Roste počet pacientů chronicky dialyzovaných, ale také pacientů s nutností akutní dialýzy. Rozšiřují se možnosti použití náhrady funkce ledvin. Farmakokinetika řady léčiv je ovlivňována renální insuficiencí a/nebo aplikací náhrady funkce ledvin. Problematika farmakokinetiky a dávkování léčiv u pacientů s renální insuficiencí a dialyzovaných je velmi obsáhlá a je nemožné ji pojmout celou v jednom přehledovém sdělení. Tabulkové literární zdroje s dávkováním léčiv by v těchto případech měly sloužit k rychlé orientaci, nikoli jako dogma. Při individualizaci je třeba skloubit tři věci: vlastnosti léčiva, parametry použité modality náhrady funkce ledvin a klinický stav pacienta. Článek je proto rozdělen do tří částí. V první je pozornost věnována obecným vlastnostem léčiv, které určují chování léčiv u dialyzovaných pacientů. Jsou zde uvedeny i příklady léčiv s komentářem, který má podtrhnout to, co bylo řečeno v obecné rovině, a upozornit na specifika, která lze snadno v běžné praxi přehlédnout. Ve druhé části se článek věnuje modalitám náhrady funkce ledvin z hlediska jejich dopadu na clearance léčiv. A ve třetí části je na konkrétních příkladech léčiv – dabigatran, lithium, metformin – ukázáno propojení znalostí z předchozích dvou částí a možný dopad do klinické praxe.

The number of patients on chronic dialysis and patients with the necessity of acute dialysis is growing. The possibilities of using renal replacement therapy are expanding. Pharmacokinetics series of drugs are influenced by renal insufficiency and/or the application of renal replacement therapy. The issue of pharmacokinetics and drug dosing in patients with renal insufficiency and dialysis is very extensive. Therefore, it is impossible to cover it in a single overview. Tables/charts of literature sources for drug dosing in these cases should serve as brief orientation, not as dogma. There should be three pieces of information combined in the case of individualization: the drug properties, the parameters of the modality of renal replacement therapy, and the patient‘s clinical condition. Therefore, the article is divided into three parts. The first part of the article focuses on the general properties of medicines that determine the effects of drugs in dialysis patients. There are also examples of medicines with commentary to underline what was said in the general norm and to highlight specifics that can be easily overlooked in common practice. In the second part, the article focuses on the modalities of renal replacement therapy in terms of their impact on medication clearance. And the third part shows the combined knowledge from the previous two parts on specific examples of drugs - dabigatran, lithium, metformin and the possible impact on clinical practice.
