Kazuistika ukazuje na hrozbu hypoglykemie u 76leté aktivní diabetičky 2. typu, léčené metforminem a gliklazidem, u které byla navyšována medikace perorálními antidiabetiky pro zhoršení kompenzace diabetu. Cílem bylo dosáhnout co nejlepší kompenzace diabetu. Místo zlepšení však docházelo k četnějším hypoglykemiím a tím také zhoršení kvality života pacientky. Pacientka žila velmi aktivním životem, příležitostně pracovala jako vychovatelka ve školní družině, hlídala svá tři vnoučata. Pro časté ná- valy nervozity a třesu se však obávala další budoucnosti. Většina z nás diabetologů si je tohoto rizika vědoma, proto léčbu vede individuálně s přihlédnutím na životní styl nemocného. Velké zlepšení přineslo vysazení sulfonylurey z léčby a nasazení dlouhodobě působícího inzulínového analoga glargin (Abasaglar).

This case report demonstrates a risk of hypoglycemia in a 76 years old active patient with the type 2 diabetes mellitus treated with metformin and gliclazide, in whom the medication with oral antidiabetic agents was enhanced due to a worsened compensation of diabetes. The aim was to achieve the best possible compensation of diabetes. Instead of the improved condition the patient suffered from more frequent hypoglycemic episodes and consequently worsened quality of life. The patient led an active lifestyle, she occasionally worked as an educator in a school club, she took care about her three grandchildren. Due to frequent attacks of anxiety and tremor she was worried about her future. Many of us, diabetologists, are aware of such risks therefore we manage the treatment individually considering patients’ lifestyles. A withdrawal of sulphonylurea and the initiation of a treatment with a long-acting insulin analogue glargine (Abasaglar) resulted in a big improvement.
