Viry, bakterie, a plísně mohou podstatně ovlivnit průběh astmatu a alergických onemocnění. Viry tak mohou způsobovat zánět dýchacích cest, bronchiální hyperreaktivitu a být spouštěčem exacerbací astmatu. Chronický zánět dýchacích cest při astmatu může způsobit zvětšení odpovědi na respirační virovou nákazu. Vztah mezi infekcí Chlamydia nebo Mykoplasma pneumoniae a astmatem je také prokázán, tyto bakterie mohou ovlivnit tíži onemocnění. Není vyřešeno, zda hrají roli také jako původci astmatu. Makrolidová antibiotika mohou zlepšit kontrolu astmatu, není však jasné, zda k tomuto účinku dochází více cestou jejich antimikrobiální aktivity nebo protizánětlivým působením. Antigeny a superantigeny Staphylococcus aureus, stejně jako endotoxin, mají také prozánětlivý účinek. Vztah mezi expozicí alergenu a endotoxinu vytváří u astmatiků vzájemně potenciační účinek na zánětlivou odezvu. Téma tohoto sdělení bývá ve vztahu k praxi neprávem opomíjené, přestože právě zde může mít zásadní význam.

Microbes in the form of bacteria, viruses and fungi can have a profound impact on the course of asthma and allergic diseases. Viruses, through cytopathic effect, can induce airway inflammation and respiratory symptoms. Chronic airway inflammation causes amplification of the response to common cold infection. Evidence has emerged of a connection between infection with Chlamydia or Mycoplasma pneumoniae and asthma. It is not clear if these bacteria have a role in the iniciation of asthma, there is rather more evidence that they may influence the severity of asthma. Macrolide antibiotics improve asthma control but it is not clear if this improvement is due to their antimicrobial activity or their antiinflammatory actions. Stafylococcus aureus antigens and superantigens as the endotoxins are proinflammatory substances, too. The iteraction between allergen and endotoxin exposure in asthmatic subjects suggests that a prior allergen challenge significantly augments the endotoxin induced inflammation.
