Úvod: Stereotaktická radioterapie (SBRT) je moderní metodou v zevní radioterapii. Přináší možnost aplikovat vysokou dávku záření s vyšší biologickou účinností do malého cílového objemu. Zabývali jsme se pacienty s karcinomem štítné žlázy indikovaných k této terapii. Pacienti a metody: Retrospektivně jsme zhodnotili soubor pacientů s diferencovaným karcinomem štítné žlázy z období let 2011 až 2019 indikovaných k této léčbě pro přetrvávající reziduum. Výsledky: V daném období 9 let podstoupilo terapii z důvodu přetrvávajícího rezidua 14 pacientů, 10 žen, 4 muži. Radiojód akumulující reziduum bylo před terapií přítomno u 9 pacientů, 5 pacientů bylo indikováno na základě elevované hladiny tyreoglobulinu (Tg) a rezidua přítomného na jiných zobrazovacích metodách. Po terapii došlo k plné nebo parciální regresi radiojód akumulujícího rezidua ve šech 9 případech. U 5 pacientů bez radiojód akumulujícího rezidua nedošlo k progresi ložiska na zobrazovacích metodách, odpověď ve sledované hladině Tg byla variabilní. Závěr: Radioterapie pomocí robotického systému CyberKnife představuje další léčebnou modalitu u pacientů s diferencovaným karcinomem štítné žlázy, umožňující lokální kontrolu nemoci.
Introduction: Stereotactic body radiation therapy (SBRT) is a modern method in external radiation therapy. It offers an option to use a high dose of radiation with a higher biological effect in a small target volume. We assessed the patients with the thyroid cancer indicated to this type of therapeutic approach. Patients and methods: We evaluated a group of patients with the differentiated thyroid cancer retrospectively, indicated to SBRT for the persisting residuals on the neck, from the time interval 2011–2019. Results: During the nine years interval, 14 patients (10 female, 4 male), underwent this type of therapy. Radioiodine accumulating remnants were present before SBRT in 9 patients, higher thyroglobulin (Tg) levels or residuals diagnosed by other imaging modalities were indications in 5 patients. The radioiodine accumulating residuals regressed fully or partially in all 9 cases. In the later 5 patients there was not observed any progression on the imaging examinations, response according to the thyroglobulin levels was variable. Conclusion: SBRT with the robotic CyberKnife system represents another therapeutic modality for patients with differentiated thyroid cancer, allowing local control of the disease.

Východiska: Meningeomy jsou častými primární nádory mozku s roční incidencí 3–8 případů na 100 000 obyvatel. Přestože se většinou jedná o benigní onemocnění, zhoršuje často svým charakterem a lokalizací svému nositeli kvalitu života. Základními léčebnými metodami jsou neurochirurgická exstirpace nebo stereotaktické ozáření. Obě metody dosahují velmi dobrých léčebných výsledků, přesto část pacientů trpí recidivou onemocnění vyžadující další intervenci s rizikem vzniku nebo zhoršení neurologického deficitu. Nevelké meningeomy bez růstové tendence a s absencí symptomatologie bývají často sledovány. Na druhou stranu se nelze spokojit s dlouhodobými výsledky léčby meningeomů s agresivnějším biologickým chováním. Známým rizikem recidivy meningeomů je vyšší grade, často je však obtížné identifikovat podskupiny, které vykazují prospěch z kombinované neurochirurgické operace a radioterapie, resp. u kterých je vhodné hledat cesty k další eskalaci léčby, a to i za cenu jistých vedlejších účinků. Pro rozvoj personalizované medicíny se perspektivně jeví analýza hormonální, cytogenetické i epigenetické výbavy nádorů. Cíl: Cílem práce je shrnout poznatky o epidemiologii, léčebných algoritmech a prognóze meningeomů mozku a navrhnout perspektivní prognostické a prediktivní faktory pro maximálně individualizovanou terapii.
Background: Common primary brain tumors are meningiomas with year incidence 3–8 cases per 100 000 people. Even though we mainly speak about benign disease, with its character and localization, it worsens the life quality to its bearer. The main therapeutic methods are neurosurgery extirpation and stereotactic radiosurgery. Despite reaching very good therapeutic results with both methods, some of the patients suffer with recurrence of the disease requiring other interventions with the risk of creating or worsening a neurological deficit. Small meningiomas without growing tendency and with the absence of symptomatology are observed. On the other hand, we cannot be satisfied with long-term results of the therapy of meningiomas with more aggressive biological behavior. A higher grade is a known risk of meningioma recurrence; however, it is often difficult to identify subgroups that show advantage of a combination of neurosurgery and radiosurgery, respectfully at which it is appropriate to look for ways to treatment escalation, and that is with the risk of side effects. The analysis of hormonal, cytogenetics and epigenetics tumor equipment seems to be the most promising for the development of personalized treatment. Purpose: The aim of this work is to sum up the facts about epidemiology, treatment algorithms and the prognosis of brain meningiomas and to suggest perspective prognostic and predictive factors for maximally individualized therapy.

Východiska: Nazofaryngeální tumory jsou v našich zeměpisných šířkách vzácným onemocněním. Kvůli anatomickým poměrům a špatné operabilitě je jako primární léčebná modalita využívána radikální radioterapie s chemoterapií nebo bez ní. Vzhledem k očekávanému dlouhodobému přežití jsou hledány cesty ke zvýšení lokální kontroly za únosné toxicity léčby. Soubor pacientů a metody: Na Klinice onkologické Fakultní nemocnice Ostrava bylo v letech 2010–2014 léčeno 14 pacientů s pokročilým nemetastatickým spinocelulárním karcinomem nazofaryngu stadia II–IVa. Jedenáct (79 %) pacientů bylo léčeno dávkou 70 Gy na primární tumor a postižené lymfatické uzliny normofrakcionovaně (à 2 Gy) a 56 Gy (à 1,6 Gy) na elektivní lymfatické uzliny technikou radioterapie s modulovanou intenzitou s integrovaným konkomitantním boostem. Tři (21 %) pacienti z důvodů komplikací nebo celkového stavu ukončili radioterapii dříve. Devět (64 %) pacientů bylo ozářeno s konkomitantní chemoterapií a 6 (43 %) pacientů dále podstoupilo adjuvantní chemoterapii. Čtrnáct (100 %) pacientů do měsíce od radikální radioterapie absolvovalo cyberboost na oblast primárního tumoru a metastatických lymfatických uzlin, 5 (36 %) nemocných 5 Gy v 1 frakci a 9 (64 %) nemocných 10 Gy ve 2 frakcích. Výsledky: Z 14 pacientů dosáhlo lokální kompletní remise po léčbě 10 (71 %) pacientů. Dva (14 %) pacienti měli poléčebný lokální nález zhodnocen jako stacionární, u 1 byla konstatována lokální progrese s generalizací za 18 měsíců a u druhého pacienta za 13 měsíců. Jeden pacient s poradiační kompletní remisí zgeneralizoval za 43 měsíců při lokální kompletní remisi onemocnění. Průměr 24,6 měsíce do progrese. Pětiletého přežití dosáhlo 9 (64 %) pacientů za akceptovatelné akutní i pozdní toxicity. Závěr: Stereotaktický boost po absolvování komplexního radiačního programu, s přidáním konkomitantní a adjuvantní chemoterapie nebo bez ní, přináší dobrou lokální kontrolu a 5leté přežití za uspokojivé toxicity léčby.
Background: Nasopharyngeal tumors are a rare disease in our latitudes. Due to anatomical conditions and poor operability, radical radiotherapy with or without chemotherapy is used as a primary treatment modality. Due to the unsatisfactory treatment outcomes, ways to intensify the therapy and increase the curable potential under tolerable toxicity are sought. Patients and methods: Fourteen patients with advanced nasopharyngeal non-metastatic squamous cell carcinoma, stage II–IVa, were treated at the Department of Oncology of the University Hospital Ostrava in 2010–2014. Eleven (79%) patients were treated with 70 Gy per primary tumor normalized (2 Gy) and affected lymph nodes and with 56 Gy (à 1,6 Gy) for elective lymph nodes by intensity modulated radiotherapy with integrated concomitant boost. Three (21%) patients completed radiotherapy earlier due to complications or general condition. Nine (64%) patients were exposed to concomitant chemotherapy and 6 (43%) received adjuvant chemotherapy. Fourteen (100%) patients underwent cyberboost in the primary tumor and affected lymph nodes (if any) area within a month of radical radiotherapy, five (36%) patients received 5 Gy in one fraction and nine (64%) patients received 10 Gy in 2 fractions. Results: Out of the 14 patients, local complete remission was achieved in 10 (71%) of them after the treatment. Two (14%) patients had a post-treatment local finding as stationary, local progression with generalization occurred in one case at 18 months and in another patient at 13 months. In one patient with complete remission after the primary treatment, distant metastasis occurred in 43 months at the time of local complete remission of the disease. The average time to progression was 24.6 months. Five (64%) patients achieved 5-year survival with satisfactory toxicity of treatment. Conclusion: Stereotactic boost after a comprehensive radiation program, with or without addition of concomitant and adjuvant chemotherapy, provides good local control and 5-year survival with satisfying toxicity of the treatment.

BACKGROUND AND AIMS: Modern stereotactic body radiotherapy (SBRT) techniques and systems that use online image guidance offer frameless radiotherapy of spinal tumors and the ability to control intrafraction motion during treatment. These systems allow precise alignment of the patient during the entire treatment session and react immediately to random changes in this alignment. Online tracking data provide information about intrafractional changes, and this information can be useful for designing treatment strategies even if online tracking is not being used. The present study evaluated spine motion during SBRT treatment to assess the risk of verifying patient alignment only prior to starting treatment. METHODS: This study included 123 patients treated with spine SBRT. We analyzed different locations within the spine using system log files generated during treatment, which contain information about differences in the pretreatment reference spine positions by CT versus positions during SBRT treatment. The mean spine motion and intra/interfraction motion was evaluated. We defined and assessed the spine stability and spine significant shifts (SSHs) during treatment. RESULTS: We analyzed 462 fractions. For the cervical (C) spine, the greatest shifts were in the anterior-posterior (AP) direction (2.48 mm) and in pitch rotation (1.75 deg). The thoracic (Th) spine showed the biggest shift in the AP direction (3.68 mm) and in roll rotation (1.66 deg). For the lumbar-sacral (LS) spine, the biggest shift was found for left-right (LR) translation (3.81 mm) and roll rotation (3.67 deg). No C spine case exceeded 1 mm/1 deg for interfraction variability, but 7 of 54 Th spine cases exceeded 1 mm interfraction variability for translations (maximum value, 2.5 mm in the AP direction). The interfraction variability for translations exceeded 1 mm in 2 of 24 LS spine cases (maximum value, 1.7 mm in the LR direction). Only 13% of cases had no SSHs. The mean times to SSH were 6.5±3.9 min, 8.1±5.9 min, and 8.8±7.1 min for the C, Th, and LS spine, respectively, and the mean recorded SSH values were 1.6±0.66, 1.43±0.33, and 1.46±0.47 mm/deg, respectively. CONCLUSION: Positional tracking during spine SBRT treatments revealed low mean translational and rotational shifts. Patient immobilization did not improve spine shifts compared with our results for the Th and LS spine without immobilization. For the most precise spine SBRT, we recommend checking the patient's position during treatment.

Nenádorová radioterapie se používá nejčastěji k léčbě zánětlivých i degenerativních nemocí pohybového ústrojí. Stereotaktické techniky se nejčastěji používají pro léčbu neuralgie trigeminu a perspektivně se jeví i její využití při terapii funkčních onemocnění. Kromě České republiky je nenádorová radioterapie velmi využívána i v Německu, kdy počty pacientů s nenádorovou radioterapií převyšují počty onkologických pacientů. Při uvážlivé indikaci se jedná o vysoce efektivní a bezpečnou metodu, která zjevně splňuje požadavky na radiační ochranu.
Non-cancer radiotherapy is most commonly used to treat inflammatory and degenerative diseases of the locomotor system. Stereotactic techniques are typically used for the treatment of trigeminal neuralgia and their use in treating functional diseases also appears to be promising. Apart from the Czech Republic, non-cancer radiotherapy is used widely in Germany where the numbers of patients receiving non-cancer radiotherapy exceed the numbers of cancer patients. When indicated with discretion, it is a highly effective and safe method which clearly meets the requirements for radiation protection.
