Last 40 years of growing knowledge of the mechanisms of cardiac arrhythmias has unambiguously demonstrated therapeutic superiority of non-pharmacological treatment over antiarrhythmic drugs, which are in these days rather used in acute phase or moderate forms of tachyarrhythmias. Diagnosis of bradyarrhythmias relies mainly on bradycardia symptoms, bradycardia detection on the surface ECG and their mutual time correlation. Electrophysiological study stands aloof in these cases. Permanent cardiac stimulation dominates unequivocally in the therapy of bradyarrhythmias. Electrophysiological study has taken a unique place in the tachycardia therapy, since it allows inducing reproducibly tachyarrhythmia and by means of its mapping to determine its course. Thus, a completely new, "intracardiac" view on the arrhythmia mechanism has evolved and considerably weakened the diagnostic importance of the surface ECG. Radiofrequency catheter ablations represent a rapidly evolving causal therapeutic modality of tachyarrhythmias, especially in the absence of structural heart disease. Significantly decreased left ventricular ejection fraction combined with tachycardia symptoms should raise suspicion of life threatening ventricular arrhythmias, which are nowadays successfully treated by implantation of a cardioverter-defibrillator.

Antiarrhythmic therapy of ventricular arrhythmias in patients after myocardial infarction, even if it reduces the incidence of arrhythmias, does not influence the mortality, as demonstrated in the American CAST study where the most effective antiarrhythmic drugs class Ic, encainide and flecainide, were used. As a result of proarrhythmogenic action therapy may even enhance the risk. The authors present a classification of ventricular arrhythmias and recommend therapeutic strategies. They draw attention to the fact that the proarrhythmogenic action may develop in the early as well as in the late stage of treatment. Beta-blockers still remain the only group of drugs which markedly reduce the incidence of sudden death in patients after myocardial infarctions.

The authors present a review of optimal treatment and prevention of dysrhythmias in the acute stage of myocardial infarction (AIM). They emphasize the necessity to shorten the pre-hospitalization stage of the disease (the prevalence of malignant dysrhythmias culminates) and elaborate in every hospital region an optimal plan of emergency care (ensure CPR, electrical defibrillation, pharmacological prevention of VT and VF). They draw attention to the fundamental importance of the therapeutic limitation ensuing from the size of the focus which implies at the same time prevention and treatment of an impaired cardiac rhythm. The authors recommend early recanalization treatment with subsequent anti-thrombocytic treatment, and if contraindications are absent, standard administration of beta blockers in the early stage of AIM. They draw attention to the asset of anti-arrhythmic treatment as well as to its undesirable effects. They discuss also indications and complications of temporary transvenous cardiac pacing.
