A better understanding of risks associated with allogeneic blood transfusions along with a growing population of patients seeking surgical care without the use of blood transfusion has led to the development of complex clinical strategies, intending to minimize blood loss, conserve autologous blood, enhance hematopoiesis, and augment tolerance of anemia. Advances in surgical technique, current technology, knowledge, and perioperative management significantly contributed to the fact that successful treatment without transfusion is a realistic option but it does require a concerted patient-centered effort from the multidisciplinary team. In this review, we discuss effective bloodless strategies in the setting of perioperative management, acute blood loss, trauma, and postpartum hemorrhage. Additionally, the valuable experience gained from bloodless care has paved the way to develop the concept of Patient Blood Management as a standard care to benefit all patients, and not only those for whom blood is not an option.

- $a Lepší pochopení rizik spojených s alogenními krevními transfuzemi spolu s rostoucí populací pacientů hledajících chirurgickou péči bez použití krevní transfuze vedly k rozvoji komplexních klinických postupů, které mají za cíl omezit krevní ztráty, šetřit autologní krví, stimulovat krvetvorbu a zlepšit toleranci anémie. Pokroky v operační technice, používaných technologiích, perioperačním managementu a znalostech výrazně přispěly k tomu, že úspěšná léčba bez použití transfuze je reálnou možností, která ale vyžaduje soustředěné úsilí multidisciplinárního týmu zaměřené na pacienta. V tomto přehledu probereme osvědčené bezkrevní postupy pro perioperační management, akutní krevní ztrátu, úraz a peripartální krvácení. Zkušenosti získané z bezkrevní léčby navíc vydláždily cestu k rozvoji konceptu Patient Blood Management, který se stal standardem úspěšné léčby všech pacientů, tedy nejen těch, kterým krev podat nemůžeme.
