Detekce tumorů papily Vaterovy, včetně ampulárních adenomů byla zlepšena rutinním screeningovým endoskopickým vyšetřením a zobrazovacími modalitami. Časná a přesná diagnóza ampulomů vede k časné terapii a lepší prognóze. ERCP se ukázala být nejlepší diagnostickou metodou ke stanovení povahy obstrukce u nemocných s ikterem. Endoskopická ultrasonografie (EUS) umožňuje určit velikost, rozsah, invazi do pankreatu, je vhodná k určení terapeutické alternativy. Chirurgický přístup zůstává základní léčebnou modalitou v léčbě ampulárního karcinomu. Endoskopická terapie benigních nádorů je bezpečná metoda s nulovou mortalitou a akceptovatelnou morbiditou, jeví se být dobrou alternativou chirurgické resekce zejména u rizikových nemocných. Diagnostika a léčba ampulomů je v současnosti příkladem multidisciplinární spolupráce mezi gastroenterologem, patologem a hepatobiliárním chirurgem.

Většina karcinomů pankreatu je diagnostikována v lokálně pokročilém nebo metastatickém klinickém stadiu. Léčebný přístup k lokálně pokročilému a metastazujícímu onemocnění je odlišný. Budoucnost systémové cílené léčby je v identifikaci nových cílových struktur, které mají vůdčí roli v přenosu signálů pro proliferaci nádorových buněk a ovlivňují jejich metastatický potenciál.
The majority of cases of pancreatic cancer is diagnoses in locally advanced or metastatic clinical stage. Therapeutic approach to locally advanced and metastasising disease is different. The future of targeted systemic treatment is in identification of new structures to target, which have a leading role in transferring signals for proliferation of cancer cells and influence their metastatic potential.
