AIM: Our study aimed to evaluate the use of positron emission tomography/computed tomography (PET/CT) in the initial staging of head and neck squamous cell carcinoma (HNSCC), including assessment of local and distant spread of the disease. We also aimed to compare the accuracy of PET/CT in the evaluation of human papillomavirus (HPV) positive and HPV-negative oropharyngeal carcinoma. MATERIAL AND METHODS: This single-center, prospective study was conducted between August 2016 and September 2021. A total of 198 patients with HNSCC who underwent PET/CT within the primary staging were included. We compared PET/CT results with histological findings. We calculated the accuracy, sensitivity, specificity, and positive and negative predictive values to assess the primary tumor, cervical lymph nodes, and distant metastases. RESULTS: PET/CT showed a high success rate (32%) in revealing the primary site of carcinoma of unknown primary (CUP). The accuracy of PET/CT in displaying the primary tumor, cervical lymph node metastases, and distant metastases was 89.4%, 85.4%, and 87.4%, respectively. The method provided high sensitivity but lower specificity in all three areas. Specifically, PET/CT showed low specificity in the assessment of small tumors (75%), metastatic involvement of cervical lymph nodes (69.6%), and HPV-positive oropharyngeal carcinoma (55.6%). CONCLUSIONS: The high accuracy of PET/CT to identify distant metastases and whole-body staging in one diagnostic step accelerated primary staging and resulted in earlier commencement of therapy. However, it also led to an overestimation of clinical findings and thus to extensive surgical treatment, especially in patients with small tumors, metastatic involvement of cervical lymph nodes, and HPV- positive oropharyngeal carcinoma. PET/CT is also useful for CUP diagnostics.

Systémová progresivní skleróza je autoimunitní onemocnění nejasné etiologie charakterizované vaskulopatií a fibrózou. Výskyt maligních nádorů u pacientů s tímto závažným onemocněním je vyšší než u běžné populace. Příčina této predispozice není dosud jasně definována. V naší práci uvádíme případ pacientky se systémovou progresivní sklerózou, u níž byl diagnostikován orofaryngeální karcinom. Cílem práce je upozornit na komplikace v diagnostické rozvaze a terapii u souběžně probíhajícího systémového autoimunitního a nádorového onemocnění.
Systemic progressive sclerosis is an autoimmune disease of unclear etiology characterized by vasculopathy and fibrosis. The incidence of malignancies in patients with this serious disease is higher than in the general population. The cause of this predisposition is not yet clearly defined. In our case we present a patient with systemic progressive sclerosis who was diagnosed with oropharyngeal carcinoma. The aim of this work is to draw attention to the complications in differential diagnosis and therapy in concurrent systemic autoimmune and cancer.

I přes pokrok v terapii nádorů hlavy a krku se celkové pětileté přežití za posledních několik dekád prakticky nezměnilo, a to zejména kvůli pozdní diagnóze. Vzhledem k tomu, že v minulosti byly slizniční nádory v ORL oblasti zachycovány až ve vyšších stadiích nemoci, byly vyvinuty nové endoskopické optické zobrazovací metody pro lepší a dřívější detekci těchto lézí. Dělíme je na dvě základní skupiny. Horizontální metody zobrazují povrch sliznice (úzkopásmové zobrazení – narrow band imaging, autofluorescence, fotodynamická diagnostika, zvětšovací a kontaktní endoskopie). Vertikální metody umožňují zobrazit různé vrstvy sliznice (optická koherentní tomografie a konfokální laserová endomikroskopie). V zahraničí jsou běžně v současné klinické praxi v různých oborech využívány narrow band imaging a autofluorescence. Do ORL působnosti se stále častěji začleňuje i zvětšovací a kontaktní endoskopie. Autoři předkládají souhrnný přehled o využití narrow band imaging a autofluorescenci při diagnostice karcinomu orofaryngu.
Despite progress in the treatment of head and neck cancer, the five-year overall survival rate is still low because of late dia gnosis. The head and neck mucosal tumours were usually diagnosed in higher stages in past, therefore new endoscopic optical imaging methods were developed for better and earlier detection of these lesions. They are divided in two main groups – horizontal and vertical methods. The horizontal ones show the surface of the mucous membrane (narrow band imaging, autofluorescence, photodynamic diagnosis, magnifying and contact endoscopy). The vertical ones show different layers of the mucosa (optical coherence tomography and confocal endomicroscopy). Narrow band imaging and autofluorescence endoscopy are already used routinely in foreign countries, magnifying and contact endoscopy are getting to practice recently. The authors present a summary of narrow band imaging and autofluorescence endoscopy usage in the diagnostics of oropharyngeal carcinoma.

V posledních 10 letech se objevila možnost využít k odstranění vhodně lokalizovaných T1 a T2 spinocelulárních karcinomů orofaryngu, zejména kořene jazyka, tonzily a supraglotis, miniinvazivní robotický přístup skrze ústní dutinu - TORS (TransOralRoboticSurgery). Lokalizací a tvarem jsou pro TORS nejlépe vhodné povrchově uložené a ohraničené spinocelulární karcinomy, dostupné pro robotické nástroje. Rozhodující úlohu při indikaci k TORS hraje zobrazení karcinomů hlavy a krku pomocí magnetické rezonance.
In the last 10 years, it became possible to be used by appropriately localized T1 and T2 squamous cell carcinomas of oropharynx and supraglottis robotic minimally invasive approach trough oral cavity – TORS (TransOralRoboticSurgery). Localization and shape best suited tumors for TORS are surface stored and bounded squamous cell carcinomas available for robotic tools. Decisive role in indication for removal of cancers by the TORS plays magnetic resonance imaging and computer tomography of the head and neck.
