BACKGROUND: Pernicious anaemia is an autoimmune disease that causes acquired vitamin B12 deficiency. The diagnostic process includes the detection of typical changes in the blood count, low serum levels of vitamin B12, endoscopic and histological signs of gastritis and autoantibodies against the gastric parietal cells antigen H+/K+ ATPase and intrinsic factor. OBJECTIVE: Our aims were to establish immunological tests for the detection of autoantibodies against intrinsic factor and target gastric parietal cell antigen H+/K+ ATPase and to evaluate their diagnostic benefits in patients with pernicious anaemia. MATERIAL AND METHODS: Sera from 95 patients were tested for autoantibodies against H+/K+ ATPase and intrinsic factor by multiplex Luminex assay. The results were compared with those of the immunofluorescence assay for the detection of autoantibodies against gastric parietal cells and with the diagnostic criteria. RESULTS: The autoantibodies against gastric parietal cell H+/K+ ATPase had a sensitivity of 68.2% with a specificity of 91.7% for the diagnosis of pernicious anaemia. The respective rates for the autoantibodies against intrinsic factor were 40.9% and 98.6%. The combined sensitivity and specificity rates for both autoantibodies were 86.36% and 90.28%, respectively, the combined positive predictive value was 73.08% and the combined negative predictive value was 95.59%. CONCLUSION: The detection of both autoantibodies is helpful in diagnosing pernicious anaemia and the combination of the two assays increases diagnostic sensitivity.

Autoimmune thyropathies are frequently linked to many infections, such as Helicobacter pylori, which are also supposed to play a role in their pathogenesis. The aim of this study was to evaluate the relationships between thyroid and gastric autoimmunity and H. pylori infection on a large sample of Czech population (n=1621) by monitoring the autoantibodies against thyroglobulin (anti-Tg) and thyroid peroxidase (anti-TPO) and gastric parietal cell (anti-GPC, representing thyrogastric syndrome) in correlation with antibodies against Helicobacter pylori (anti-H. pylori) of classes IgG and IgA. The interrelation between autoantibodies and H. pylori antibodies was assessed by H. pylori seropositivity. In H. pylori seropositive persons as compared to seronegative irrespective of age and sex, a higher occurrence of anti-TPO (10.4 % vs. 5.8 %, p=0.001) and anti-GPC (6.1 % vs. 1.7 %, p<0.001) was found. Differences in anti-TPO occurrence were significant in both men (7.0 % vs. 3.3 %, p=0.03) and women (12.7 % vs. 8.0 %, p=0.02), differences in anti-GPC occurrence were significant only in women (7.2 % vs. 1.7 %, p<0.001). Results of this study support the idea of a connection between infection of H. pylori and the occurrence of anti-TPO autoantibodies representing thyroid autoimmunity and gastric parietal cells autoantibodies representing the thyrogastric syndrome.

The authors examined antibodies against antigen of gastric parietal cells using the method of reaction of complement-binding and the immunofluorescence test in 662 patients. Concurrently they examined in 443 patients of this group also antibodies against thyroglobulin. The group comprised 312 men and 350 women. For statistical evaluation the chi 2 test was used. Antibodies against the antigen of parietal cells were positive at the level of significance p less than 0.05 in the group with other diseases, at the level of significance of p less than 0.01 in dyspeptic syndrome, and at the level of significance p less than 0.001 in hypothyroidism and in chronic hepatopathies. The immunofluorescence test was statistically significant at the level of p less than 0.01 only in duodenal ulceration, at the level of p less than 0.001 in dyspeptic syndrome, hypothyroidism, hyperthyroidism, chronic hepatopathies, chronic pancreatitis, cholecystopathies, nephropathies and in the group of other diseases. Thyroid antibodies were positive with titres of p less than 1: 50 in 5.63, with titres of p less than 1: 5000 in 3.03%. The mentioned disease have not only pathomorphological and aetiological similarities but there is also serological overlapping.
