BACKGROUND: To date, no longitudinal study using a compositional approach has examined sedentary behavior (SB) patterns in relation to adiposity in the pediatric population. Therefore, our aims were to (1) investigate the changes in SB patterns and adiposity from childhood to adolescence, (2) analyze the prospective compositional associations between changes in SB patterns and adiposity, and (3) estimate the changes in adiposity associated with substituting SB with physical activity (PA) of different intensities. METHODS: The study presents a longitudinal design with a 5-year follow-up. A total of 88 participants (61% girls) were included in the analysis. PA and SB were monitored for seven consecutive days using a hip-worn accelerometer. Adiposity markers (fat mass percentage [FM%], fat mass index [FMI], and visceral adiposity tissue [VAT]) were assessed using the multi-frequency bioimpedance analysis. The prospective associations were examined using compositional data analysis. RESULTS: Over the follow-up period, the proportion of time spent in total SB increased by 154.8 min/day (p < 0.001). The increase in total SB was caused mainly by an increase in middle and long sedentary bouts, as these SB periods increased by 79.8 min/day and 62.0 min/day (p < 0.001 for both), respectively. FM%, FMI, and VAT increased by 2.4% points, 1.0 kg/m2, and 31.5 cm2 (p < 0.001 for all), respectively. Relative to the remaining movement behaviors, the increase in time spent in middle sedentary bouts was significantly associated with higher FM% (βilr1 = 0.27, 95% confidence interval [CI]: 0.02 to 0.53) at follow-up. Lower VAT by 3.3% (95% CI: 0.8 to 5.7), 3.8% (95% CI: 0.03 to 7.4), 3.9% (95% CI: 0.8 to 6.9), and 3.8% (95% CI: 0.7 to 6.9) was associated with substituting 15 min/week spent in total SB and in short, middle, and long sedentary bouts, respectively, with an equivalent amount of time spent in vigorous PA. CONCLUSIONS: This study showed unfavorable changes in SB patterns and adiposity status in the transition from childhood to adolescence. Incorporating high-intensity PA at the expense of SB appears to be an appropriate approach to reduce the risk of excess adiposity in the pediatric population.

INTRODUCTION: Only international studies can provide the full variability of built environments and accurately estimate effect sizes of relations between contrasting environments and health-related outcomes. The aims of the International Physical Activity and Environment Study of Adolescents (IPEN Adolescent) are to estimate the strength, shape and generalisability of associations of the community environment (geographic information systems (GIS)-based and self-reported) with physical activity and sedentary behaviour (accelerometer-measured and self-reported) and weight status (normal/overweight/obese). METHODS AND ANALYSIS: The IPEN Adolescent observational, cross-sectional, multicountry study involves recruiting adolescent participants (ages 11-19 years) and one parent/guardian from neighbourhoods selected to ensure wide variations in walkability and socioeconomic status using common protocols and measures. Fifteen geographically, economically and culturally diverse countries, from six continents, participated: Australia, Bangladesh, Belgium, Brazil, Czech Republic, Denmark, Hong Kong SAR, India, Israel, Malaysia, New Zealand, Nigeria, Portugal, Spain and USA. Countries provided survey and accelerometer data (15 countries), GIS data (11), global positioning system data (10), and pedestrian environment audit data (8). A sample of n=6950 (52.6% female; mean age=14.5, SD=1.7) adolescents provided survey data, n=4852 had 4 or more 8+ hours valid days of accelerometer data, and n=5473 had GIS measures. Physical activity and sedentary behaviour were measured by waist-worn ActiGraph accelerometers and self-reports, and body mass index was used to categorise weight status. ETHICS AND DISSEMINATION: Ethical approval was received from each study site's Institutional Review Board for their in-country studies. Informed assent by adolescents and consent by parents was obtained for all participants. No personally identifiable information was transferred to the IPEN coordinating centre for pooled datasets. Results will be communicated through standard scientific channels and findings used to advance the science of environmental correlates of physical activity, sedentary behaviour and weight status, with the ultimate goal to stimulate and guide actions to create more activity-supportive environments internationally.

The study aimed to adapt the standardized Physical Activity Questionnaire for Older Children (PAQ-C) into the Czech language and assess its psychometric properties. A forwards-and-backwards translation method was carried out to prepare the Czech PAQ-C version (PAQ-C/CZ), followed by adjustments based on socio-cultural aspects. In the next phase, participants (n = 169) wore accelerometers for seven days. On the 8th day, participants completed the PAQ-C/CZ in school so that it was possible to determine the concurrent validity (correlation between the accelerometer and PAQ-C/CZ data, Spearman's r), internal consistency (Cronbach's alpha), item/scale properties (corrected item-total correlations, CITCs), and factor structure (exploratory factor analysis, EFA) for PAQ-C/CZ. In the last phase, participants (n = 63) completed the PAQ-C/CZ twice on two consecutive school days in the morning to determine the test-retest reliability (intraclass correlation coefficient, ICC; independent sample T-test). The PAQ-C/CZ indicated moderate internal consistency (alpha = 0.77), acceptable item/scale properties (CITCs = 0.29-0.61), and good test-retest reliability (ICC = 0.73-0.94). The EFA findings suggested a single factor model (factor load = 0.32-0.73) for PAQ-C/CZ, with items 2, 3, and 4 contributing low to the total score. Results on concurrent validity showed low but significant association (r = 0.28, p < 0.05) between accelerometer and PAQ-C/CZ data. Considering the study results, the PAQ-C/CZ can be recommended as a tool for moderate-to-vigorous physical activity assessment in large-sample research studies only, but with an emphasis on the interpretation of the correct results.

BACKGROUND: Physical activity (PA) in adolescence is crucial for lifelong healthy lifestyle, and attention is needed to adolescents at health risk due to insufficient PA. This study investigates the composition of weekly PA in adolescents by PA level and provides a rationale for change in their lifestyles. METHODS: The research was conducted at 66 schools in Poland between 2009 and 2016, among 949 girls and 650 boys aged 15-18 years. We used pedometers to monitor weekly PA with data assessed using the Indares web app. The participants were split into three groups by mean daily step count (lower, < 9000; middle, 9000-12,999; and higher, ≥ 13,000 steps/day), as a reliable and non-expensive indicator of PA. RESULTS: We did not observe statistically significant differences in composition of 7-day PA between participants with varying levels of PA, regardless of their gender (F(12,9558) = 0.60; p = 0.841; ηp2 > 0.000). The smallest differences in daily step counts by PA level were found on Mondays and the biggest on Fridays and Saturdays, in boys and girls; the differences between average school and average weekend days were most pronounced in less active girls (1677 steps/day) and boys (1886 steps/day). During the school week, the highest proportion of less active girls met the 11,000 steps/day recommendation on Fridays (21.9%), statistically significantly less than on other school days (p < 0.001). Similarly, less active boys (22.2%) had significantly less activity on Fridays than on other school days, except for Mondays (p = 0.143) Analogous pattern was apparent also in more active adolescents. CONCLUSIONS: Less active adolescents have comparable composition of weekly PA to the more active ones but they can hardly meet the generally accepted PA recommendations. Better understanding of weekly PA composition and rates of meeting PA recommendations by day of the week can lead to more efficient interventions improving lifestyles. The recommendation of 9000 steps/day most days of the week, thus, appears appropriate for less active adolescents, as a motivating achievable goal.

BACKGROUND: Although it is accepted that parents play a key role in forming children's health behaviours, differences in parent-child physical activity (PA) have not previously been analysed simultaneously in random samples of families with non-overweight and overweight to obese preschool and school-aged children. This study answers the question which of the health-related parental indicators (daily step count (SC), screen time (ST), and weight status and participation in organized leisure-time PA) help their children achieve the step count recommendations. METHODS: A nationally representative sample comprising 834 families including 1564 parent-child dyads who wore the Yamax Digiwalker SW-200 pedometer for at least 8 h a day on at least four weekdays and both weekend days and completed a family log book (anthropometric parameters, SC, and ST). Logistic regression analyses were used to investigate whether parental achievement of the daily SC recommendation (10,000 SC/day), non-excessive ST (< 2 h/day), weight status, and active participation in organized PA were associated with children's achievement of their daily SC (11,500 SC/day for pre-schoolers and 13,000/11,000 SC/day for school-aged boys/girls). RESULTS: While living in a family with non-overweight parents helps children achieve the daily SC recommendation (mothers in the model: OR = 3.50, 95% CI = 2.29-5.34, p < 0.001; fathers in the model: OR = 2.41, 95% CI = 1.37-4.26, p < 0.01) regardless of their age category, gender, or ST, for families with overweight/obese children, only the mother's achievement of the SC recommendations and non-excessive ST significantly (p < 0.05) increase the odds of their children reaching the daily SC recommendation. The active participation of children in organized leisure-time PA increases the odds of all children achieving the daily SC recommendations (OR = 1.80-2.85); however, for overweight/obese children this remains non-significant. The participation of parents in organized leisure-time PA does not have a significant relationship to the odds of their overweight/obese or non-overweight children achieving the daily SC recommendations. CONCLUSIONS: The mother's health-related behaviours (PA and ST) significantly affect the level of PA of overweight/obese preschool and school-aged children. PA enhancement programmes for overweight/obese children cannot rely solely on the active participation of children in organized leisure-time PA; they also need to take other family-based PA, especially at weekends, into account.

There is no general consensus regarding which accelerometer cut-off point (CoP) is most acceptable to estimate the time spent in moderate-to-vigorous physical activity (MVPA) in children and choice of an appropriate CoP primarily remains a subjective decision. Therefore, this study aimed to analyze the influence of CoP selection on the mean MVPA and to define the optimal thresholds of MVPA derived from different accelerometer CoPs to avoid overweight/obesity and adiposity in children aged 7 to 12 years. Three hundred six children participated. Physical activity (PA) was monitored for seven consecutive days using an ActiGraph accelerometer (model GT3X) and the intensity of PA was estimated using the five most frequently published CoPs. Body adiposity was assessed using a multi-frequency bioelectrical impedance analysis. There was found a wide range of mean levels of MVPA that ranged from 27 (Puyau CoP) to 231 min∙d-1 (Freedson 2005 CoP). A receiver operating characteristic curve analysis indicated that the optimal thresholds for counts per minute (cpm) and MVPA derived from the Puyau CoP was the most useful in classifying children according to their body mass index (BMI) and fat mass percentage (FM%). In the total sample, the optimal thresholds of the MVPA derived from the Puyau CoP were 22 and 23 min∙d-1 when the categories based on BMI and FM%, respectively, were used. The children who did not meet these optimal thresholds had a significantly increased risk of being overweight/obese (OR = 2.88, P < 0.01) and risk of having excess fat mass (OR = 2.41, P < 0.01). In conclusion, the decision of selecting among various CoPs significantly influences the optimal levels of MVPA. The Puyau CoP of 3 200 cmp seems to be the most useful for defining the optimal level of PA for pediatric obesity prevention.

The pedometer is a widely used research tool for measuring the level and extent of physical activity (PA) within population subgroups. The sample used in this study was drawn from a population of university students to examine the influence of the monitoring interval and alternate starting days on step-count activity patterns. The study was part of a national project during 2008-2010. Eligible subjects (641) were selected from a sample of 906 university students. The students wore pedometers continuously for 7 days excluding time for sleep and personal hygiene. Steps per day were logged on record sheets by each student. Data gathering spanned an entire week, and the results were sorted by alternate starting days, by activity for an entire week, by activity for only the weekdays of the one-week monitoring interval and for the two-day weekend. The statistical analysis included ANOVA, intra-class correlation (ICC) analysis, and regression analysis. The ICC analysis suggested that monitoring starting on Monday (ICC = 0.71; 95%CI (0.61-0.79)), Tuesday (ICC = 0.67; 95%CI (0.59-0.75)) or Thursday (ICC = 0.68; 95%CI (0.55-0.79)) improved reliability. The results of regression analysis also indicated that any starting day except Sunday is satisfactory as long as a minimum of four days of monitoring are used.
