Glifloziny - inhibitory zpětného vstřebávání glukózy v proximálním tubulu, tj. inhibitory sodíko-glukózového kotransportéru 2 (sodium-glucose co-transporter 2, SGLT-2), ačkoli byly iniciálně vyvinuty jako antidiabetika, prokázaly v posledních třech letech svou účinnost v léčbě srdečního selhání se sníženou ejekční frakcí (ejection fraction, EF) a nejnověji se ukazují být i důležitou terapeutickou možností u pacientů se zachovalou EF či mírně redukovanou EF. Jejich pozitivní účinky jsou popisovány jak u diabetiků, tak u nediabetiků. Přehledový článek se zabývá nejen výsledky recentních randomizovaných studií, ale také nastiňuje možné mechanismy pozitivního působení inhibitorů SGLT-2 u pacientů se srdečním selháním a jejich terapeutické perspektivy v budoucnu.

Gliflozins, sodium-glucose co-transporter 2 (SGLT2) inhibitors, although originally developed as antidiabetic drugs, have shown in last three years their effectiveness in the therapy of heart failure with reduced ejection fraction (EF) and, moreover, have been emerging to be an important therapeutic option in heart failure with preserved EF or mildly reduced EF. Their positive actions have been described in diabetic as well as non-diabetic patients. This review article discusses not only the results of major recent randomized clinical trials, but also outlines possible mechanisms of positive effects of SGLT2 inhibitors in heart failure patients and their therapeutic perspectives in future.
