Východisko: Prevalence onemocnění diabetes mellitus 2. typu (DM2) má celosvětově narůstající charakter. S počtem případů, přesahujícím 430 milionů, se diabetes řadí mezi nejčastěji vyskytující se neinfekční choroby na světě, přičemž incidence onemocnění prudce narůstá u osob starších 65 let. Přirozené involuční procesy i komorbidity u seniorů mohu vést k výskytu tzv. geriatrických syndromů, které se významnou měrou podílejí na terapii, prognóze i kvalitě života pacienta. Z posledních výzkumů vyplývá, že diabetes mellitus 2. typu může hrát významnou roli při vzniku i prognóze geriatrických syndromů. Studie naznačují reciproční vztahy v etiologii i genezi mezi diabetes mellitus 2. typu, depresí a kognitivními poruchami. Cíl a účel sdělení: Cílem prezentace je na základě mapping review odhalit opakující se zjištění souvislosti diabetes mellitus 2. typu, kognitivních poruch a deprese jako geriatrických syndromů ve vztahu geneze i následků onemocnění. Metody: Ke zpracování dat z odborných článků byla zvolena metoda maping review, tj. analýza souboru odborných prací za vymezené období 2009–2019, včetně rešeršní strategie a přehledová tabulka těchto studií včetně jejich zkrácených výstupů a lokalizace, která v prezentaci bude uvedena také. Rovněž bude uvedeno grafické ztvárnění výsledků studií do systematických celků se znázorněním vzájemných vztahů. Výsledky: Review odhalilo opakující se zjištění souvislosti diabetes mellitus 2. typu a deprese ve vztahu geneze i následků onemocnění, přičemž obě zmiňovaná onemocnění pak mohou vést k poruchám kognice. Nerozpoznaná porucha kognice může naopak komplikovat terapii diabetes mellitus 2. typu a vést k dekompenzaci tohoto onemocnění. Závěr: Popis a analýza vzájemných vztahů mezi diabetes mellitus 2. typu a uvedenými geriatrickými syndromy, jak je zkoumána v uvedené literatuře, s cílem efektivní léčby deprese i kognitivního deficitu vede ke zlepšení compliance a adherence pacienta i samotné úpravě diabetes mellitus 2. typu. Tato forma sekundární prevence by mohla významně zvýšit kvalitu života seniorů.
Background: The prevalence of type 2 diabetes mellitus (DM2) is increasing worldwide. With more than 430 million cases, diabetes ranks among the most common non-infectious diseases in the world. The incidence of the disease is rising sharply in people over 65 years of age. Natural involutional processes and comorbidities in the elderly can lead to the occurrence of the geriatric syndrome, which significantly affects the therapy, prognosis and quality of life of the patient. Recent research shows that DM2 can play a significant role in the development and prognosis of geriatric syndromes. Studies suggest reciprocal relationships in both etiology and genesis between DM2, depression, and cognitive impairment. Aim and purpose of the message: The aim of the presentation is to reveal, with a mapping review, the repeated findings of the connection between DM2, cognitive disorders and depression in relation to the genesis and consequences of the disease. Methods: The methodology for collecting and processing data from professional articles was chosen as a mapping review – an analysis of scientific papers from the defined period 2009–2019 and a graphical representation of their results into systematic units with a representation of mutual relationships. The presentation will also include an overview table of the studies, including their short outputs and localization. Results: The mapping review revealed repeated findings of the connection between DM2 and depression in relation to the genesis and consequences of the disease, while both mentioned diseases can then lead to cognitive disorders. An unrecognized cognitive disorder can, on the other hand, complicate DM2 therapy and lead to decompensation of this disease. Conclusion: Unraveling the interrelationships between DM2 and the mentioned geriatric syndromes with the aim of effective treatment of depression and cognitive deficit leads to improvement of patient compliance and adherence as well as the treatment of DM2. This form of secondary prevention could significantly increase the quality of life of seniors.

AIMS: We investigated the quality of life (QoL), treatment satisfaction and perception of genetic results in participants with Maturity-Onset Diabetes of the Young (MODY) and compared the results with those of subjects with type 1 (T1D) or type 2 (T2D) diabetes. METHODS: A total of 162 adults with GCK-MODY, 62 with HNF1A-MODY and 29 with HNF4A-MODY answered the questionnaire Audit of Diabetes Dependent Quality of Life, the Diabetes Treatment Satisfaction Questionnaire and non-validated instrument examining the respondent's perception of the genetic results. Data from GCK-MODY patients were compared with 84 participants with T2D and HNF-MODY subjects were compared with 81 participants having T1D. RESULTS: Higher age (p=0.004), higher haemoglobin A1c (p=0.026) and medication (p=0.019) were associated with lower general QoL in GCK-MODY patients. In HNF-MODY patients, lower general QoL was associated with a longer time since diagnosis (p=0.005), worse haemoglobin bA1c (p=0.006) and insulin treatment (p=0.019). Similar numbers of participants with GCK- and HNF-MODY considered the genetic diagnosis of MODY to be positive, negative and without significance. The patient with GCK-MODY did not differ from those with T2D in terms of their QoL, but they were less satisfied with their treatment (p<0.001). QoL was better in patients with HNF-MODY compared with patients with T1D (p=0.006), and they did not differ in terms of treatment satisfaction. CONCLUSIONS: QoL was affected in both GCK-MODY and HNF-MODY subjects. Apprehension of genetic diagnosis was not single-valued in MODY respondents.

Aim: The objective of this study was to translate and evaluate the psychometric properties of the Thai version of the Diabetes Self-Management Scale (T-DSMS). Design: A cross-sectional study. Methods: This study was performed in two phases: 1) a forward-backwards translation of the existing version of the Diabetes Self-Management Scale (DSMS); and 2) evaluation of its psychometric properties using confirmatory factor analysis. Between February to June 2016, a total of 700 type 2 diabetes mellitus (T2DM) patients were recruited from outpatient diabetes clinics of both community and university hospitals in Khon Kaen and Bangkok province Thailand, using a self-administered questionnaire. Results: The results, based on confirmatory factor analysis using Unweighted Least Squares, confirmed the construct validity of the T-DSMS (CFI = 0.985; RMSEA = 0.258; TLI = 0.981; SRMR = 0.040). The T-DSMS contains 13 items across four domains: Diet, Blood, Exercise, and Foot care. Conclusion: We translated and appropriately validated the DSMS in Thai T2DM patients. The T-DSMS was shown to have good psychometric properties, including content validity and criterion validity. The T-DSMS is an adequate instrument with which to assess diabetes self-management in Thai T2DM patients, and can provide valuable insights into the epidemiology of diabetes self-management, and enable evaluation of prevention programs for diabetes self-management in T2DM patients.

Chtěl bych popsat příběh pacientky, na který nejsem jako její diabetolog zrovna pyšný. Jde o příběh diabetičky 2. typu, 83leté ženy, u které jsem vyzkoušel konvenční a nejčastěji používané způsoby terapie a stejně jsem dlouho nebyl úspěšný ve snaze o lepší kompenzaci jejího diabetu. Až jedna z hospitalizací pacientky v nemocnici pomohla nastartovat obrat v její léčbě.
As the patient's diabetologist I am not proud of her case report, but I would like to share it. The patient is 83-year-old, she has the type 2 diabetes mellitus and she was treated by a conventional and the most commonly used therapy. Nevertheless, it was not possible to achieve a better glucose control for a long time. Only one of her hospitalizations helped to initiate the turning point in her treatment.

Komplexní pohled na současného diabetika se v dnešní době stává nutností. Diabetes mellitus je chronické vleklé onemocnění, které s postupnou délkou jeho trvání a prodlužující se věkovou časovou osou z metabolického onemocnění vytváří spletitou síť napříč všemi odbornostmi. V současné době je v diabetologii pro pacienty rozsáhlé spektrum antidiabetické terapie, zvláště pro pacienty s diabetem 2. typu. Taktika léčby by měla být ryze individuální. Léčba perorálními antidiabetiky je dominantou v péči o diabetiky 2. typu, z léčby pomocí inzulínu mohou profitovat jak diabetici 1. tak 2. typu. Dlouhodobě působící analoga přinášejí pro pacienty celou řadu benefitů. Hlavním benefitem je zejména snížení rizika hypoglykemie a s tím související zlepšení kvality života a snížení komplikací jak zdravotních, tak i sociálních. Terapie inzulínem glargin 300 U/ml i v reálné praxi redukuje variabilitu glykemií, umožňuje lepší compliance a adherenci k léčbě. Předpokladem racionální individualizované léčby diabetiků je odebrání podrobné anamnézy, zohlednění na jedné straně všech úrovní pacientova života, a to složky zdravotní, sociální, psychické, finanční, tak na druhé straně všech možností současné antidiabetické terapie se zohledněním všech dostupných informací o benefitech jednotlivé kategorie léčiv.
Nowadays, it becomes inevitable to accept a complex approach to diabetic patients. Diabetes mellitus is a chronic long-lasting disease that due to its gradual progress and increased life expectancy develops from a simply metabolic disease into a complex net across all medical specialties. Diabetology now offers a broad range of antidiabetic treatment, especially for patients with the type 2 diabetes mellitus. Therapeutical strategy should be highly individualized. The treatment with oral antidiabetic agents dominates in patients with the type 2 diabetes mellitus, both patients with the type 1 and the type 2 may profit from insulin therapy. Long acting insulin analogues bring many benefits for patients. The main benefit is a reduced risk of hypoglycaemic episodes, which is accompanied by an improved quality of life and less medical and social complications. A treatment with insulin glargine 300 U/mL reduces a variability of glucose levels, enables better compliance and adherence to the treatment in a real practice. A knowledge of a thorough medical history and a careful consideration of all aspects of a patient’s life, i.e. medical, social, mental and financial aspects on one hand, as well as all options of a current antidiabetic treatment having regard all available information about benefits of individual categories of medical products on the other hand, are preconditions of a rational individualized treatment in patients with diabetes.

Převod diabetického pacienta z léčby perorálními antidiabetiky na injekční formy léčby, ať už se jedná o inzulinoterapii nebo aplikaci GLP-1 analog, může být komplikován, ale někdy také usnadněn vnějšími faktory, které nemusí přímo souviset ani s osobou lékaře, ani s pacientem. Tento určitý akt razantní změny při absenci subjektivních obtíží, který je prováděn jen na základě jakýchsi „čísel” sledovaných diabetologem, je pro pacienta mnohdy nepochopitelný. Budeme se ale muset připravit na tyto čím dál častější situace, protože tak jak se neustále zpřísňuje naše hledisko ideální kompenzace diabetu měřené glykemií a HbA1c, měníme terapii většinou v subklinickém stavu. V uvedené kazuistice bylo důležitým faktorem, který pomohl ke změně léčby (zavedení inzulinoterapie), provedení amputace dolní končetiny (DK) u manžela pacientky, který má rovněž diabetes. Tento okamžik vedoucí k razantní změně kvality života diabetika dokázal pacientku posunout k uvědomění si nutnosti změny, pokud chceme podobným komplikacím zamezit. Přes počáteční úspěch inzulinoterapie ve zlepšení kompenzace diabetu došlo ale po určité době k jejímu selhání a bylo potřeba nabídnout nový impuls v terapii, který by opět zajistil požadované parametry. Takovým impulzem byla změna bazálního analoga detemir na inzulín glargin o koncentraci 300 jednotek/ml (Gla-300). Tato změna se ukázala jako správný krok a potvrdila užitečnost používání nových bazálních inzulínových analog – v tomto případě Gla-300. Tato dyskomfortní léčebná změna byla u referované pacientky paradoxně oddalována dlouhým asymptomatickým obdobím, přestože úroveň kompenzace diabetu při zavedené perorální antidiabetické léčbě byla špatná. Kontroly se tak často mohly zdát jen jakousi společenskou záležitostí zakončenou výdejem receptů s neměnící se medikací. To bude do budoucna asi častější problém, když při současné možnosti použít vysoce efektivní antidiabetika bude snadné zajistit na dlouhou dobu velmi dobrou kompenzaci diabetu. Pacient pak nebude ochoten měnit po této delší bezproblémové době léčbu, a to nejen pro absenci subjektivních obtíží, ale už jen z podstaty změny, která je člověkem velmi často vnímána negativně. A nemusí to být jen změna zdravotní.
In diabetic patients a switch from oral antidiabetic agents to injective forms (either insulin treatment or administration of GLP-1 analogues) may be complicated or sometimes facilitated by external factors that are not necessarily directly associated with either physician's or patient's personalities. Such intense change of treatment without any patient's subjective complaints that is made only based on certain “numbers” monitored by diabetologist is often considered incomprehensible by patients. Nevertheless, we must get ready for such situations that are becoming more frequent as we, due to more strict judgement of optimal diabetes control measured by glucose levels and HbA1c, levels, mostly change treatment already during subclinical symptoms of the disease. In the presented case report the important factor that facilitated the change of patient's treatment (initiation if insulin administration) was the limb amputation of her husband, who had also diabetes. This moment resulting in a significant change of diabetic patient's quality of life helped our patient to accept the need of change the treatment in order to prevent such complications. Despite initially successful insulin treatment that improved her diabetes control this treatment failed after a certain time as well, and it was necessary to offer a new therapeutic impulse that could achieve desirable parameters. This impulse was a change of basal analogue detemir to Gla-300. This change was considered as an appropriate step and it proved the utility of using new basal insulin analogues – in this case Gla-300. In the presented patient this uncomfortable therapeutic change was paradoxically postponed due to a long asymptomatic period, even though the extent of diabetes control during the treatment with oral antidiabetic agents remained low. Regular check-ups were often conducted as just a kind of social conversation terminated by issuing prescription containing the same medication. As highly effective antidiabetic agents that are available nowadays provide excellent and a long lasting diabetes control, we will face this problem more and more frequently in future. After such longer lasting period of treatment without any problems patients will not be probably willing to change it, not only due to absence of their subjective complaints but also just by definition of change per se that is very often perceived as negative by people. And it does not need to be only a change of their health condition.

Kazuistika ukazuje přínos indikace inkretinového analoga semaglutidu (preparátOzempic) u 79leté ženy s diabetem 2. typu trvajícím 21 let. Tato žena nesnášela met-formin a trpěla inzulinofobií. Při monoterapii gliptiny u ní vzrostl HbA1caž na61 mmol/mol. Během čtyřměsíční ambulantní léčby semaglutidem se průměrná gly-kemie desetibodového profilu (MPG) snížila ze 7,4 mmol/l na 7 mmol/l, HbA1cpokleslz 61 mmol/mol na 49 mmol/mol a tělesná hmotnost z 80,9 kg na 77,7 kg. Neobjevilyse žádné nežádoucí projevy. Včasná indikace semaglutidu aplikovaného podkožně je-denkrát týdně tedy přispěla ke zlepšení všech sledovaných ukazatelů látkové přeměnysacharidů.
This case report demonstrates the benefits of the incretin analogue semaglutide(Ozempic) in a 79-year-old lady with long-lasting type 2 diabetes . She has been suf-fering from both metformin intolerance and insulinofobia. In the course of a long-lasting period of gliptin therapy, the patient's HbA1cconcentration increased to61 mmol/mol. During the following 4-month treatment with semaglutide, the patient'smean plasma glucose concentration of a 10-point daily profile (MPG) decreased from7.4 mmol/l to 7.0 mmol/l, her HbA1cconcentration decreased from 61 mmol/mol to49 mmol/mol and her body mass from 80,9 kg to 77,7 kg. No adverse events appeared.Therefore, the early indication of once-a-week subcutaneously administered sema-glutide resulted in the improvement of all investigated parameters of saccharide me-tabolism.
