Bazaliom je nejčastější z kožních nádorů a jeho incidence stoupá. K léčbě recidivujících a rozsáhlých inoperabilních bazaliomů nebo vzácných metastazujících bazaliomů, které není možné řešit chirurgicky a není u nich vhodná ani radioterapie, můžeme v současné době indikovat moderní cílenou léčbu – inhibitory signální dráhy Hedgehog. V ČR je v současné době dostupný vismodegib, v zahraničí ještě sonidegib. V případě selhání této cílené léčby lze jako další linii zvolit imunoterapii anti-PD-1 protilátkou cemiplimabem.

Basal cell carcinoma is the most common skin cancer, and its incidence is increasing. For the treatment of recurrent and extensive inoperable basaliomas or rare metastasizing basaliomas, which cannot be solved surgically and for which radiotherapy is not suitable, we can currently indicate a modern targeted treatment - inhibitors of the Hedgehog signaling pathway. Vismodegib is currently available in the Czech Republic, while sonidegib is still available abroad. In case of failure of this targeted treatment, immunotherapy with the anti-PD-1 antibody cemiplimab can be chosen as the next line.
