UNLABELLED: Disease phobia refers to a psychological state when the person continuously thinks that he/she is sick and improvement from the condition is impossible. Disease phobia in patients suffering from pain, secondary to temporomandibular disorders (TMDs), is usually the consequence of long-term problems; diagnosis and treatment of this group is a real challenge for healthcare professionals. AIM: The purpose of this prospective study was to objectively evaluate the role of Illness Attitude Scale (Kellner or IAS) in measuring cancerophobia and heart disease phobia in patients suffering from pain, as a consequence of TMDs. SUBJECTS AND METHODS: The cohort included 22 patients with TMDs who underwent evaluation of these phobias; pain was acute in 7 and chronic in 15. The patients were asked to complete the "Kellner" questionnaire, and this was followed by full clinical examination of the temporomandibular region. RESULTS: When measuring the correlation between the cancerophobia and heart disease phobia patients, the outcome was found significant in the total cohort, p < 0.01. Comparisons were carried out in the chronic group (n = 15) and was significant (p = 0.034 and r = 0.549); while in the acute group no significance was identified. CONCLUSION: Cancerophobia and heart disease phobia in TMD patients are factors that need to be taken in consideration when managing chronic pain in this group.

The paper has analysed hypochondriasis in terms of defence behaviour. The study sample consisted of 39 treated neurotics who felt themselves menaced by a serious disease which was not confirmed by appropriate medical examination and who came to the therapist with anxious states. Specific defence acts induced by hypochondriacal threat, stimuli inducing the states of threat and mechanisms responsible for formation and maintenance of their capability for inducing emotion were found in every patient. The results demonstrated that many manifestations of hypochondriasis considered generally as psychopathological symptoms were in fact meaningful defence responses against a threat which the patient believed existed.

The author draw attention to psychological aspects of subjective somatic complaints of neither functional nor organic origin. They stress the role of subjective somatic complaints played in the patient's life. The process of somatization is clarified. It is difficult for the doctor to understand subjective somatic complaints as he is predominantly used to diagnose diseases from the biological viewpoint. The paper gives more detailed characterization of hypochondria using alternative theories to explain it. It also briefly mentions essential characteristics of larvate depression and somatized anxiety.

Pimozide was tried in the treatment of delusion of parasitosis (a type of monosymptomatic hypochondriacal psychosis), a syndrome that is found with increasing frequency. Ten patients were selected on the basis of unambiguously defined psychopathological criteria. Pimozide therapy was assessed by means of placebo control and the double-blind method using a rating scale evolved for this particular therapy. The results have quite definitely proved the effect of small doses (two to eight mg per day) of pimozide compared to the administration of a placebo. The very few side-effects observed could easily be influenced. The dynamics of the regression of the psychopathological symptoms suggest that the efficacy of pimozide therapy is decisively linked to the central dopamine receptor blocking effect of the drug.
