Urodynamická stresová inkontinence je definována jako nechtěný únik moči při zvýšení intraabdominálního tlaku za nepřítomnosti detruzorových kontrakcí. Tento typ inkontinence moči může zlepšit či napravit řada operačních metod, které indikujeme po kompletním urogynekologickém vyšetření. Jaký typ operace bude proveden, záleží těž na celkovém fyzickém a psychickém stavu starších pacientek. Pokud je dobrý, pak je operační léčba stresového typu inkontinence moči obdobná jako u žen mladších. Pokud však starší pacientka trpí řadou dalších onemocnění, je volba správně léčby velmi obtížná, často i proto, že pacientku nemůžeme kompletně vyšetřit. Jednou z nejčastěji prováděných operací u starších žen je metoda, která užívá k podpoře uretry polypropylenovou pásku. Je to zákrok minimálně invazivní a operační čas je zde krátký. V poslední době byly nejčastěji prováděny 2 typy minimálně invazivní páskové operace TVT (the tension-free vaginal tape; tahuprostá poševní páska) and TOT (the tension - free obturator tape; tahuprostá obturátorová páska). Později byl popsán nový postup v této operační technice „zevnitř-ven" (TVT - O). Tato operační technika snižuje riziko perforace močového měchýře, jakož i poškození střev a velkých cév. V současností začínáme s novou páskovou operací s TVT S-metodou (tahuprostá poševní páska - bezpečná metoda)

This type of urinary inconúnence can be improved and corrected by a number of surgical procedures, which are indicaied after an overall uro-gynecological examination. For elderly patients, the type of operation to be carried out also depends on overall physical and psychical condition. If this is good, the surgical treatment of stress incontinence is similar to that applied to young women. If, however, the patient suffers from a number of other diseases, it is quite difficult to select the most suitable treatment, often also because we cannot examine the patient to a sufficient degree. The operation that is carried out most t often for elderly patients, being the one that involves minimum invasion and a very short operation time, is a surgical procedure using a polypropylene tape (sling) to support the urethra. Two minimally invasive sling procec^dures have been used recently: TVT (tension-free vaginal tape) and TOT (tension-free obturator tape). Lately a a new approach has been described, an „inside-out" technique (TVT - O). This technique has reduced the risk of bladder perforation and injuries to the bowels and large vessels. At present we are starting to work with a new ^ sling procedure, the TVT S-System (tension-free vaginal tape secure system).
