V práci jsou shrnuty zkušenosti autorů s léčením některých typů komplikovaných zlomenin bérce pomoci llizarevova aparátu. Běžně je na pracovištích autorů k operační léčbě zlomenin bérce používán zevní fixatér nebo metoda nitrodřeňového, zajištěného hřebování. v některých případech se ukázalo použití llizarevova aparátu jako výhodnější. Jedná se o typy zlomenin, jejichž lomná linie je v metafýze, v blízkosti kloubní štěrbiny, či o nitrokloubní zlomeniny. Ilizarevův aparát (lA) byl použit rovněž v případech primárně léčených na jiném pracovišti, které vyžadovaly z různých důvodů reoperaci a stabilizaci zlomeniny. Jednoznačně je použití llizarevova aparátu výhodnější u víceetážových zlomenin s desaxaci úlomků, kde je možné využít bočných tahů k dotažení korekce osového postavení úlomků. Kromě indikací k použití lA jsou v práci uvedeny jednotlivé výhody a nevýhody fixatéru. Práce shrnuje zkušenosti s léčením 13 pacientů v období let 1989-1993. Kromě jednoho případu, kdy se dosud nepodařilo vyléčit osteomyelitidu tibie, jsou výsledky metody jednoznačně pozitivní. Práce je dopiněna kazuistikami a vyobrazeními.

The authors summarize their experience with the treatment of some types of complicated fractures of ne leg by means of an llizarev apparatus. At the authors' department in surgery of fractures of the leg usually an external fixation device is used or intramedullary, locking nailing. In some instances the use of an llizarev apparatus proved more useful. This applies to fractures where the line of fractures is in the metaphysis, near the articular space or in case of intraarticular fractures. llizarev' s apparatus was used also in patients who had received primary treatment in another department, and who needed for different reasons surgical revision and stabilization of the fracture. The use of llizarev' s apparatus is, beyond doubt, more useful in fractures at several levels with desaxation of the fragments where lateral tractions can be used to complete the correction of the axial position of the fragments. In addition to indications for use of llizarev' s apparatus (lA) the authors mention also the advantages and disadvantages of a fixation device. The authors summarize the experience with treatment of 13 patients during 1989-1993. Except for one patient where it did not prove possible so far to cure osteomyelitis of the tibia, the results of the method are unequivocally positive. The paper is supplemented by casehistories and illustrations.
