(1) Objectives: Intestinal failure in home parenteral nutrition patients (HPNPs) results in oxidative stress and liver damage. This study investigated how a high dose of fish oil (FO) added to various lipid emulsions influences antioxidant status and liver function markers in HPNPs. (2) Methods: Twelve HPNPs receiving Smoflipid for at least 3 months were given FO (Omegaven) for a further 4 weeks. Then, the patients were randomized to subsequently receive Lipoplus and ClinOleic for 6 weeks or vice versa plus 4 weeks of Omegaven after each cycle in a crossover design. Twelve age- and sex-matched healthy controls (HCs) were included. (3) Results: Superoxide dismutase (SOD1) activity and oxidized-low-density lipoprotein concentration were higher in all baseline HPN regimens compared to HCs. The Omegaven lowered SOD1 compared to baseline regimens and thus normalized it toward HCs. Lower paraoxonase 1 activity and fibroblast growth factor 19 (FGF19) concentration and, on the converse, higher alkaline phosphatase activity and cholesten concentration were observed in all baseline regimens compared to HCs. A close correlation was observed between FGF19 and SOD1 in baseline regimens. (4) Conclusions: An escalated dose of FO normalized SOD1 activity in HPNPs toward that of HCs. Bile acid metabolism was altered in HPNPs without signs of significant cholestasis and not affected by Omegaven.
Úvod: Senioři s demencí jsou vysoce závislí na pečovatelích; jak nemoc postupuje a její závažnost se zvyšuje, vyžadují větší pomoc a dohled. Rodinní pečovatelé mají různé reakce na péči o seniory s demencí. Cílem této studie bylo prozkoumat reakce indonéských rodinných pečovatelů na péči o seniory s demencí. Metody: Pro tuto studii byl vybrán kvalitativní výzkumný design. Na základě polostrukturovaných rozhovorů byla provedena deskriptivní fenomenologická studie mezi 15 účelově vybranými rodinnými pečovateli. Výsledky: Z dat vyplynula tři hlavní témata a osm podtémat. Prvním tématem bylo vnímání demence ze strany rodinných pečovatelů. Dílčími tématy bylo, že demence je běžné onemocnění způsobené procesem stárnutí a demence je onemocnění způsobené Parkinsonovou chorobou, mrtvicí, stresem a osamělostí. Druhým tématem byl smysl péče o seniory s demencí. Dílčími tématy byly povinnost pečovat o seniory s demencí, forma filiální zbožnosti a duchovní smysl péče o seniory s demencí. Třetím tématem byl proces přijetí do role pečovatele o seniora s demencí. Dílčími tématy byly popírání, deprese a přijímání. Závěr: Každé z těchto témat má kulturní a duchovní rozměr a pečovatelé vyžadují odbornou pomoc zdravotníků, která odpovídá těmto rozměrům péče. Stejně tak je důležité identifikovat depresi u rodinných pečovatelů, aby mohly být poskytnuty vhodné intervence
Introduction: Elderly people with dementia are highly dependent on caregivers; they require more assistance and supervision as the disease progresses and its severity increases. Family caregivers have different responses to caring for elderly people with dementia. This study aimed to explore Indonesian family caregivers' responses to caring for elderly people with dementia. Methods: A qualitative research design was selected for this study. On the basis of semi-structured interviews, a descriptive phenomenological study was conducted among 15 purposively-recruited family caregivers. Results: Three main themes and eight sub-themes emerged from the data. The first theme was family caregivers' perceptions of dementia. The sub-themes were that dementia is a common disease due to the aging process, and dementia is a disease due to Parkinson's, stroke, stress, and loneliness. The second theme was the meaning of caring for elderly people with dementia. The sub-themes were that obligation to care for older people with dementia, form of filial piety and the spiritual meaning of caring for older people with dementia. The third theme was the process of acceptance in taking on the role of caregiver to an elderly person with dementia. The sub-themes were that denial, depression, and acceptance. Conclusion: Each of these themes has a cultural and spiritual dimension, and caregivers require professional assistance from health workers that accounts for these dimensions of caregiving. Likewise, it is important to identify depression in family caregivers so that suitable interventions can be provided.

It is important for older and disabled people who live alone to be able to cope with the daily challenges of living at home. In order to support independent living, the Smart Home Care (SHC) concept offers the possibility of providing comfortable control of operational and technical functions using a mobile robot for operating and assisting activities to support independent living for elderly and disabled people. This article presents a unique proposal for the implementation of interoperability between a mobile robot and KNX technology in a home environment within SHC automation to determine the presence of people and occupancy of occupied spaces in SHC using measured operational and technical variables (to determine the quality of the indoor environment), such as temperature, relative humidity, light intensity, and CO2 concentration, and to locate occupancy in SHC spaces using magnetic contacts monitoring the opening/closing of windows and doors by indirectly monitoring occupancy without the use of cameras. In this article, a novel method using nonlinear autoregressive Neural Networks (NN) with exogenous inputs and nonlinear autoregressive is used to predict the CO2 concentration waveform to transmit the information from KNX technology to mobile robots for monitoring and determining the occupancy of people in SHC with better than 98% accuracy.

Falls are a serious problem in the hospital setting and home environments. However, this problem does not only affect the elderly, but also people who have had surgery, have disabling problems, have associated diagnoses (such as poor eyesight, confusion, etc.) or are dizzy or have walking aids. The aim of research was to find, compare and implement fall detectors especially for the hospital environment. This paper summarizes possible fall detectors. Various technological solutions were selected for testing, including wearable technologies as well as contactless technologies based on PIR detectors and mmWave technologies. The selected fall detectors were tested in living laboratory of HEALTHLab.vsb.cz and then in Hospital AGEL Třinec - Podlesí. The best result of the testing was the use of two Vayyar Home Care devices in one room, thus achieving a detection accuracy of 92.50 % and a sensitivity of 92.50 %.

Aim: The aim was to understand the lived experiences of family caregivers caring for a dying patient at home. Design: A qualitative phenomenological approach was used. Methods: Data were collected between September and December 2018. Data collection was took the form of in-depth interviews with family caregivers (n = 4), followed by the use of interpretative phenomenological analysis (IPA). Results: Five main themes were identified in terms of which family caregivers described their experience: the Role of caregiver; I'll do everything for him / her; Support; "Time out" after death; and Benefits of care. The family caregivers had to establish their new caregiver roles, often they were the mainstay of the whole family. In the various areas of care they did everything for the dying person. They experienced pain, mental and physical burdens, and doubts, and spirituality was vital to them. It was important to experience support from different sources. After this experience, the caregivers needed time to release emotions after the death. Overall, the caregivers came to regard this experience as beneficial: revealing other priorities, increasing time spent with family, clarifying relationships, and changing attitudes and values. Conclusion: The study offers a unique insight into family caregivers' experiences in the Czech Republic. In confirmation of previous results, experiences are generally similar for all caregivers and point to a need for significant improvement in the quality of support and assistance from health professionals and support from family members, particularly in the areas of practical demonstration of care, the expression of condolences, provision of pain medication, handling of formalities related to the death of a loved one (e.g., when arranging the death certificate and widow's pension), and the sharing of emotions as they occur.
