Inguinálna hernia je jedným z najčastejších chirurgických diagnóz v detskom veku. Často sa diagnostikuje už počas prvého roku života. Klinicky sa inguinálny pruh manifestuje vyklenutím v oblasti slabiny a predstavuje vysunutie orgánov peritoneálnej dutiny cez preformovaný alebo sekundárne vzniknutý otvor. Inguinálna hernia sa môže prejaviť aj ako tuhá, výrazne bolestivá rezistencia v oblasti slabiny alebo miešku, ktorá sa nedá voľne reponovať, vtedy sa jedná o tzv. inkarcerovanú alebo zaškrtenú inguinálnu herniu. Inkarcerovaná hernia u dieťaťa patrí medzi náhle príhody brušné. Liečba inguinálnej hernie je iba operačná. Operácia neinkarcerovanej inguinálnej hernie patrí medzi plánované výkony. Cieľom článku je ponúknuť prehľad o aktuálnych operačných metódach pri liečbe inguinálnej hernie u detí. Okrem klasickej operácie inguinálnej hernie je možno zvoliť aj minimálne invazívny prístup, a to metódou PIRS (percutaneous internal ring suturing) alebo laparoskopicky purse-string, prípadne inými laparoskopickými prístupmi. Operácie inguinálnej hernie u detí sa vykonávajú v celkovej anestézii a možno ich vykonávať aj formou jednodňovej zdravotnej starostlivosti.
Inguinal hernia is one of the most common surgical diagnoses in childhood and is often diagnosed during the first year of life. Clinically, the inguinal hernia usually manifests as a bulge in the groin area and represents the protrusion of the organs of the peritoneal cavity through a preformed or secondary made aperture. An inguinal hernia can also manifest as a stiff, significantly painful resistance in the groin or scrotum area, which cannot be freely reponed, it is a so-called „incarcerated“ or strangulated inguinal hernia. An incarcerated hernia in a child is one of the acute abdominal events. Treatment of inguinal hernia is only surgical. Non-incarcerated inguinal hernia surgery is a planned procedure. The aim of the article is to offer an overview of current surgical methods in the treatment of inguinal hernia in children. The operations of inguinal hernia in children can be performed in several ways. In addition to open inguinal surgery, a minimal invasive approach can also be chosen, using the PIRS (Percutaneous Internal Ring Suturing) method, or the laparoscopic purse-string method, or other laparoscopic approaches. Inguinal hernia is operated under general anaesthesia. Inguinal hernia operations in children can also be performed in the form of one-day surgery.

Článek shrnuje historii operace tříselné kýly. V různých dobách byly pro opravu kýly použity různé postupy a různé materiály. Účinnost a bezpečnost opravy tříselné kýly se však objevila až poté, co byly anatomické rysy tříselné oblasti popsány v monografii Henriho Fruchauda „Anatomie des hernies de l’aine” publikované v roce 1956. Italský chirurg Edoardo Bassini zahájil novou éra v herniologii. Jeho klasický zákrok se svými modifikacemi byl v chirurgické praxi dlouhodobě nejoblíbenější. V roce 1959 navrhl Lloyd M. Nyhus opravu tříselné hernie podle konceptu preabdominálního (zadního) přístupu, který se později stal základem pro vývoj transabdominální preperitoneální opravy hernie (TAPP). V roce 1992 provedl M. Arregui první ТАРР pomocí prolenové sítě. V roce 1986 navrhl Irving Lichtenstein koncept „opravy bez napětí“. Lichtenstein na základě svého konceptu popsal otevřenou techniku opravy tříselné kýly, která nyní nese jeho jméno a je oblíbená v chirurgické praxi. V roce 1993 se v článku Edwarda H. Phillipse poprvé objevil termín „extraperitoneální oprava kýly“. J. Dulucq však vyvinul moderní techniku ТЕР. V chirurgii tříselné kýly dnes dominují tři operace tříselné kýly bez napětí (TAPP, ТЕР a Lichtenstein) a jedna operace tříselné kýly s tenzní (Shouldice).
The article reviews the history of inguinal hernia surgery. At various times, different procedures and diverse materials were used for hernia repair. However, the effectiveness and safety of inguinal hernia repair emerged only after the anatomic features of the inguinal region had been elucidated in a monograph by Henri Fruchaud „Anatomie des hernies de l’aine” published in 1956. The Italian surgeon Edoardo Bassini began a new era in herniology. For a longtime, his classic procedure with its modifications was the most popular in surgical practice. In 1959, Lloyd M. Nyhus proposed inguinal hernia repair according to the concept of the pre-abdominal (posterior) approach that later became the basis for developing the transabdominal preperitoneal hernia repair (TAPP). In 1992, M. Arregui performed the first ТАРР using a prolene mesh. In 1986, Irving Lichtenstein proposed the concept of „tension-free repair”. Basing on his concept, Lichtenstein described an open technique of inguinal hernia repair, which now bears his name and is popular in surgical practice. In 1993, the term „extraperitoneal hernia repair” first appeared in an article by Edward H. Phillips. However, J. Dulucq developed the modern ТЕР technique. Currently, three tension-free inguinal hernia repairs (TAPP, ТЕР and Lichtenstein procedure) and one tension inguinal hernia repair (Shouldice procedure) dominate in inguinal hernia surgery.

BACKGROUND: There is a reasonable body of evidence around oral/dental health and implant infection in orthopaedic and cardiovascular surgery. Another large area of surgical practice associated with a permanent implant is mesh hernia repair. This study aimed to review the evidence around oral/dental health and mesh infection. METHODS: The research protocol was registered in PROSPERO (CRD42022334530). A systematic review of the literature was undertaken according to the PRISMA 2020 statement. The initial search identified 582 publications. A further four papers were identified from references. After a review by title and abstract, 40 papers were read in full text. Fourteen publications were included in the final review, and a total of 47,486 patients were included. RESULTS: There is no published evidence investigating the state of oral hygiene/health and the risk of mesh infection or other infections in hernia surgery. Improvement in oral hygiene/health can reduce surgical site infection and implant infection in colorectal, gastric, liver, orthopaedic and cardiovascular surgery. Poor oral hygiene/health is associated with a large increase in oral bacteria and bacteraemia in everyday activities such as when chewing or brushing teeth. Antibiotic prophylaxis does not appear to be necessary before invasive dental care in patients with an implant. CONCLUSION: Good oral hygiene and oral health is a strong public health message. The effect of poor oral hygiene on mesh infection and other complications of mesh hernia repair is unknown. While research is clearly needed in this area, extrapolating from evidence in other areas of surgery where implants are used, good oral hygiene/health should be encouraged amongst hernia patients both prior to and after their surgery.
BACKGROUND: There is an increasing number of patients following hernia surgery with implanted mesh reporting symptoms that could indicate autoimmune or allergic reactions to mesh. 'Allergy' to metals, various drugs, and chemicals is well recognised. However, hypersensitivity, allergy or autoimmunity caused by surgical mesh has not been proven by a scientific method to date. The aim of this study was twofold: to describe the pathophysiology of autoimmunity and foreign body reaction and to undertake a systematic review of surgical mesh implanted at the time of hernia repair and the subsequent development of autoimmune disease. METHODS: A systematic review using the PRISMA guidelines was undertaken. Pubmed (Medline), Google Scholar and Cochrane databases were searched for all English-written peer-reviewed articles published between 2000 and 2021. The search was performed using the keywords "hernia", "mesh", "autoimmunity", "ASIA", "immune response", "autoimmune response". RESULTS: Seven papers were included in the final analysis-three systematic reviews, three cohort studies and one case report. Much of the current data regarding the association of hernia mesh and autoimmunity relies on retrospective cohort studies and/or case reports with limited availability of cofounding factor data linked to autoimmune disease such as smoking status or indeed a detailed medical history of patients. Three systematic reviews have discussed this topic, each with a slightly different approach and none of them has identified causality between the use of mesh and the subsequent development of autoimmune disease. CONCLUSION: There is little evidence that the use of polypropylene mesh can lead to autoimmunity. A large number of potential triggers of autoimmunity along with the genetic predisposition to autoimmune disease and the commonality of hernia, make a cause and effect difficult to unravel at present. Biomaterials cause foreign body reactions, but a chronic foreign body reaction does not indicate autoimmunity, a common misunderstanding in the literature.

Úvod: Chronická bolest po plastice tříselné kýly, která je pro pacienta jednou z nejvíce obtěžujících komplikací, se vyskytuje dle literatury v 5−20 % případů. Jednou z příčin chronické bolesti může být i dráždění periferních nervů síťkou či stehem. Rizikovým faktorem chronické bolesti je již předoperační bolest. Kazuistika: Kazuistika se týká 59letého muže s chronickou bolestí v třísle po Lichtensteinově plastice tříselné kýly vlevo. Pokus o konzervativní postup fyzikální terapií nebyl úspěšný a pacientovi byla provedena trineurektomie z klasického přístupu s ponecháním původní polypropylenové síťky. Bezprostředně po operaci se mu od bolestí ulevilo a po měsíci i po roce od operačního přerušení periferních nervů třísla nepociťoval žádné bolesti. Závěr: Péče o pacienta s chronickou bolestí po plastice tříselné kýly by měla být komplexní, ideálně centralizovaná. Po selhání veškeré konzervativní léčby je indikována ev. extirpace síťky a periferní neurektomie.
Introduction: Chronic pain is a distressing complication that can occur after inguinal hernia repair, affecting between 5% and 20% of patients as reported in literature. There are several reasons for chronic pain, including peripheral nerve irritation caused by surgical mesh or stitches. Preoperative pain is a risk factor for chronic pain. Case report: We present the case of a 59-year-old man who experienced chronic inguinal pain following Lichtenstein hernia repair. Conservative therapy was ineffective, and he subsequently underwent triple neurectomy without removal of the original polypropylene mesh. The patient experienced significant pain relief immediately after the surgery. There was no reported pain 1 month and 1 year post-surgery. Conclusion: The management of patients with chronic pain following hernia repair should be comprehensive and, ideally, centralized. Conservative procedures should be attempted first, but neurectomy and mesh removal may be necessary in cases where conservative measures are unsuccessful.

Úvod: Cílem práce je analyzovat komplikace operací pro kýlu v jizvě a zhodnotit význam jednotlivých rizikových faktorů podílejících se na vzniku infekčních komplikací na Chirurgické klinice Fakultní nemocnice Královské Vinohrady za rok 2015. Metody: Analyzovali jsme 138 pacientů se zaměřením na rizikové faktory, které potenciálně zvyšují riziko vzniku pooperační infekční komplikace, jako jsou implantace síťky, kouření, přítomnost onemocnění diabetes mellitus, ischemické nemoci srdeční, malignity a imunosupresivní terapie a také akutnost operace. Výsledky: Při vícerozměrné logistické regresní analýze se akutní výkon ukázal jako rizikový faktor pro vznik infekce a při ad hoc Fisherově testu byl poměr 2,73. Diabetes mellitus byl hraničně významný faktor (p=0,071). Žádné další sledované parametry nebyly spojeny se zvýšeným rizikem SSI. Závěr: Studie prokázala jako významný rizikový faktor vzniku pooperační infekce akutní výkon. Naopak použití síťky nebylo spojeno se zvýšeným rizikem infekce.
Introduction: The aim of this study was to analyse complications associated with inicisional hernia repair surgery and to assess individual risk factors for surgical site infections occurring at the Department of Surgery of the University Hospital Kralovske Vinohrady in 2015. Methods: We analysed 138 patients with a focus on risk factors that potentially increase the risk of postoperative infectious complications such as mesh implantation, smoking, diabetes mellitus, ischemic heart disease, malignancies, immunosuppressive therapy and urgency of the procedure. Results: Based on multivariate logistic regression analysis, acute surgery proved to be a risk factor for infection, and in the ad hoc Fisher test, the ratio was 2.73. Diabetes mellitus as a risk factor reached the limit of significance (p=0.071). No other variables were associated with an increased risk of surgical site infections. Conclusion: Acute surgery was assessed as a significant risk factor for postoperative wound infection. On the contrary, mesh repair was not associated with an increased risk of infection.
Tříselné kýly představují nejčastější typ zevních kýl. Dominují u mužů, u kterých se incidence v České republice pohybuje okolo 3 %. Plastiky tříselných kýl jsou jedny z nejběžněji prováděných operací na chirurgických klinikách. V současné době dominují tzv. „tension-free“ plastiky s užitím prolenových sítěk. Vůbec nejužívanějším typem operace je otevřený přístup podle Lichtensteina. Bohužel při nesprávně provedeném zákroku zde může dojít ke komplikacím ve smyslu poranění probíhajících nervů (n. iliohypogastricus a n. ilioinguinalis), k rozvoji zánětu, vzniku hematomu, seromu. Při laparoskopickém přístupu hrozí riziko poranění semenného provazce, vnitřních orgánů, vzácně k rozvoji epididymitis. A v neposlední řadě každá operace je zatížena rizikem rozvoje chronické bolesti v třísle. Ta je definována jako oslabující bolest omezující pacienty v běžných denních činnostech po dobu 3 a více měsíců od operace. U kýl je snaha co nejvíce zabránit rozvoji silné pooperační bolesti, jejímu přechodu do chronicity a snížit užití opioidních analgetik na minimum. V tomto článku bychom rádi demonstrovali nejnovější poznatky týkající se operačního i analgoterapeutického přístupu u problematiky tříselných kýl.
Groin hernias are the most common type of ventral hernias. They occur in men mainly and the incidence is approximately three percents in the Czech Republic. It is the reason why these operations are so frequent. Nowadays, the most popular types of surgery are tension–free procedures. During these procedures we close the defect with prolene mesh. Surgeons have to be very careful not to damage the main nerves iliohypogastricus and ilioinguinalis, spermatic funniculus and internal organs especially during laparoscopic operations. Other complications are seromas, hematomas, infection and of course, chronic pain in groin. The definition is a debilitating pain in groin affecting daily activities for the course of more than three months after the operation. The prevention of chronic pain in groin is about a proper surgical approach and a use of analgesics in such a way that minimizes the dosage of opioids.

Úvod: Rozsáhlé chronické defekty a kýly břišní stěny jsou velmi častou komplikací opakovaných operačních výkonů a/nebo jejich doprovodných komplikací. Rekonstrukce břišní stěny je u těchto defektů/kýl obtížná, ideální způsob léčby je stále předmětem diskuzí. Primární sutura je technicky nemožná. Při operačním řešení je většinou nutné použití implantátu (síťky). Plastiky s použitím síťky onlay, inlay, sublay, underlay nebo IPOM mají své limity a mnohdy neuspokojivé výsledky. Technika CST (component separation technique, tj. separace složek břišní stěny) je terapeutickým přístupem umožňujícím komplexní řešení rozsáhlých defektů či kýl stěny břišní s velmi dobrými, zatím krátkodobými výsledky. TAR (transversus abdominis release) je metodou zadního přístupu v CST využívající uvolnění musculus transversus abdominis (MTA) k mobilizaci zadního listu pochvy musculus rectus abdominis (MRA). TAR zachovává MRA a jeho neurovaskulární zásobení, vytváří obrovský prostor pro inzerci síťky a umožňuje komplexní rekonstrukci vrstev břišní stěny. Kazuistika: 55letý pacient byl akutně operován pro perforovanou divertikulitidu sigmoidea s difuzní peritonitidou. Byla provedena Hartmanova operace, 7. pooperační den byl revidován pro nekrózu kolostomie a dehiscenci fascie. Po zhojení s odstupem šesti měsíců byl indikován k zanoření kolostomie. Samotný operační výkon proběhl bez komplikací, kolorektální anastomóza end to end cirkulárním staplerem. Pooperačně nastaly komplikace charakteru nitrobřišního abscesu v malé pánvi a následné destruktivní flegmony a nekrózy břišní stěny rezultující v nehojící se rozsáhlý chronický defekt stěny břišní. Po neúspěchu konzervativní léčby byl chronický defekt excidován, břišní stěna reparována metodou TAR. Závěr: TAR je akceptovatelnou metodou v řešení rozsáhlých defektů/kýl břišní stěny vykazující nízkou perioperační morbiditu a nízkou míru recidivy.
Introduction: Chronic defects and hernias of the abdominal wall are a common complication of repeated surgical procedures and/or their accompanying complications. Reconstruction of the abdominal wall is difficult in these defects/hernias and debates of an ideal method of treatment have not reached a conclusion. Primary suture is usually impossible. Onlay, inlay, sublay, underlay and IPOM plasty procedures have their limits and often lead to unsatisfactory results. CST (component separation technique) technique is a new therapeutic approach enabling a solution of large defects and hernias of the abdominal wall with very good short-term results. TAR (transversus abdominis release) is a posterior approach in CST. It releases transversus abdominis muscle (MTA) to mobilize the posterior sheath of the rectus abdominis muscle (MRA). TAR preserves MRA and its neurovascular bundle, creates a large space for mesh insertion and allows complex reconstruction of the abdominal wall. Case report: A 55-year-old patient underwent surgery for perforated diverticulitis of colon sigmoideum with diffuse peritonitis. Hartman’s operation was performed. The patient was reoperated for colostomy necrosis and fascia dehiscence on the 7th postoperative day. After healing 6 months later, colostomy occlusion was indicated. The operation itself – colorectal anastomosis using an end-to-end circular stapler – was without complications. However, complications occurred in the postoperative period including an intra-abdominal abscess in the lesser pelvis and subsequent destructive phlegmona and necrosis of the abdominal wall, resulting in a non-healing extensive chronic abdominal wall defect. After the failure of conservative treatment, the chronic defect was excised and the abdominal wall was reconstructed using the TAR method. Conclusion: TAR is an acceptable method in the treatment of large defects and hernias of the abdominal wall, associated with low perioperative morbidity and low recurrence rates.
