Cíl: Uvést naše zkušenosti s downstagingem hepatocelulárního karcinomu (HCC) u pacientů původně překračujících obecně uznávaná kritéria pro transplantaci jater. Metoda. V letech 2013-2017 bylo 13 pacientů s HCC překračujícím milánská i UCSF kritéria k transplantaci léčeno transarteriální chemoembolizací s cílem downstagingu. U všech nemocných byla provedena drug-eluting bead superselektivní chemoembolizace, do přívodně tepny tumoru byly aplikovány částice DC bead 100-300 μm s navázaným doxorubicinem. Výsledky: Downstaging byl úspěšný u všech nemocných. Odpověď nádoru na embolizaci byla hodnocena dle modifikovaných RECIST kritérií na CT, všichni pacienti splňovali Milánská kritéria a byli zařazeni na čekací listinu k transplantaci. Deset nemocných bylo úspěšně transplantováno. Závěr: Drug-eluting bead TÁCE je vhodnou metodou pro dosažení úspěšného downstagingu do Milánských kritérií u vybraných pacientů.

Aim: Presentation of our experience with downstaging of hepatocellular carcinoma (HCC) in patients originally exceeding current liver transplant criteria. Method: Between 2013 and 2017 thirteen patients with HCC exceeding Milan and UCSF liver transplant criteria were treated by transarterial chemoembolization (TACE) as downstaging method. Superselective embolization with drug-eluting beads was performed in all patients, doxorubicin loaded DC beads 100-300 μm were applied to tumor feeding artery. Results: Downstaging was successful in all patients. Tumor response to embolization was evaluated by modified RECIST criteria on CT, all patients met Milan criteria and joined the waiting list for transplantation. Ten patients were successfully transplanted. Conclusion: Drug-eluting bead TACE is acceptable method to achieve successfull downstaging of HCC to Milan criteria in selected patients.
