The authors describe a group of 13 patients where during treatment of aseptic necrosis of the head of the metatarsal bone a new surgical method was used--wedge-shaped resection of the head. They describe the aetiology, surgical technique and results. This surgical technique is used at the Orthopaedic Clinic of the Institute for Postgraduate Medical Training Prague-Bulovka since 1987. In the discussion the authors evaluate the disadvantages of hitherto used surgical techniques and compare the methods with the method they use. In the conclusion the achieved results are evaluated which justify wider use of the described method in routine practice of orthopaedic departments.

Transplantation of a vascular bundle into an avascular spongiosa replant and the head of the femur after interruption of its blood supply was carried out in experiment (66 experiments on 34 dogs). The periods of observation reached 72 weeks. The mechanisms of revascularization of bones devoid of vessels and the results of morphometric analysis were investigated. It has been established that revascularization of a spongiosa replant sets in after 3 weeks and that of the femoral head after 12-16 weeks. Clinical observations (69 cases) were concerned with transplantation of a vascular bundle in the treatment of patients with aseptic osteonecrosis.

The authors present a group of 49 patients treated over the period of ten years for the Osgood-Schlatter disease. Conservative treatment was employed in 5 patients and 44 patients were treated surgically by the excision of the loose fragment of apophysis. On the basis of the evaluation of the data the authors state that on average 40 months after operation there was no improvement of the condition in as many as 34 per cent of patients and they were not satisfied with the result of operation. After the analysis of the potential causes of failures they stress the necessity of deliberate indication for operation and the priority of the conservative treatment.
