Kontaktní fixace se úspěšně používají v léčbě syndromu diabetické nohy. Byl popsán jejich pozitivní vliv na hojení dia-betických ulcerací, Charcotovy osteoarthropatie i neuropatic-kých fraktur. Jako každá léčebná metoda má i aplikace kontaktních fixací své výhody, nevýhody a komplikace. Za nejzávažnější komplikace považujeme progresi lokálního nálezu a rozvoj osteomyelitidy. Ovšem podle výsledků našich studií lze kontaktní fixace s úspěchem využít v léčbě specifických případů osteomyelitidy. Proto osteomyelitida není absolutní kontraindikací léčby kontaktní fixací, jak bylo doposud uváděno v guidelines pro léčbu syndromu diabetické nohy.

Total contact casts are usually used successfully in the treatment of the diabetic foot. Their positive effects on hea-ling of diabetic foot ulcers, Charcot osteoarthropathy and neuropathic fractures were well described. Similarly to ot-her medical methods, the application of total contact casts has its advantages, disadvantages and complications. Based on the results of our studies the total contact casts could be applied with achievements in specific cases of osteomyelitis therapy. Therefore osteomyelitis is not an absolute contrain-dication of total contact cast treatment as has been previ-ously recommended by the diabetic foot therapy guidelines.
