The spleen is one of the most commonly injured organ in blunt traumas to the chest and abdomen. Splenic injury can be a serious complication of fracture of the left 9th to 11th rib. The authors present a case report of a 65-year-old male patient with a blunt trauma to the left chest and abdomen, diagnosed with multiple left rib fractures, left hemothorax and splenic injury with a small subcapsular hematoma with no signs of active splenic bleeding. Due to hemodynamic instability and a large volume of blood loss via the chest drain, the patient was indicated for emergency left thoracotomy. A perforation in the lower lobe of the left lung caused by rib fractures was found, which was treated with sutures. Furthermore, the diaphragm was examined, two ruptures were identified from which blood was coming out, and thus a phrenotomy was performed. The bleeding central splenic rupture came as a big surprise. A spleen preserving surgery was impossible, therefore a splenectomy had to be performed, followed by chest wall stabilization with splints. Transthoracic approach to manage the splenic injury through phrenotomy should not be used as a standard. In a selected group of patients with concomitant chest and upper abdominal organ injuries, the use of this surgical approach appears to be highly beneficial. Úvod: Robotická chirurgie slinivky břišní dosud není v České republice výrazně rozšířena. V celosvětovém měřítku je postupně, zejména v high-volume centrech, systematicky implementována. Studie prezentuje první zkušenosti s robotickou operativou slinivky břišní v pankreatochirurgickém high-volume centru v ČR. Metody: Srovnávací analýza první desítky pacientů operovaných roboticky pro patologii na slinivce břišní se srovnatelným souborem operovaných otevřeně (výkony menšího rozsahu: levostranná pankreatektomie s/bez splenektomie, enukleace, centrální resekce) hodnotí věk, délku hospitalizace, pobyt na JIP, krevní ztráty a operační čas pomocí neparametrického Mann-Whitneyova U-testu, a komplikace a rehospitalizace pomocí deskriptivní statistiky. Výsledky: Srovnávané soubory tvoří 10 pacientů (5 žen) s provedeným robotickým výkonem (5× enukleace, 4× distální pankreatektomie a centrální resekce – medián věku 65,3 [46–76] let) a 10 pacientů (5 žen) se srovnatelnými otevřenými výkony (5× enukleace, 4× distální resekce a centrální resekce – medián věku 65,5 [52–78] let). Délka hospitalizace 4,5 vs. 9 dnů (p = 0,0003) a délka operace 275 vs. 142 min (p = 0,004) byly statisticky rozdílné u robotické vs. otevřené skupiny. Pobyt na JIP a krevní ztráty byly nižší ve skupině robotické, ale ne statisticky významné, věk byl srovnatelný. Komplikace podle Clavien-Dindovy klasifikace ve skupině otevřené: 3× typ II, 1× typ IIIa, 1× typ IIIb, ve skupině robotické 1× typ II a 1× typ IIIa. Rehospitalizace v souboru otevřených byla 2×, v souboru robotických 1×, vždy pro pozdní pooperační komplikaci. Závěr: Robotická chirurgie slinivky břišní je v současnosti v ČR v úplných začátcích. Dosavadní zkušenost s první desítkou nemocných operovaných prostřednictvím robotického systému DaVinci Xi přinášejí slibné výsledky v podobě zkrácení hospitalizace, ale za cenu delšího operačního času.
Introduction: Robotic pancreatic surgery is still not widely used in the Czech Republic. On a global scale, it is gradually being systematically implemented, especially in high-volume centers. The study presents the first experiences with robotic pancreatic surgery in a pancreatic surgery high-volume center in the Czech Republic. Methods: Comparative analysis of the first ten patients operated on robotically for pancreatic pathology with a comparable group operated open (smaller procedures: left pancreatectomy with/without splenectomy, enucleation, central resection) evaluates age, length of hospitalization, ICU stay, blood loss and operative time using non-parametric Mann--Whitney U-test, and complications and rehospitalization using descriptive statistics. The results: The compared sets consist of 10 patients, 5 women, with a robotic procedure (5× enucleation, 4× distal pancreatectomy and central resection – median age 65.3 [46–76] years) and 10 patients, 5 women, with comparable open procedures (5× enucleation, 4× distal resection and central resection – median age 65.5 [52–78]years. Hospitalization length 4.5 days vs. 9 days (P = 0.0003) and operation time 275 min vs. 142 min (P = 0.004) were statistically different in the robotic vs open group. ICU stay and blood loss were lower in the robotic group, but not statistically significant, age was comparable. Complications according to the Clavien-Dindo classification in the open group: 3× type II, 1× type IIIa, 1× type IIIb, in the robotic group 1× type II and 1× type IIIa. Rehospitalization in the open group was 2×, in the robotic group 1x, always for late postoperative complications. Conclusion: Robotic surgery of the pancreas is currently in its infancy in the Czech Republic. The experience so far with the first ten patients operated on using the DaVinci Xi robotic system has yielded promising results in form of shorter hospitalization, but at cost of longer operating time.

Úvod: Hepatická tepna se vyznačuje vysokou variabilitou svého průběhu. Cílem této studie je zhodnocení četnosti výskytu jednotlivých typů variantního průběhu v populaci nemocných podstupujících pravostrannou pankreatoduodenektomii (PD), přesnosti předoperačního CT stagingového vyšetření v diagnostice těchto variant a zhodnocení vlivu přítomnosti variantního průběhu hepatické tepny na peroperační a pooperační morbiditu u pacientů podstupujících PD. Metody: Prospektivní observační studie zahrnující 147 pacientů, kteří podstoupili PD pro patologii v oblasti hlavy pankreatu na I. chirurgické klinice Fakultní nemocnice Olomouc v období 1/2015 až 12/2018. Předoperační diagnostika průběhu hepatické tepny byla provedena na základě analýzy stagingového CT vyšetření břicha, výsledek byl klasifikován podle Michelsovy škály a byl korelován s definitivním peroperačním nálezem. Demografická, histopatologická, peroperační a pooperační data pacientů byla součástí prospektivně vedené databáze. Výsledky: Do studie bylo zařazeno celkem 147 pacientů, 83 (56,5 %) mužů a 64 (43,5 %) žen, medián věku 65,0 (37–83) let. Variantní průběh hepatické tepny byl zjištěn u 37 (25,2 %) pacientů. Přesnost předoperační CT diagnostiky v určení typu cévní varianty byla 100 %. Nebyl prokázán statisticky signifikantní rozdíl ve výskytu pooperačních komplikací mezi skupinou pacientů s/a bez variantního průběhu hepatické tepny – CD I–II (50,0 % vs. 47,2 %), CD III–IV (8,3 % vs. 13,8 %). Obdobně je v obou skupinách srovnatelná 30denní (4,2 % vs. 2,4 %) a 90denní mortalita (4,2 % vs. 3,3 %). Závěr: Předoperační diagnostika cévních variant založená na multidetektorovém CT vyšetření břicha je rutinně dostupná a vykazuje vysokou přesnost detekce. Nebyl prokázán rozdíl v pooperační morbiditě a mortalitě u pacientů s a bez přítomnosti variantní hepatické tepny podstupujících PD.
Introduction: Variations in hepatic artery anatomy are very common in the population. The aim of this study is to evaluate the rates of individual types of hepatic artery variants in the population of patients undergoing pancreaticoduodenectomy (PD), assess the accuracy of preoperative staging CT imaging to identify hepatic artery variants, and evaluate the impact of the hepatic artery variants on perioperative and postoperative morbidity of PD patients. Methods: A prospective observation study of 147 patients undergoing PD for a pancreatic head pathology at the 1st Department of Surgery, University Hospital Olomouc between 1/2015–12/2018. Preoperative diagnosis of the course of the hepatic artery was made based on staging CT imaging analysis of the abdomen. The result was classified according to the Michels’ scale and correlated with the final perioperative finding. Demographic, histopathological and clinicopathological data were included in a prospectively maintained database. Results: A total of 147 patients were included in the study, 83 (56.5%) males and 64 (43.5%) females, median age 65.0 (37−83) years. A variant course of the hepatic artery was found in 37 (25.2%) patients. The accuracy of preoperative CT imaging in determining the variant was 100%. The presence of a hepatic artery variant was not statistically significant as a factor in terms of postoperative complications – CD I-II (50.0% vs 47.2%), CD III-IV (8.3% vs 13.8%). Similarly, the 30-day (4.2% vs 2.4%) and 90-day mortality rates (4.2% vs 3.3%) were comparable in both groups. Conclusion: Preoperative diagnosis of vascular variants based on multidetector CT imaging of the abdomen is routinely available and shows high detection accuracy. There was no difference in postoperative morbidity and mortality in patients with and without a variant hepatic artery undergoing PD.

DNA repair pathways are essential for maintaining genome stability, and understanding the regulation of these mechanisms may help in the design of new strategies for treatments, the prevention of platinum-based chemoresistance, and the prolongation of overall patient survival not only with respect to ovarian cancer. The role of hyperthermic intraperitoneal chemotherapy (HIPEC) together with cytoreductive surgery (CRS) and adjuvant systemic chemotherapy is receiving more interest in ovarian cancer (OC) treatment because of the typical peritoneal spread of the disease. The aim of our study was to compare the expression level of 84 genes involved in the DNA repair pathway in tumors and the paired peritoneal metastasis tissue of patients treated with CRS/platinum-based HIPEC with respect to overall patient survival, presence of peritoneal carcinomatosis, treatment response, and alterations in the BRCA1 and BRCA2 genes. Tumors and metastatic tissue from 28 ovarian cancer patients collected during cytoreductive surgery before HIPEC with cisplatin were used for RNA isolation and subsequent cDNA synthesis. Quantitative real-time PCR followed. The most interesting findings of our study are undoubtedly the gene interactions among the genes CCNH, XPA, SLK, RAD51C, XPA, NEIL1, and ATR for primary tumor tissue and ATM, ATR, BRCA2, CDK7, MSH2, MUTYH, POLB, and XRCC4 for metastases. Another interesting finding is the correlation between gene expression and overall survival (OS), where a low expression correlates with a worse OS.

PURPOSE: Bile duct injury (BDI) remains the most serious complication following cholecystectomy. However, the actual incidence of BDI in the Czech Republic remains unknown. Hence, we aimed to identify the incidence of major BDI requiring operative reconstruction after elective cholecystectomy in our region despite the prevailing modern 4 K Ultra HD laparoscopy and Critical View of Safety (CVS) standards implemented in daily surgical practice among the Czech population. METHODS: In the absence of a specific registry for BDI, we analysed data from The Czech National Patient Register of Reimbursed Healthcare Services, where all procedures are mandatorily recorded. We investigated 76,345 patients who were enrolled for at least a year and underwent elective cholecystectomy during the period from 2018-2021. In this cohort, we examined the incidence of major BDI following the reconstruction of the biliary tract and other complications. RESULTS: A total of 76,345 elective cholecystectomies were performed during the study period, and 186 major BDIs were registered (0.24%). Most elective cholecystectomies were performed laparoscopically (84.7%), with the remaining open (15.3%). The incidence of BDI was higher in the open surgery group (150 BDI/11700 cases/1.28%) than in laparoscopic cholecystectomy (36 BDI/64645 cases/0.06%). Furthermore, the total hospital stays with BDI after reconstruction was 13.6 days. However, the majority of laparoscopic elective cholecystectomies (57,914, 89.6%) were safe and standard procedures with no complications. CONCLUSION: Our study corroborates the findings of previous nationwide studies. Therefore, though laparoscopic cholecystectomy is reliable, the risks of BDI cannot be eliminated.

Nádory appendixu a pseudomyxom peritonea (PMP) jsou vzácné. Nejčastějším zdrojem PMP jsou perforované epiteliální nádory appendixu. Toto onemocnění je charakterizováno přítomností částečně na povrchy adherujícího mucinu různého stupně konzistence. Samotné apendikální mukokély jsou taktéž velmi vzácné a zpravidla jejich léčba zahrnuje pouze prostou apendektomii. Cílem této práce bylo vytvořit aktuální přehled doporučení pro diagnostiku a léčbu těchto malignit podle aktuálních doporučení skupiny PSOGI (The Peritoneal Surface Oncology Group International) a Modré knihy České onkologické společnosti ČLS JEP.
Appendiceal tumors and pseudomyxoma peritonei (PMP) are rare tumors. Perforated epithelial tumors of the appendix are the most common source of PMP. This disease is characterized by the presence of mucin of varying degrees of consistency, partially adherent to the surfaces. Appendiceal mucoceles themselves are also very rare and usually their treatment involves only a simple appendectomy. The aim of this study was to provide an up-to-date review of the recommendations for the diagnosis and treatment of these malignancies according to the current guidelines of The Peritoneal Surface Oncology Group International (PSOGI) and the Blue Book of the Czech Society for Oncology of the Czech Medical Association of J. E. Purkyně (ČOS ČLS JEP).
BACKGROUND: Diffuse peritonitis is an acute abdominal condition characterized by high mortality. The main treatment modality is surgery, requiring a subsequent prolonged hospital stay. These patients are, among other things, at risk of developing hospital-acquired pneumonia (HAP), which considerably worsens their treatment outcomes. This study aimed to extend the existing knowledge by providing more detailed microbiological characteristics of complicating HAP in patients with secondary peritonitis, including the identification of isolated bacterial pathogens and their potential sources. METHODS: The 2015-2019 retrospective study comprised all patients with an intraoperatively confirmed diagnosis of secondary diffuse peritonitis who were classified in accordance with the quick Sepsis Related Organ Failure Assessment scoring system. RESULTS: HAP developed in 15% of patients. The 90-day mortality rates were 53% and 24% in patients with and without HAP; respectively. The most frequent pathogens responsible for HAP were Pseudomonas aeruginosa, Klebsiella pneumoniae, Escherichia coli, Enterobacter cloacae complex and Enterococcus faecalis. Multidrug resistance to antibiotics was found in 38% of bacterial pathogens. Clonal spread of these bacterial pathogens among patients was not detected. Rather, the endogenous characteristic of HAP was confirmed. CONCLUSIONS: The initial antibiotic therapy of complicating HAP in patients with secondary peritonitis must be effective mainly against enterobacteria, including strains with the production of ESBL and AmpC beta-lactamases, Pseudomonas aeruginosa and Enterococcus faecalis. The study further highlighted the importance of monitoring the respiratory tract bacterial microflora in patients with secondary peritonitis. The results should be used for initial antibiotic treatment of complicating HAP instances.

Úvod: K rozšiřování miniinvazivní chirurgie dochází i v segmentu chirurgie slinivky břišní. Cílem práce je analýza současného stavu miniinvazivní chirurgie slinivky břišní v České republice a oprávněnosti a potenciálu implementace miniinvazivních operačních postupů. Metody: Analýza dat poskytovatelů a plátců zdravotní péče v segmentu high volume center. Výsledky: Třináct pracovišť splňuje navrhovaná kritéria pro označení za tzv. high volume centrum – centrum vysoce specializované péče v chirurgii slinivky břišní na základě ročního počtu minimálně 17 provedených velkých resekčních výkonů na pankreatu. Podle dat plátců zdravotní péče bylo v jednotlivých centrech využito u těchto výkonů laparoskopie v 0,6−65,7 %. V těchto případech se však nejedná o resekční výkony. Samotná pracoviště uvádějí významně menší počty miniinvazivních výkonů resekční povahy na slinivce břišní. Skutečné počty miniinvazivních resekčních výkonů v současném systému prakticky nelze ověřit. Na základě identifikace kandidátních pacientů miniinvazivní resekční chirurgie lze odhadnout potenciál implementace miniinvazivní chirurgie slinivky břišní v ČR. Závěr: Roztříštěnost tohoto segmentu operativy, jeho nákladnost a malé počty pacientů vhodných pro miniinvazivní chirurgii slinivky i v rámci segmentu high volume center tak způsobují jen velmi pomalé zavádění těchto metod. Potřeba centralizace tohoto segmentu péče se ze všech pohledů jeví jako velmi naléhavá.
Introduction: Minimally-invasive surgical methods have been becoming ever more common also in the segment of pancreatic surgery. The aim of this paper was to analyze the current state of minimally-invasive surgery in the Czech Republic and the justification and potential of implementing such procedures. Methods: Analysis of high volume centers using healthcare providers´ and payers´ data. Results: Thirteen pancreatic surgical centers meet the proposed criteria for being called a high volume center – a center of highly specialized care in pancreatic surgery based on the annual number of at least 17 major resections of the pancreas. According to data from healthcare payers, laparoscopy was used in 0.6%−65.7% of procedures in individual centers. However, these are not resection procedures. The centers themselves report a significantly smaller number of minimally-invasive pancreatic resection procedures. The actual numbers of minimally-invasive resection procedures in the current system are practically impossible to verify. The potential for implementing minimally-invasive pancreatic surgery in the Czech Republic can be estimated based on the identification of candidate patients. Conclusion: Due to the fragmentation of this operative segment, its costs and small numbers of patients suitable for minimally-invasive pancreatic surgery even among high volume centers, the implementation rate of these methods is very slow. The need to centralize this segment of care appears to be very urgent from all points of view.

Úvod: Pandemie COVID-19 celosvětově negativně ovlivnila poskytování chirurgické péče včetně onemocnění onkologických. Cílem studie je analyzovat vliv pandemie COVID-19 na krátkodobé výsledky miniinvazivního přístupu v léčbě kolorektálního karcinomu včetně průběhu multimodální léčby. Metody: Retrospektivní analýza dat pacientů čtyř fakultních nemocnic v ČR indikovaných k resekčnímu výkonu pro kolorektální karcinom během pandemie COVID-19. Kontrolní skupinou byli pacienti se stejnou diagnózou i indikací za identické období před pandemii COVID-19. Výsledky: Klinicko-demografické charakteristiky pacientů před a v průběhu pandemie COVID-19 byly srovnatelné. Krátkodobé pooperační výsledky se signifikantně nelišily. Podíl miniinvazívně prováděných kolorektálních operací v období pandemie signifikantně narostl (24 % vs. 48 %, p = 0,0004), bez navýšení rizika SARS-CoV-2 infekce pro pacienty nebo personál. Signifikantně se v době pandemie COVID-19 zkrátil interval mezi indikací k operaci a termínem výkonu (p = 0,0002). Závěr: Laparoskopický nebo robotický přístup v léčbě kolorektálního karcinomu v době pandemie COVID-19 byl bezpečný a bez navýšení rizika SARS-CoV-2 infekce pro pacienta nebo personál.
Introduction: The COVID-19 pandemic has adversely affected the delivery of surgical and oncological care worldwide. The aim of this study is to analyze the impact of the COVID-19 pandemic on short-term outcomes of minimally invasive approaches to colorectal cancer, including the timing of multimodality treatment. Methods: Retrospective data analysis of patients from four teaching hospitals in the Czech Republic referred for resection for colorectal cancer during the COVID-19 pandemic. The control group consisted of patients with the same diagnosis and indication for an identical period before the COVID-19 pandemic. Results: Clinical and demographic characteristics of colorectal cancer patients before and during the COVID-19 pandemic did not differ. Short-term postoperative outcomes were not significantly different. During the pandemic period, the proportion of minimally invasive surgeries increased significantly (24% vs. 48%, p = 0.0004), with no increase in the risk of SARS-CoV-2 infection for patients or staff. The interval between the indication and the date of surgery was significantly shorter during the pandemic COVID-19 (p = 0.0002). Conclusions: The laparoscopic or robotic approach in the surgical treatment of colorectal cancer during the COVID-19 pandemic was safe and without increasing the risk of SARSCoV-2 infection for the patient or the staff.
Krvácení do horního gastrointestinálního traktu KHGIT se vyskytuje s incidencí 47:100 000, přičemž v 8–10 % případů se setkáváme s perzistujícím nebo rekurentním krvácením. Kazuistika prezentuje případ pacientky s KHGIT u níž opakovaný pokus o endoskopickou hemostázu selhal a pro oběhovou nestabilitu byla provedena chirurgická hemostáza. Následovaly opakované epizody KHGIT a komplikace v podobě dehiscence gastrotomie, která si vyžádala další operační revizi. Opakovaná gastroskopická ošetření ani transkatetrová arteriální embolizace TAE nevedli k definitivní hemostáze. Při další operační revizi se vzhledem k nekróze přední žaludeční stěny stala totální gastrektomie posledním možným řešením, po němž se dále známky KHGIT neobjevily.
Upper gastrointestinal bleeding UGIB has an incidence of 47 per 100,000 population, and 8-10% of cases with persistent or recurrent bleeding. In this case report, the authors present a case of a patient with UGIB that cannot be stopped endoscopically, and surgical hemostasis was needed for hemodynamic instability. Recurrent episodes of UGIB and complications such as dehiscent gastrotomy resulted in the subsequent surgical revision. Repeated endoscopic intervention and transcatheter arterial embolization TAE failed to achieve definitive hemostasis. This situation resulted in the subsequent surgical revision, where the anterior gastric wall necrosis was detected, and total gastrectomy was the only possible resolution. The signs of UGIB did not appear.
