Akutní disekce aorty je život ohrožující stav s vysokou mortalitou. Jedním z mnoha rizikových faktorů tohoto onemocnění je i fyziologické těhotenství. Nutné je včasné stanovení diagnózy a adekvátní terapie podle potřeb obou organismů. Kazuistika popisuje případ těhotné v 33. týdnu těhotenství s disekcí typu A a postup během celkové anestezie. V první době byl proveden císařský řez k vybavení plodu, poté výkon na ascendentní aortě.

Acute aortic dissection is a life-threatening condition with high mortality rate. One of the many risk factors of this disease is the physiological pregnancy. It is necessary to make early diagnosis and start adequate treatment according to the needs of both the mother and the phoetus. We present a case of a pregnant woman at 33 weeks of gestation with type A dissection, and the management of general anaesthesia. Following a Caesarian Section, the ascending aorta was repaired.
