Autori vyšetrovali lyzozým v pleurálnom výpotku v súbore 101 pacientov s rôznou diagnózou: tuberkulóáza (18), pleuropneumónia (21), empyém (10), malígne ochorenie (38), systémové ochorenie (3), transsudát a iná etiológia (1 l). Priemerná hodnota lyzozýmu bola v skupine tuberkulózy signifikantne vyššia ako v skupine pleuropneumónie, malígnych výpotkov a v skupine transsndátov a výpotkov inej etiológie (45,88 ± 23,98 versus 8,5 ± 7.11, 6,23 ± 11,70 a 3,3 ± 2,61 μg/ml, P < 0,001), ale signifikantne sa nelíšila od priemernej hodnoty v skupine empyémov a systémových ochorení (64,40 ± 24,89 a 30,63 ± 17,86 μg/ml). V skupine tuberkulózy zaznamenali nízku hodnotu len jedenkrát. Senzitivita vyšetrenia bola 94,4 %, špecifickosť 95,7 %. Okrem empyémov a systémových ochorení, ktoré sme pri hodnotení špecifickosti vylúčili, bol falošne pozitívny výsledok v troch prípadoch. Súčasne hodnotili koreláciu vyšetrenia lyzozýmu s vyšetrením adenozíndeaminázy. Zistili signifikantnú koreláciu obidvoch parametrov (k = 0,83, p < 0,01), avšak zvýšenie validity pri súčasnom použití obidvoch vyšetrovaných parametrov nepotvrdili. Zo záverov vyplýva, že stanovenie lyzozýmu v pleurálnom výpotku predstavuje rýchlu a pohodlnú vyšetrovaciu metódu s vysokou senzitivitou, ktorá je prinosom v diferenciálnej diagnostike tuberkulózy.
Authors determined lysozyme in pleural effusions in a group of 101 patients with various diagnoses: tuberculosis (18), pleuropneumonia (21), empyema (10), malignant diseases (38), systemic disease (3), transsudate and other diseases (1 1). The mean value of lysozyme was in the group of tuberculosis significantly higher than in the groups o pleuropneumonia, malignant effusions and in the group of transudates and effusion of other ethiology (45,88 ± 23,98, versus 8,5 ±7,1, 6,23 ± 11,70 and 3,3 ± 2,61 μg/ml, p < 0,001), but did not differ significantly from the mean values in the group of empyema and systemic diseases (64,40 ± 24,89 and 30,63 ± 17,86 μg/ml). In the group of tuberculosis they observed low value only in one case. The sensitivity was 94,4 %, and the specificity 95,7 %. Excluding the group of empyema and systemic disease, false negative value was found in three cases. Authors evaluated the correlation between determination of lysozyme and adenosindeaminase. They found a significant correlation of both parameters (r = 0,83, p < 0,01). Using simultaneously both parameters, the validity of determinations was not higher. In conclusion authors states, that the determination of lysozyme in pleural effusion is a quick and comfortable method with high sensitivity and is a contribution in the differential diagnosis of tuberculosis.
