Kazuistika popisuje úpravu léčby u diabetika 2. typu ve středním věku, u něhož zave-dená terapie glarginem 100 U/ml s kombinací perorálních antidiabetik (repaglinid,metformin, sitagliptin) nevedla ke kýženým parametrům kompenzace. Po vysazenísitagliptinu a sdružení glarginu do fixní kombinace s lixisenatidem došlo již běhemkrátké doby k zásadnímu zlepšení glykemií bez obtěžujících hypoglykemií, opomenutaby neměla zůstat také spokojenost s provedenou změnou léčby u samotného pacienta– lékaře operačního oboru.

The case report describes treatment adjustment in a middle-aged patient with the type2 diabetes mellitus who wasn't able to achieve desirable parameters of compensationduring treatment with glargine 100 U/mL combined with oral antidiabetic agents (re-paglinide, metformin, sitagliptin). Withdrawing sitagliptin and adding glargine intoa fixed combination with lixisenatide resulted in a prompt and significant improve-ment of glucose control without inconvenient hypoglycaemic episodes. It shouldn'tbe omitted that this adjustment was also highly appreciated by our patient who workedas a surgeon.
