Haemophilia in its most severe clinical form can lead to alterations in the physical and psychosocial state with important repercussions on the quality of life. Purpose. A retrospective study was conducted to highlight the impact of haemophilic arthropathy on quality of life. Materials. We considered 25 patients, with a mean age of 42 years (min 17 - max 71) with haemophilia A, 18 had the severe clinical form (72%). The WFH Physical Examination Score, specific for haemophilia, was used for joint function; the joints examined were: knee, ankle, hip and elbow. To assess the quality of life, two generic self-filling questionnaires were used, SF-36 and the EQ 5D. Results. Significant statistical values have shown that arthropathy affects the SF-36 domain of general health and the subjective well-being EQ-VAS. Conclusions. In the treatment of haemophilic arthropathy, prevention is essential, understood both as a prophylactic medical therapy and as a physiotherapeutic treatment in order to maintain or improve joint function and at the same time play a fundamental role in improving the quality of life of patients.

L’emofilia nella forma clinica più severa può portare ad alterazioni dello stato fisico e psicosociale con importanti ripercussioni sulla qualità della vita. Scopo. È stato condotto uno studio retrospettivo per evidenziare quanto incide l’artropatia emofilia sulla qualità della vita. Materiali. Sono stati considerati 25 pazienti, di età media di 42aa (min17-max71) affetti da emofilia A, 18 presentavano la forma clinica severa (72%). Per la funzionalità articolare è stata utilizzata la scala di valutazione WFH Physical Examination Score, specifica per l’emofilia; le articolazioni prese in esame sono state: ginocchio, tibio-tarsica, anca e gomito. Per la valutazione della qualità della vita sono stati utilizzati due questionari generici auto compilativi, SF-36 e l’EQ 5D. Risultati. Valori statistici significativi hanno dimostrato che l’artropatia influenza il dominio SF-36 della salute generale e lo stato di benessere soggettivo EQ-VAS. Conclusioni. Nel trattamento dell’artropatia emofilica risulta fondamentale la prevenzione intesa sia come terapia medica profilattica, sia come trattamento fisioterapico al fine di mantenere o migliorare la funzione articolare e nello stesso tempo avere un ruolo fondamentale nel migliorare la qualità di vita dei pazienti.

With about 65000 volumes, the Roudnice Lobkowicz Library, recently restituted to the owner and reconstructed at Nelahozeves Castle, is one of the greatest aristocratic libraries of Middle Europe. The library holds an extremely rich collection of medical books, starting from manuscripts and incunabula. Following up the development of studies started in the last years in the Library focused on provenances, collecting and with the reception of books throughout the centuries, the research is a survey of the medical literature present in the library. It is an attempt to individuate the kind of edition as well as to determinate the origin of the items, to explain their presence according to the different provenances and possibly their use through the different periods. As result, is possible to determinate models of reception of medical books in aristocratic libraries.

Overhead traction in the treatment of congenital dislocation of the hip

Long-term results of traction treatment for congenital hip dislocation (CDH) performed between 1984 and 1990 are presented. The new traction scheme consists of 4 distinctive phases. It is based on gradually changed direction of traction--first, from the longitudinal direction to flexion and abduction against the hypomochilion. The traction is applied by means of non irritable adhesive tape using 1/4-1/5 of the child's body weight. The period of active traction is followed by retention period in Hanausek's biomechanical device. The success rate of the scheme, in terms of reduction, was 90 per cent. In 31 per cent the Salter innominate osteotomy was performed because of residual deformity. In 10 per cent of the cases open reduction was necessary.
