PURPOSE: Group B streptococcus (GBS) remains a leading cause of invasive disease, mainly sepsis and meningitis, in infants < 3 months of age and of mortality among neonates. This study, a major component of the European DEVANI project (Design of a Vaccine Against Neonatal Infections) describes clinical and important microbiological characteristics of neonatal GBS diseases. It quantifies the rate of antenatal screening and intrapartum antibiotic prophylaxis among cases and identifies risk factors associated with an adverse outcome. METHODS: Clinical and microbiological data from 153 invasive neonatal cases (82 early-onset [EOD], 71 late-onset disease [LOD] cases) were collected in eight European countries from mid-2008 to end-2010. RESULTS: Respiratory distress was the most frequent clinical sign at onset of EOD, while meningitis is found in > 30% of LOD. The study revealed that 59% of mothers of EOD cases had not received antenatal screening, whilst GBS was detected in 48.5% of screened cases. Meningitis was associated with an adverse outcome in LOD cases, while prematurity and the presence of cardiocirculatory symptoms were associated with an adverse outcome in EOD cases. Capsular-polysaccharide type III was the most frequent in both EOD and LOD cases with regional differences in the clonal complex distribution. CONCLUSIONS: Standardizing recommendations related to neonatal GBS disease and increasing compliance might improve clinical care and the prevention of GBS EOD. But even full adherence to antenatal screening would miss a relevant number of EOD cases, thus, the most promising prophylactic approach against GBS EOD and LOD would be a vaccine for maternal immunization.

Sepse vyvolané beta-hemolytickým streptokokem skupiny B (GBS) Streptococcus agalacatiae patří k nejčastějším infekcím novorozeneckého období, a to i přes snížení incidence časných novorozeneckých infekcí (GBS EOD) v posledních 25 letech v důsledku zavedení těhotenského screeningu a následné cílené intrapartální antibiotické profylaxe. Asi 70 % pacientů s časnou GBS infekcí jsou donošení novorozenci, přesto jsou nejrizikovější skupinou novorozenci narození před 35. gestačním týdnem. Vedoucím klinickým symptomem jsou především dechové obtíže dítěte. Nejčastější klinickou manifestací GBS EOD je sepse a/nebo pneumonie, typicky asociovaná s různým stupněm orgánové dysfunkce. Na rozdíl od GBS EOD nejsou pozdní GBS infekce (GBS LOD) peripartální antibiotickou profylaxí ovlivněny a jejich výskyt zůstává neměnný. Nejdůležitějším rizikovým faktorem rozvoje GBS LOD je anamnéza porodu před 37. týdnem těhotenství. Vedoucím klinickým symptomem bývá horečka, respirační příznaky jsou přítomny vzácně. Nejčastější klinickou manifestací GBS LOD je sepse, téměř ve třetině případů provázená purulentní meningitidou.
Sepsis caused by group B streptococcus (GBS) remains among the most commonly occurring infections in the neonatal period despite the decrease in incidence of early onset neonatal disease (GBS EOD) over the last 25 years. This decline in incidence can be attributed to the screening of pregnant women and subsequent targeted intrapartum antibiotic prophylaxis upon positive culture testing. About 70 % of the cases of GBS EOD occur in term neonates, however the most vulnerable group of patients are neonates born prior to 35 weeks of gestation. The leading clinical symptom in infected neonates is respiratory distress and GBS EOD frequently manifests as sepsis and/or pneumonia which is typically associated with varying degrees of organ dysfunction. Unlike GBS EOD, late onset GBS infections (GBS LOD) are not influenced by peripartum antibiotic prophylaxis, and their incidence has remained unchanged during last decades. In such cases, preterm delivery is the most important risk factor (delivery before 37 weeks of gestation). Contrary to early onset infections, GBS LOD commonly clinically presents with fever and thereafter usually also manifests as sepsis, which is accompanied by purulent meningitis in approximately 30 % of cases.

INTRODUCTION: Maternal sepsis is a leading cause of maternal and neonatal mortality. Despite the availability of management protocols, there is disparity in case fatality rates for pregnancy-related sepsis compared to other maternity-related complications. The main aim of this systematic review was to assess concordance between international evidence-based guidelines for the prevention and management of childbirth-related bacterial infections. MATERIAL AND METHODS: The PRISMA statement was followed during the conduct and reporting of this review. PubMed was searched electronically from 2009 to November 2019 for clinical guidelines covering the topic of childbirth-related infections and specific searches for relevant guidelines on the websites of the top five international professional bodies most commonly identified by our searches. We did not apply any language restrictions. Guidelines were included if they provided any information about the prevention or management of childbirth-related bacterial infections irrespective of whether the guideline stated a recommendation or not. Two independent reviewers undertook study selection, decisions about inclusion of selected guidelines and data extraction. Extracted information was synthesized under the following topics: Asymptomatic bacteriuria; group B streptococcal infection (GBS); preterm premature rupture of membranes (P-PROM); intrauterine infection; procedures; maternal sepsis; miscellaneous. Concordance was defined as absence of contradictory information between the different guidelines with regards to a specific topic, subtopic or recommendation. Quality of included guidelines was assessed against the AGREE II guideline reporting domains. RESULTS: A total of 43 guidelines were selected of which 11 were excluded leaving 32 guidelines that fulfilled our inclusion criteria. None of the guidelines fulfilled all the quality assessment domains and 11 (34%) of the guidelines satisfied 1-2 of domains only. Two guidelines covered the topic of asymptomatic bacteriuria, nine for GBS, five for P-PROM and three covered each of intra-amniotic infections maternal sepsis, obstetric procedures and interventions topics. The remaining guidelines covered miscellaneous topics. CONCLUSIONS: There was concordance between guidelines with regards to several aspects in the prophylaxis and treatment of bacteriological infections in pregnancy. Nevertheless, there were several areas of discordance, some of which reached the extent of contradictory information as in the case of antenatal screening for GBS.

Onemocnění covid-19 způsobené koronavirem těžkého akutního respiračního syndromu 2 (SARS-CoV-2) představuje i nadále globální zdravotnický problém. Od vypuknutí pandemie počátkem roku 2020 byla hlášena řada případů onemocnění gravidních žen, u nichž tato infekce vedla k závažným komplikacím vč. předčasného porodu, potratu, intrauterinní růstové restrikce či nitroděložního odumření plodu. Z výsledků současných studií vyplývá, že obávaný transplacentární přenos infekce z matky na plod je spíše výjimečnou událostí a že uvedené komplikace vznikají spíše v důsledku poškození placentární tkáně. V tomto článku popisujeme dva případy tzv. SARS-CoV-2 placentitidy se zvláštním přihlédnutím k morfologii a diferenciální diagnóze této nově definované jednotky.
Covid-19 disease caused by severe acute respiratory syndrome 2 coronavirus (SARS-CoV-2) continues to be a global public health problem. Since the pandemic outbreak in early 2020, a number of cases have been reported in pregnant women whose infection has led to severe complications including preterm birth, miscarriage, intrauterine growth restriction or intrauterine fetal demise. The results of the current studies suggest that the transplacental transmission of infection from mother to fetus is a rare event and that the complications listed above are more likely due to damage of placental tissue. In this article, we describe two cases of SARS-CoV-2 placentitis with special consideration to the morphology and differential diagnosis of this newly defined entity.

Cíl studie: Popis případu úmrtí novorozence po porodu akutním císařským řezem ve 31. týdnu gravidity jako následek syfilitické infekce matky. Výsledky: Patnáctiletá primigravida v grav. hebd. 31+3 byla přeložena do intermediárního centra pro hrozící předčasný porod. Na základě patologického kardiotokografického záznamu bylo těhotenství ukončeno akutním císařským řezem s následným úmrtím novorozence po 35 min resuscitace. Z pupečníkové krve a nezávisle z testů provedených registrujícím gynekologem ve 30. týdnu těhotenství byla hlášena pozitivita na syfilis. Ve spolupráci s dermatovenerologem byla zajištěna léčba, depistáž a hlášení Národní referenční laboratoři. Pitvou zemřelého novorozence patolog konstatoval masivní projevy adnátní pneumonie ve všech plicních lalocích s vaskulitidou a perivaskulárním krvácením, nekrotizující lymfadenitidu uzlin a pyokélu. Závěr: Diagnóza kongenitální syfilis byla stanovena až po úmrtí předčasně narozeného novorozence. Resuscitace již neměla šanci na úspěch vzhledem k rozsahu postižení plic vinou syfilis.
Objective: Description of a case of newborn death after acute caesarean section in 31st week of pregnancy because of mother's syphilitic infection. Results: Fifteen-years-old primigravid woman in 31st week of pregnancy was admitted to a secondary level hospital due to a high risk of preterm labor. The pregnancy was terminated with an acute caesarean section because of pathological cardiotocograph record. The newborn died after 35 min of resuscitation. Blood samples from the umbilical cord and mother's blood taken by her gynecologist were positive for syphilis. In cooperation with a dermatologist, the treatment has been provided and reported to the National Referential Laboratory. The autopsy of the newborn had found severe pneumonia, necrotic lymphadenitis and pyocele. Conclusion: The diagnosis of congenital syphilis had been determined after the death of a preterm delivered newborn. Resuscitation had no chance to success because of syphilitic damage of the lungs.

BACKGROUND: It has still to be ascertained whether severe acute respiratory syndrome coronavirus 2 infection in pregnancy is associated with worse maternal and fetal outcomes compared to low risk gestations. OBJECTIVE: This study aimed to evaluate maternal and perinatal outcomes in high- and low-risk pregnancies complicated by severe acute respiratory syndrome coronavirus 2 infection. STUDY DESIGN: This was a multinational retrospective cohort study involving women with laboratory-confirmed severe acute respiratory syndrome coronavirus 2 infection from 76 centers from 25 countries in Europe, the United States, South America, Asia, and Australia from April 4, 2020, to October 28, 2020. The primary outcome was a composite measure of maternal mortality and morbidity, including admission to the intensive care unit, use of mechanical ventilation, or death. The secondary outcome was a composite measure of adverse perinatal outcome, including miscarriage, fetal loss, neonatal and perinatal death, and admission to the neonatal intensive care unit. All outcomes were assessed in high- and low-risk pregnancies. Pregnancies were considered high risk in case of either preexisting chronic medical conditions in pregnancy or obstetrical disorders occurring in pregnancy. The Fisher exact test and logistic regression analysis were used to analyze the data. RESULTS: A total of 887 singleton pregnancies who tested positive for severe acute respiratory syndrome coronavirus 2 infection using reverse transcription-polymerase chain reaction of nasal and pharyngeal swab specimens were included in the study. The risk of composite adverse maternal outcomes was higher in high-risk pregnancies than in low-risk pregnancies (odds ratio, 1.52; 95% confidence interval, 1.03-2.24; P=.035). In addition, women carrying high-risk pregnancies were at higher risk of hospital admission (odds ratio, 1.48; 95% confidence interval, 1.07-2.04; P=.002), presence of severe respiratory symptoms (odds ratio, 2.13; 95% confidence interval, 0.41-3.21; P=.001), admission to the intensive care unit (odds ratio, 2.63; 95% confidence interval, 1.42-4.88), and invasive mechanical ventilation (odds ratio, 2.65; 95% confidence interval, 1.19-5.94; P=.002). When exploring perinatal outcomes, high-risk pregnancies were at high risk of adverse perinatal outcomes (odds ratio, 1.78; 95% confidence interval, 0.15-2.72; P=.009). However, such association was mainly because of the higher incidence of miscarriage in high-risk pregnancies compared with that in low-risk pregnancies (5.3% vs 1.6%, P=.008); furthermore, there was no difference in other explored outcomes between the 2 study groups. At logistic regression analysis, maternal age (odds ratio, 1.12; 95% confidence interval, 1.02-1.22; P=.023) and high-risk pregnancy (odds ratio, 4.21; 95% confidence interval, 3.90-5.11; P<.001) were independently associated with adverse maternal outcomes. CONCLUSION: High-risk pregnancies complicated by severe acute respiratory syndrome coronavirus 2 infection were at higher risk of adverse maternal outcomes than low-risk pregnancies complicated by severe acute respiratory syndrome coronavirus 2 infection.
