BACKGROUND: The use of patient-reported outcomes (PRO) in clinical practice is gaining increasing attention. This study aimed to provide a critical assessment of the current state-of-the-art and beliefs about the use of PRO in the management of people with epilepsy across some European countries. METHODS: Structured interviews were conducted with European experts to collect insights about (I) the personal experience with PRO; (II) the value and impact of PRO in the decision-making process at the national level; and (III) the interest for and use of PRO by national health authorities. RESULTS: Nine neurologists (Austria, Belgium, Czechia, Denmark, France, Greece, Italy, Poland, and United Kingdom), three health economists (Portugal, Romania, and Sweden), and one epidemiologist (Slovakia) participated. They all stated that PRO are collected at their own countries in the context of clinical trials and/or specific projects. During everyday clinical practice, PRO are collected routinely/almost routinely in Austria and Sweden and only at the discretion of the treating physicians in Czechia, Denmark, France, Greece, and Portugal. There was complete consensus about the favorable impact that the PRO can have in terms of clinical outcomes, healthcare resources utilization, and general patient satisfaction. Only participants from Portugal and Sweden answered that the PRO are perceived as very important by the National Health Authorities of their respective countries. CONCLUSIONS: Differences exist in attitudes and perspectives about PRO in epilepsy across Europe. An active plan is warranted to harmonize the measurement of PRO and ensure they can be relevant to people with epilepsy and health services.

Aim: The study aimed to describe the various ways in which Italian midwives reconcile their profession's philosophical views about labor with the use of epidural analgesia in clinical practice. Design: Interpretive description. Methods: A purposeful sample of 41 midwives and 12 midwifery students participated in the study. Information about their perceptions and experiences of the use of epidural analgesia and how this practice aligns with their professional values were explored through in-depth interviews (n = 10 focus groups; n = 1 one-to-one interview). The constant comparative method was used to analyze the data. Results: With respect to aligning midwifery care with the administration of epidural analgesia, two midwifery positions were identified: the midwife that acted "in harmony" and the "disoriented midwife", defined by three dimensions: 1) the midwife's conception of her professional role; 2) the woman's attitude towards labor and; 3) the midwife's relationship and comfort with other professionals in the labor room. Conclusion: The results of this research highlight how difficult it can be for a midwife to reconcile a specific philosophical view of labor with the use of epidural analgesia. These findings can provide useful insights to help midwives in the challenging task of combining epidural analgesia with their philosophical view of labor to offer a better birth experience to women.

Dnes je již dobře známo, že onemocnění covid-19 může mít i různé extrapulmonální projevy. Mezi možné mechanismy postižení kardiovaskulárního systému patří přímé poškození myokardu v důsledku hyperkoagulability vyvolané systémovým zánětem, což může vyústit i v destabilizaci plátů v koronárních tepnách. Popisujeme případ 30letého mladého muže po asymptomatickém onemocnění covid-19, který byl přijat na oddělení akutních příjmů nemocnice Grande Ospedale Metropolitano di Reggio Calabria (Itálie) s typickou bolestí na hrudi. U pacienta nebyla stanovena diagnóza kardiovaskulárního onemocnění, nebyly přítomny žádné rizikové faktory, ani kardiovaskulární onemocnění v rodinné anamnéze. Urgentní koronarografie prokázala kritickou (80%) stenózu ve střední a proximální části r. interventricularis anterior tvořenou plátem vzhledu ateromového vředu. Naše kazuistika upozorňuje na nutnost - bez ohledu na rizikové faktory kardiovaskulárního onemocnění - ověřit přítomnost či nepřítomnost onemocnění covid-19 u všech pacientů s akutním infarktem myokardu během této pandemie.
It is now widely known that COVID-19 can also cause various extrapulmonary manifestations. Possible mechanisms of cardiovascular damage include direct myocardial damage due to hypercoagulability caused by systemic inflammation, which may also lead to destabilisation of coronary plaques. We present a case of a 30-year-old young man following asymptomatic COVID-19 infection, who was admit- ted to the emergency room of the Grande Ospedale Metropolitano di Reggio Calabria, Italy, with typical chest pain. The patient had no cardiovascular disease, cardiovascular risk factors including no family history of cardiovascular disease. Urgent coronary angiography showed critical stenosis of the middle-proximal portion of the left anterior descending artery (80%) with ulcerated plaque appearance. Regardless of cardiovascular risk factors, our case report emphasises the need to determine COVID-19 status in all patients with acute myocardial infarction during this pandemic.

Úvod: Měření práce myokardu (myocardial work, MW) představuje novou echokardiografickou metodu založenou na sledování smyčky tlak-deformace srdeční komory, což umožňuje kvantifikovat výkonnost srdce. Na druhé straně rychlost pulsní vlny (pulse wave velocity, PWV) určuje tuhost tepen na základě poznatku, že se zvyšující se tuhostí tepny se zvyšuje i rychlost anterográdního a retrográdního přenosu sfygmické vlny. Cíl: Cílem této studie bylo stanovit korelaci mezi parametry MW a PWV. Metody: Do studie jsme zařazovali všechny po sobě následující pacienty bez kardiovaskulárního onemocnění, kteří v období mezi červnem 2021 a červencem 2022 absolvovali transtorakální dopplerovské echokardio- grafické vyšetření. Hodnoty MW byly vypočítány ze smyčky tlak-deformace srdeční komory, do níž byly začleněny hodnoty neinvazivního vyšetření tepenného tlaku podle doporučení pro standardní echokardiogracfické vyšetření metodou "speckle tracking". Hodnota PWV se měřila tonometrem na úrovni společné karotidy a společné femorální tepny. Výsledky: Celkem bylo do studie zařazeno 66 pacientů průměrného věku 30,7 ± 8,6 roku. Byla nalezena statisticky významná negativní korelace mezi PWV a celkovou zbytečně vynaloženou energií (global wasted energy, GWE) (r = -0,317; p < 0,01) při korelaci s celkovou zbytečně vynaloženou prací (global wasted work, GWW) (r = 0,324; p < 0,01). Statisticky významná korelace přetrvávala v podskupinách žen i mužů u GWE (ženy: r = -0,280; p < 0,05; muži: r = -0,362; p < 0,05) i u GWW (ženy: r = 0,359; p < 0,05; muži: r = 0,359; p < 0,05). Závěr: Vztah mezi MW a PWV jako projev součinnosti mezi levou komorou a velkými tepnami může potenciálně představovat užitečný nástroj pro časné odhalení subklinické dysfunkce kardiovaskulárního systému.
Introduction: The myocardial work (MW) is a new echocardiographic method, based on the pressure-strain loop, which allows quantifying the cardiac performance. On the other hand, the pulse wave velocity (PWV) evaluates arterial stiffness, knowing that as the stiffness of an artery increases, the transmission velocity of the anterograde and the retrograde sphygmic wave increases. Purpose: The aim of the study is to evaluate the correlation between MW and PWV parameters. Methods: We enrolled consecutively all patients without cardiovascular disease who underwent transthora- cic Doppler echocardiography between June 2021 and July 2022. The MW parameters were derived from the strain-pressure loop, including in its calculation the measurement non-invasive arterial pressure, according to standard speckle tracking echocardiography recommendations. The PWV measurement was obtained by tonometry at the level of the common carotid artery and the common femoral artery. Results: We enrolled 66 patients (mean age: 30.7±8.6 years). There was a significant inversely proportio- nal correlation between PWV and GWE (r = –0.317; p <0.01) meanwhile there was a directly proportional correlation with GWW (r = 0.324; p <0.01). The statistically significant correlation remained in the female and male subgroups for GWE (female: r = –0.280; p <0.05; male: r = –0.362; p <0.05) and GWW (female: r = 0.359; p <0.05; male: r = 0.359; p <0.05). Conclusion: The relationship between MW and PWV as a demonstration of ventricular arterial coupling may potentially be a useful tool in the early recognition of subclinical cardiovascular dysfunction.

The European Society for Clinical Nutrition and Metabolism (ESPEN) and the European Association for the Study of Obesity (EASO) launched the Sarcopenic Obesity Global Leadership Initiative (SOGLI) to reach expert consensus on a definition and diagnostic criteria for Sarcopenic Obesity (SO). The present paper describes the proceeding of the Sarcopenic Obesity Global Leadership Initiative (SOGLI) meeting that was held on November 25th and 26th, 2022 in Rome, Italy. This consortium involved the participation of 50 researchers from different geographic regions and countries. The document outlines an agenda advocated by the SOGLI expert panel regarding the pathophysiology, screening, diagnosis, staging and treatment of SO that needs to be prioritized for future research in the field.
Vulnerability to land degradation in southern Europe has increased substantially in the last decades because of climate and land-use change, soil deterioration, and rising human pressure. The present work focuses on a quantitative evaluation of changes over time in the level of vulnerability to land degradation of a Mediterranean country (Italy) using a composite indicator, the environmentally sensitive area index (ESAI), which is the final outcome of a complex model conceived to assess land vulnerability on the basis of climate, soil, vegetation, and human pressure. Considering four different levels of vulnerability to land degradation (not affected, potentially affected, fragile, and critical), the main trajectories of this index were highlighted in a long-time perspective (1960-2010), discriminating dynamics over two sub-periods (1960-1990 and 1990-2010). The empirical results at a very detailed spatial scale (1 km2 grid) reflect spatial consolidation of degradation hot-spots over time. However, aggregated trajectories of change indicate an overall improvement in the environmental conditions between 1990 and 2010 compared with what is observed during the first period (1960-1990). Worse environmental conditions concerned southern Italian regions with a dry climate and poor soil conditions in the first time interval, large parts of northern Italy, traditionally recognized as a wet and affluent agricultural region, experienced increasing levels of land vulnerability in the second time interval. Being classified as an unaffected region according with the Italian national action plan (NAP), the expansion of (originally sparse) degradation hot-spots in northern Italy, reflective of an overall increase in critical areas, suggests a substantial re-thinking of the Italian NAP. This may lead to a redesign of individual regional action plans (RAPs) implementing place-specific approaches and comprehensive measures to be adopted to mitigate land degradation.
