A case series of the use of amniotic membrane (AM) for treating chronic nonhealing wounds. It presents five cases of polymorbid patients with a total of nine chronic nonhealing wounds. The patient group consisted of four men and one woman with various comorbidities, aged 45-72 years. The mean initial wound size was 15.8 cm2, and the mean time from the onset of the wound to the first application of AM was 122 weeks. The wounds were caused by chronic venous insufficiency and/or peripheral arterial disease. Wounds were treated in a standardized protocol. AM was applied weekly in the first month and then every two weeks. Photo documentation of the wound and microbiological colonization was carried out at each visit. In three out of five patients, the AM treatment effectively promoted healing up to complete wound closure. In two cases, the wounds stayed unhealed despite numerous AM applications. Pain relief was noted in all patients. The success of the treatment was closely tied to patient factors, such as adherence to the prescribed treatment regimen and individual patient characteristics. In some cases, treatment failure was observed, possibly due to underlying comorbidities, wound parameters, or poor patient compliance. AM treatment has the potential to become a viable treatment option for these nonhealing wounds. However, the effectiveness of the treatment may be influenced by various patient factors and the underlying cause of the wound. Therefore, it is crucial to have an individualized treatment plan that considers these particular factors.

To compare the therapeutic efficacy of cryopreserved amniotic membrane (AM) grafts and standard of care (SOC) in treating nonhealing wounds (NHW) through a prospective multicenter clinical trial, 42 patients (76% polymorbid) with 54 nonhealing wounds of various etiologies (mainly venous) and an average baseline size of 20 cm2 were included. All patients were treated for at least 6 weeks in the center before they were involved in the study. In the SOC group, 29 patients (36 wounds) were treated. If the wound healed less than 20% of the baseline size after 6 weeks, the patient was transferred to the AM group (35 patients, 43 wounds). Weekly visits included an assessment of the patient's condition, photo documentation, wound debridement, and dressing. Quality of life and the pain degree were subjectively reported by patients. After SOC, 7 wounds were healed completely, 1 defect partially, and 28 defects remained unhealed. AM application led to the complete closure of 24 wounds, partial healing occurred in 10, and 9 remained unhealed. The degree of pain and the quality of life improved significantly in all patients after AM application. This study demonstrates the effectiveness of cryopreserved AM grafts in the healing of NHW of polymorbid patients and associated pain reduction.

This study aimed to evaluate the efficacy of cryopreserved amniotic membrane (AM) grafts in chronic wound healing, including the mean percentage of wound closure per one AM application, and to determine whether the healing efficiency differs between AM grafts obtained from different placentas. A retrospective study analyzing inter-placental differences in healing capacity and mean wound closure after the application of 96 AM grafts prepared from nine placentas. Only the placentas from which the AM grafts were applied to patients suffering from long-lasting non-healing wounds successfully healed by AM treatment were included. The data from the rapidly progressing wound-closure phase (p-phase) were analyzed. The mean efficiency for each placenta, expressed as an average of wound area reduction (%) seven days after the AM application (baseline, 100%), was calculated from at least 10 applications. No statistical difference between the nine placentas' efficiency was found in the progressive phase of wound healing. The 7-day average wound reduction in particular placentas varied from 5.70 to 20.99% (median from 1.07 to 17.75) of the baseline. The mean percentage of wound surface reduction of all analyzed defects one week after the application of cryopreserved AM graft was 12.17 ± 20.12% (average ± SD). No significant difference in healing capacity was observed between the nine placentas. The data suggest that if there are intra- and inter-placental differences in AM sheets' healing efficacy, they are overridden by the actual health status of the subject or even the status of its individual wounds.

Cíl: Představit použití lidské kryokonzervované amniové membrány (AM) v léčbě chronických ran, specifikovat indikační kritéria pro její aplikaci, protokol aplikace a metodiku převazů. Dalším cílem je seznámení se souhrnnými výsledky studie léčby chronických nehojících se ran pomocí AM a s predikcí zhojení dle dynamiky léčby. Materiály a metody: Standardizace léčby pomocí kryokonzervované AM proběhla v rámci multicentrické studie ve třech klinických centrech. Pacientům s chronickými ranami, kteří se nehojili v průměru po dobu 34 měsíců (minimálně šest týdnů), byla aplikována AM, s pravidelnými vizitami po sedmi dnech. Výsledky: Z 35 pacientů s 43 ranami různé etiologie, bylo po aplikaci AM zcela zhojeno 24 ran (56 %), 10 ran bylo zhojeno částečně (> 50 % plochy rány) (23 %), 9 ran (21 %) se zhojit nepodařilo. Během léčby AM zaznamenali pacienti silný analgetický účinek. Sledování dynamiky hojení vedlo ke stanovení predikčního intervalu pro posouzení schopnosti rány zhojit se pomocí AM na 10–12 týdnů. Závěr: Léčba chronických ran pomocí AM přináší prospěch indikovaným pacientům, jak v akceleraci hojení, tak ve zlepšení kvality života, a to zejména snížení bolesti v oblasti rány.
Objective: This work presents the use of human cryopreserved amniotic membrane (AM) in the treatment of chronic wounds, specifying the indication criteria for its application, application protocol, and wound dressing methodology. At the same time, it presents the summary results of the study of the treatment of chronic non-healing wounds using AM with the prediction of healing according to the dynamics of the treatment. Materials and methods: Standardization of treatment using cryopreserved AM took place within the framework of a multicentre study in three specialist centres. Patients with chronic wounds that did not heal for an average of 34 months (min. six weeks) were treated with AM, with regular visits every seven days. The AM application was carried out 1–2 times in 14 days. Results: Out of 35 patients with 43 wounds of various etiologies, 24 wounds (56%) were completely healed after AM application, 10 wounds were partially healed (> 50% of the wound area) (23%), 9 wounds (21%) failed to heal. During AM treatment, patients experienced a strong analgesic effect. Monitoring the dynamics of healing led to the establishment of a prediction interval for assessing the ability of the wound to heal using AM at 10–12 weeks. Conclusion: The treatment of chronic wounds using AM brings benefits to the indicated patients, both in the acceleration of healing and in the improvement of the quality of life, especially in the reduction of pain in the wound area.

We evaluated the effect of the application of cryo-preserved amniotic membrane on the healing of 26 non-healing wounds (18 patients) with varying aetiologies and baseline sizes (average of 15.4 cm2 ), which had resisted the standard of care treatment for 6 to 456 weeks (average 88.8 weeks). Based on their average general responses to the application of cryo-preserved AM, we could differentiate three wound groups. The first healed group was characterised by complete healing (100% wound closure, maximum treatment period 38 weeks) and represented 62% of treated wounds. The wound area reduction of at least 50% was reached for all wounds in this group within the first 10 weeks of treatment. Exactly 19% of the studied wounds responded partially to the treatment (partially healed group), reaching less than 25% of closure in the first 10 weeks and 90% at maximum for extended treatment period (up to 78 weeks). The remaining 19% of treated wounds did not show any reaction to the AM application (unhealed defects). The three groups have different profiles of wound area reduction, which can be used as a guideline in predicting the healing prognosis of non-healing wounds treated with a cryo-preserved amniotic membrane.

Úvod: Endovenózní laserová ablace (EVLA) je uznávaná alternativa chirurgické léčby varixů, ale optimální laserový generátor a jeho nastavení jsou předmětem diskuse. Účelem sdělení bylo korelovat klinické výsledky s teoretickou výhodou užití laserového diodového generátoru vlnové délky 1940 nm, který generuje záření s vysokou absorpcí tepla v tenké vrstvě koagulované tkáně. Metody: V období od 1/2010 do 12/2021 byla provedena EVLA celkem u 3529 konsekutivních pacientů s křečovými žilami a sonograficky prokázanou insuficiencí povrchových žil dolních končetin. Postupně jsme užívali tři typy laserových generátorů s vlnovou délkou 1064, 1470, resp. 1940 nm. Všichni pacienti byli prospektivně zařazeni do registru, absolvovali časnou pooperační kontrolu s objektivizací neprůchodnosti žilního segmentu a další kontroly v případě komplikací. Výsledky: Úspěšnost žilního uzávěru se u všech tří laserů nelišila (p=0,054) a přesáhla 98 %. Katetrizační metoda umožnila provést více ablací během jednoho výkonu s trendem 1,08, 1,31 a 1,62. V roce 2021 byl u laseru Tethys 1940 nm počet výkonů na pacienta 1,79. U tohoto laseru bylo, z důvodu vyššího absorpčního koeficientu záření pro vodu, možno snížit celkovou aplikovanou energii na polovinu (8 W, 50−80 J/cm). Pacienti po EVLA tímto laserem měli příznivější pooperační průběh, přestože bylo provedeno více ablací najednou. U 95,6 % pacientů byla provedena jen časná pooperační kontrola (p<0,001). Závěr: Studie prokázala, že všechny užité laserové generátory měly stejnou efektivitu, co se týče uzávěru žilního segmentu. Při užití diodového laseru vlnové délky 1940 nm bylo možno snížit výkon laseru na polovinu. Pacienti měli lepší pooperační průběh, přestože bylo provedeno více výkonů během jedné operace.
Introduction: Endovenous laser ablation (EVLA) is a recognized alternative to surgical treatment of varicose veins, although an optimal laser generator and its settings still remain a matter of debate. The aim of our study was to correlate clinical results with the theoretical advantage of the 1940nm diode laser characterized by high absorption of heat in a thin layer of coagulated tissue. Methods: From 1/2010 to 12/2021 EVLA was performed in a total of 3529 consecutive patients with varicose veins and ultrasonographically documented superficial venous reflux of lower extremities. Three types of laser were used successively with the wavelengths of 1064 nm, 1470 nm and 1940 nm, respectively. All patients were prospectively enrolled in our registry. An early postoperative follow−up visit was scheduled including an assessment of venous closure; additional visits were performed only in case of complications. Results: The success of venous closure did not differ (p=0.054) between the three laser types and was over 98%. The catheter−based method made it possible to perform multiple ablations in one procedure − the trend was 1.08, 1.31 and 1.62. In 2021 the number of ablations per patient with the laser DL Tethys 1940 nm was 1.79. With this laser it was possible to reduce the total energy applied to one half (8 W, 50−80 J/cm). The postoperative course of patients treated using the 1940nm laser was smoother – no other but the early follow−up visit was needed in 95.6% cases (p<0.001). Conclusion: This study showed that all three types of the near−infrared lasers had an equal closure efficacy. With the 1940nm DL it was possible to decrease the energy to one half. Despite multiple ablations performed in one procedure the postoperative course of the patients was smoother.
Úvod: Endovenózní laserová ablace (EVLA) je běžně užívaná alternativa chirurgické léčby varixů. Účelem sdělení je potvrdit trvalost uzávěru a prokázat, že lze všechny pacienty i s žilním segmentem nad 10 mm operovat ambulantně a provést více EVLA najednou. Metody: V období od 1/2017 do 12/2019 byla provedena EVLA celkem u 1551 neselektovaných pacientů s křečovými žilami a sonograficky prokázanou insuficiencí povrchových žil dolních končetin. K zjištění střednědobých výsledků jsme vybrali pacienty operované v období 1/2017 až 6/2017 (316 pacientů). K ověření výsledku operace jsme vybrali rizikovou skupinu pacientů s žilním segmentem průměru >10 mm (40 pacientů). Tyto pacienty považuje dosud řada odborníků za kandidáty klasické chirurgické metody. Kontrolní skupinou s žilním segmentem <10 mm bylo zbylých 276 pacientů operovaných ve stejném období. Výsledky: Katetrizační metoda umožnila provést více ablací během jednoho výkonu. V roce 2019 byl počet výkonů na pacienta 1,44. V rizikové skupině mělo jednu pooperační kontrolu (hladký průběh, bez větší lokální reakce) 87,5 % pacientů. V kontrolní skupině mělo 100 % pacientů hladký pooperační průběh a jen jednu kontrolu včetně ultrasonografického vyšetření (p<0,001). Výskyt sledovaných pooperačních komplikací byl v obou skupinách srovnatelný. Sonograficky byl zkoumán uzávěr v rizikové skupině. Žíly byly dobře koagulovány, se známkou dezintegrace peroperačně, ale při poslední kontrole (střední doba sledování 33±6 měsíců) nebyla již žíla patrná. Závěr: Studie prokázala příznivé střednědobé výsledky ambulantně prováděné EVLA vena saphena. Potvrdila se proveditelnost více výkonů najednou v neselektovaném souboru pacientů s užitím lokální anestezie. Bylo prokázáno, že destrukce žíly laserem je trvalá. Podmínkou reprodukovatelnosti je dokonalá znalost UZ diagnostiky a časná kontrola pacientů.
Introduction: Endovenous Laser Ablation (EVLA) is a common alternative to surgical treatment of varicose veins. The aim of our study was to demonstrate that laser occlusion is durable, that we can treat all patients in a one day setting, even with veins >10mm in diameter, and that multiple EVLAs can be done at the same time. Methods: In the period from 1/2017 to 12/2019 EVLA was performed in a total of 1551 consecutive patients with varicose veins and ultrasonographically documented venous reflux. The mid-term results were evaluated in a group of patients operated from 1/2017 to 6/2017 (316 pts.). We compared a risk group that consisted of patients with veins >10mm in diameter (40 pts.) with a control group (the remaining 276 pts.). Patients with veins >10mm are traditionally considered as candidates for conventional surgery. Results: The catheter-based method enabled us to perform more ablations in one procedure. In 2019 we performed 1.44 EVLA procedures per patient. There was only one postoperative follow-up visit, indicating an uncomplicated postoperative course, in 87.5% of patients of the risk group. In the control group 100% of patients had only one follow-up visit including ultrasound examination, showing an uncomplicated postoperative course (p<0.001). There was no difference in the incidence of complications. Ultrasonographic follow-up was performed in the risk group. Veins larger in diameter revealed complete disintegration early, but at the end of the follow-up period (mean 33±6 months) they disappeared completely. Conclusion: The study showed good mid-term results of EVLA of the saphenous vein in the outpatient setting and the feasibility of performing multiple ablations in a non-selected group of patients using local anaesthesia. Laser destruction of the vein was shown to be permanent. Perfect familiarity with the diagnostic procedure using ultrasonography and early patient follow-up are required to achieve reproducibility.

Úvod: Katetrizační endovenózní laserová ablace (EVLA) je běžně užívaná alternativa chirurgické léčby varixů dolních končetin. V poslední době se katetrizační metody osvědčují jako metoda volby vzhledem k preferenci pacientů, kteří si cení minimální invazivity V současnosti se ve výzkumu EVLA soustředíme na optimalizaci procedury včetně studia výhod jednotlivých typů laserových generátorů a použitých vlnových délek. V této observační studii jsme porovnali naše časné výsledky u neselektovaných, konsekutivních pacientů léčených dvěma různými typy laseru. Metody: V období od února 2010 do června 2017 byla provedena EVLA celkem u 1747 po sobě jdoucích pacientů (74 % ženy) s žilním refluxem. Průměrná šíře žíly byla 8,5 mm (5−25 mm). Naše studie se pokusila srovnat ekonomičtější 1470nm diodový laserový (DL) generátor (Velas 2, Čína) – operováno 630 pacientů, s krystalovým generátorem Nd-Yag (Fotona – Slovinsko) – operováno 1117 pacientů. Všechny operace byly provedeny shodnou metodikou, ambulantně, v jednom specializovaném centru. Všechny výkony byly provedeny v místním znecitlivění s užitím tumescentní anestezie a s peroperační sonografickou kontrolou. Všichni pacienti měli časnou sonografickou kontrolu. Výsledky: Neprokázali jsme statisticky významný rozdíl v časném uzávěru (98,8 % u Nd-Yag vs. 99,8 u DL p-ns). Časná recidiva byla pozorována u 9 pacientů (15 žilních segmentů). Všechny byly úspěšně časně reintervenovány a uzavřeny. Mezi příčiny neúplného uzávěru patřily zejména běžně známé rizikové faktory (antikoagulační terapie, varikoflebitida v anamnéze). Nebyla zjištěna korelace s větším průměrem žíly. U 6 pacientů byl pozorován prolaps trombu do lumen hluboké femorální žíly. Všechny případy byly úspěšně vyléčeny po týdnu léčby nízkomolekulárním heparinem. Byl zaznamenán pouze jeden případ nízce rizikové plicní embolie u pacientky, která neuposlechla režimová doporučení. Závěr: Tato studie neprokázala signifikantní rozdíl ve spolehlivosti uzávěru a míře komplikací při provedení endovenózní laserové ablace velké a malé safény pomocí krystalového 1060nm Nd-Yag generátoru ve srovnání s ekonomičtějším 1470nm diodovým laserovým generátorem.
Introduction: Catheter-Based Endovenous Laser Ablation (EVLA) is a commonly used alternative to surgical treatment of varicose veins. Recently, catheterization methods have proved to be methods of choice due to the preference of patients who value minimal invasiveness. Research of EVLA currently focuses on optimization of the procedure, which includes study of the benefits of the individual types of laser generators and the wavelengths used. In this observational study we compared our early results in a non-selected population of consecutive patients treated with two different types of lasers. Methods: In the period from February 2010 to June 2017, EVLA was performed in a total of 1747 consecutive patients (74% were female) with venous reflux. The average vein width was 8.5 mm (5−25 mm). Our study sought to compare a more economical 1470nm diode laser (DL) generator (Velas 2, China) – used to operate on 630 patients – with a Nd-Yag crystal generator (Fotona - Slovenia) used in 1117 patients. All operations were performed using the same methodology, in an outpatient setting, in one specialized center. All procedures were completed in local tumescent anesthesia under peroperative ultrasound control. Postoperative sonography was performed in all patients. Results: The results did not show a statistically significant difference in early closure rates (98.8% for Nd-Yag versus 99.8 for DL p-ns). Early recurrence was observed in 9 patients (15 vein segments) and managed successfully with early re-intervention and closure in all cases. The causes of incomplete closure included mainly the known risk factors (anticoagulation therapy, history of varicophlebitis). There was no correlation with larger venous diameter. In 6 patients, thrombus prolapse was observed in the deep femoral vein lumen. All cases were successfully cured after a week of low-molecular-weight heparin therapy. Only one case of low-risk pulmonary embolism was reported in a patient who failed to follow the regime recommendations. Conclusion: This evidence did not show a significant difference in closure reliability and the amount of complications of the endovenous laser ablation of large and small saphenous vein with a 1060nm Nd-Yag crystal compared to the more economical 1470nm diode laser generator.

V této observační studii jsme si kladli za cíl prokázat, zda je tato metoda bezpečná a proveditelná v souboru neselektovaných pacientů a zda je možno provést ablaci více segmentů v rámci jednoho výkonu. Metodika: V období od února 2010 do března 2016 byla provedena EVLA celkem v 1 209 žilních segmentech u 1 117 po sobě následujících pacientů (74 % ženy) s žilním refluxem. Průměrná šíře žíly byla 8,5 mm (5-25 mm). Všechny operace byly provedeny ambulantně v místním znecitlivění s užitím tumescentní anestezie. Všechny výkony byly provedeny s peroperační sonografickou kontrolou. Výsledky: Časný uzávěr byl úspěšný u 98,8 % intervenovaných žil. Časná recidiva byla pozorována u devíti pacientů (15 žilních segmentů). Všechny byly úspěšně časně reintervenovány a uzavřeny. Mezi příčiny neúplného uzávěru patřily zejména běžně známé rizikové faktory (antikoagulační terapie, varikoflebitida v anamnéze). Nebyla zjištěna korelace s větším průměrem žíly. U šesti pacientů byl pozorován prolaps trombu do lumen hluboké femorální žíly. Všechny případy byly úspěšně vyléčeny po týdnu léčby nízkomolekulárním heparinem. Byl zaznamenán pouze jeden případ nízkorizikové plicní embolie u pacientky, která neuposlechla režimová doporučení. Závěr: Endovenózní laserová ablace velké a malé safény a jejich velkých větví v léčbě symptomatického žilního refluxu je proveditelná, bezpečná a reprodukovatelná metoda. Uzávěr žilních segmentů nebyl limitován jejich průměrem ani počtem.
Introduction: Catheter-based endovenous laser ablation (EVLA) is a widely used alternative to surgery and is slowly becoming a state of the art method in the treatment of varicose veins. In this observational study we sought to determine whether the method is safe and feasible in an unselected population of patients and if it can be used on more vein segments during one procedure. Methods and results: From February 2010 to March 2016 EVLA was performed in 1209 venous segments in 1117 consecutive patients (74% women) with venous reflux. Mean diameter of ablated segments was 9.3 mm (range 5-25 mm). All procedures were performed on an out-patient basis using local tumescent anesthesia. Intraoperative sonographic guidance was mandatory. Early closure was successful in 98.8% of treated venous segments. Early non-closure occurred in 9 patients (15 treated segments) and all were successfully resolved by early re-EVLA. The causes of incomplete closure were mainly of technical character due to the learning period - low energy, non-closure of proximal tributaries, non-complete emptying of the vein during procedure. Non-closure was not related to diameter of the vein. In 6 patients the prolapse of the thrombus into the lumen of deep femoral vein was noted. The thrombus resolved after one week treatment with low molecular weight heparin. Low-risk pulmonary embolism was observed in one non-compliant patient. Conclusion: EVLA of the greater saphenous vein, small saphenous vein and large tributaries in the treatment of symptomatic reflux is a flexible, safe, effective and reproducible method. The closure of venous segments was performed without limitation in venous diameter and amount of segments.
