Fondaparinux je jedním z novějších parenterálních antikoagulancií. V současné době je jeho účinek a stejně tak i bezpečnost potvrzena klinickými studiemi v celé řadě indikací. Fondaparinux je syntetický pentasacharid, který nepřímo inhibuje v koagulační kaskádě aktivovaný faktor X. Ve srovnání s nefrakciovaným heparinem (UFH) a nízkomolekulárními hepariny (LMWH) neovlivňuje aktivitu trombinu. Výhodou je dlouhý poločas eliminace, který umožňuje aplikaci v jedné denní dávce, nevýhodou je zejména clearance závislá zcela na funkci ledvin. Lék je indikován v prevenci tromboembolické nemoci (TEN) u ortopedických a dalších chirurgických pacientů, u nechirurgicky nemocných s vysokým rizikem trombembolizmu, v léčbě hluboké žilní trombózy a jako alternativní léčba u akutních koronárních syndromů. Výhodnou alternativou je také u pacientů s rozvojem nebo anamnézou heparinem indukované trombocytopenie 2. typu (HIT 2), kdy jsou UHF i LMWH kontraindikovány.

Fondaparinux is a new drug which belongs to parenteral anticoagulants. The efficacy and safety of this drug were proven in numerous clinical trials for several indications. Fondaprinux is a synthetic and specific inhibitor of activated factor X in the coagulation cascade. In comparison to unfractionated heparin (UFH) and low molecular weight heparins (LMWH), fondaparinux does not interact with thrombin. Its main advantage is the long half time which allows once daily administration. On the other hand, the main disadvantage is the high renal dependence of its clearance. The drug is indicated for prophylaxis of thromboembolic disease in patients undergoing major orthopedic surgery or other major surgery and in non-surgical patients with high risk of thromboembolic complications. Fondaparinux is also an alternative therapy for patient with pulmonary embolism, deep venous thrombosis and acute coronary syndromes in specific clinical settings. Hence, it is also a good option for patients having heparin induced thrombocytopenia where both LMWH and UFH are contra-indicated.
