlled fracture endpoint VERO study included postmenopausal women with established osteoporosis, who had at least 2 moderate or 1 severe baseline vertebral fractures (VFx), and bone mineral density (BMD) T-score ≤-1.5. Patients were treated with either s.c. daily teriparatide 20 μg or oral weekly risedronate 35 mg. As previously reported, the risk of new VFx and clinical fractures (a composite of clinical VFx and nonvertebral fragility fractures [NVFFx]) was statistically significantly reduced with teriparatide compared with risedronate. Here we present the prospectively planned subgroup analyses of fracture data across subgroups, which were predefined by the following baseline characteristics: age, number and severity of prevalent VFx, prevalent nonvertebral fractures (NVFx), glucocorticoid use, prior osteoporosis drugs, recent bisphosphonate use, clinical VFx in the year before study entry, and baseline BMD. Heterogeneity of the treatment effect on the primary endpoint (new VFx), and the four key secondary endpoints (including clinical fractures and NVFFx) were investigated by logistic and Cox proportional hazards regression models. A total of 1360 women were randomized and treated (680 per group). Mean age was 72.1 years, mean (SD) number of prevalent VFx was 2.7 (2.1), 55.4% had a BMD T-score <-2.5, 36.5% had a recent clinical VFx, 28.3% had a prior major NVFx, 43.2% were osteoporosis drug-naïve, 39.3% were recent bisphosphonate users, and 9.3% were taking glucocorticoids at a prednisone-equivalent dose of >5 mg/d. For most fracture endpoints, the risk reduction of teriparatide versus risedronate did not significantly differ in any of the subgroups analyzed (treatment-by-subgroup interaction p > 0.1), with most subgroups mirroring results from the total study population. In conclusion, in postmenopausal women with severe osteoporosis, the antifracture efficacy of teriparatide compared with risedronate was consistent in a wide range of patient settings, including treatment-naïve and previously treated patients. © 2018 The Authors. PURPOSE: To present a method of posterior arch and lateral mass screw (PALMS) insertion and to prove its feasibility. METHODS: Four formalin-fixed specimens and 40 macerated atlas vertebras were used to describe the relevant anatomy. The height of the posterior arch was measured on 42 consecutive patients using standard CT of the cervical spine. The operative technique and the special CT reconstructions used for preoperative planning are described. Eight patients underwent posterior fixation using this technique. RESULTS: We described the relevant anatomy and important anatomical landmarks of the posterior arch of the atlas. PALMS placement was modified according to these anatomical findings. Fifteen PALMSs were placed in eight patients using this technique without vascular or neural injury. CONCLUSION: It is feasible to place PALMS using the described technique. CT angiography is of crucial importance for preoperative planning using the described special reconstructions. The arch posterior to the lateral mass (APLM) is defined as the bone stock situated posterior to the lateral mass, respecting its convergence. The ideal entry point for a PALMS is on the APLM above the center of the converging lateral mass. A complete or incomplete ponticulus posticus and a retrotransverse foramen or groove can be used as an accessory landmark to refine the entry point.

Křehká a rigidní, osteoporotická a lehce zranitelná „bambusová páteř", která vzniká transformací normální páteře ve finální fázi m. Bechtěrev, obsahuje normální neurální elementy. Nestabilní zlomenina vzniká často i po minimálním traumatu a morbidita a mortalita je významně vyšší než ve skupině rutinních poranění krční páteře, především v důsledku plicních komplikací. Diagnóza zlomeniny na základě prostých RTG snímkuje obtížná, CT vyšetření je nezbytné. Autori referují o zkušenostech se třemi pacienty. Poukazují na problémy se skeletální trakcí pri chorobou změněné páteřní ose a na rizika spojená se semikonzervativní léčbou. Autori proto doporučují trakci s minimální zátěží s následnou časnou dekompresí a stabilizací kombinovaným předním i zadním pristupem v jednom sezení. Tímto postupem lze předejít sekundárnímu postižení neurálních struktur pri nestabilní zlomenině a časná rehabilitace s vertikalizací bez omezení dýchacích exkurzí ortezou potom výrazně sníží riziko plicních komplikací.
A brittle and rigid, osteoporotic and injury prone „bamboo" spine which develops as a result of transformation of the normal spine in the final stage of Bekhterev's disease contains normal neural elements. Unstable fractures occur frequently even after a minimal injury and the morbidity and mortality is significantly higher than in the group of routine injuries of the cervical, spine, in particular as a result of pulmonary complications. Diagnosis of the fracture based on simple X-ray pictures is difficult, CT examination is essential. The authors present an account on their experience with three patients. They draw attention to problems of skeletal traction when the spinal axis is altered by disease and to risks associated with semiconservative treatment. The authors therefore recommend traction with a minimal load and with subsequent early decompression and stabilization by a combined anterior and posterior approach in a single session. By this approach secondary affection of neural structures in unstable fractures can be prevented, and early rehabilitation with verticalization without restriction of respiratory excursions by an orthesis then significantly reduces the risk of pulmonary complications.
