Metastatický karcinom prostaty (mKP) je heterogenní skupinou onemocnění s mediánem přežití 42 měsíců. Základ léčby mKP tvoří vždy kombinace androgen deprivační terapie (ADT – androgen deprivation therapy) s novými antiandrogeny (ARTA – androgen receptor targeted agents), jako jsou enzalutamid, apalutamid, darolutamid, nebo s novým blokátorem syntézy testosteronu abirateron acetátem (AA) či s chemoterapií (docetaxel, kabazitaxel). Pro stanovení prognózy užíváme prognostické faktory, které korelují s délkou přežití bez ohledu na systémovou terapii. V této práci podáváme přehled aktuálně využívaných prognostických faktorů.
Metastatic prostate cancer (mPC) is a heterogeneous group of diseases with a median survival of 42 months. The basic treatment of mPC is always a combined treatment with androgen deprivation therapy (ADT - androgen deprivation therapy) with new antiandrogens (ARTA - androgen receptor targeted agents) such as enzalutamide, apalutamide, darolutamide or with the new testosterone synthesis blocker abiraterone acetate (AA) or with chemotherapy (docetaxel, cabazitaxel). To determine prognosis, we use prognostic factors that correlate with survival independent of systemic therapy. In this article, we provide an overview of the currently used prognostic factors.

Stresová inkontinencia moču (SUI - stress urinary incontinence) u mužov je častou komplikáciou po radikálnej operácii pre karcinóm prostaty. U každého konkrétneho pacienta je potrebné posúdiť individuálne riziko SUI. Liečba post-prostatektomickej SUI (PPSUI) môže byť zložitá. Ak zlyháva konzervatívny manažment, je možné využiť chirurgické možnosti. V súčasnosti je k dispozícii niekoľko chirurgických postupov na liečbu PPSUI. Z nich umelý močový zvierač (AUS - artificial urinary sphincter) vykazoval dlhodobú bezpečnosť a účinnosť u pacientov so stredne závažnou až závažnou SUI. Mužské slingy sú alternatívnym prístupom pre mužov s miernou až stredne závažnou SUI. Ostatné menej populárne možnosti, ako sú injikovateľné objemové látky alebo nastaviteľné balóniky, by sa mali brať do úvahy, iba ak sú overené možnosti kontraindikované. PPSUI je jednou z mála urologických chorôb, ktoré sú iatrogénne, a preto je ich možné predvídať a predchádzať im.
Male stress urinary incontinence (SUI) is common complication after radical surgery for prostate cancer. Patient ́s individual features should be well kept in mind, with the aim of better assessing the individual risk of SUI. Therapy of post-prostatectomy SUI (PPSUI) can be difficult. If conservative management fails, surgical options can be used. Several surgical procedures are currently available to treat PPSUI. Out of these, artificial urinary sphincter (AUS) showed the longest record of safety and efficacy for patients with moderate to severe SUI. Male slings are an alternative approach for men with mild to moderate SUI. Other less popular options, such as injectable bulking agents or adjustable balloons should only be considered when more established procedures are contraindicated. PPSUI is one of the few urologic diseases that is iatrogenic, and therefore predictable and perhaps preventable.

Karcinom prostaty je v současné době třetím nejčastějším onkologickým onemocněním u mužů v ČR a druhým nejčastěji se vyskytujícím nádorem u mužů v celosvětovém měřítku. Jeho incidence každým rokem stoupá. Každým rokem jsou aktualizovány diagnosticko-terapeutické algoritmy léčby karcinomu prostaty, které zohledňují míru lokální progrese tumoru, jeho stupeň histologické diferenciace a míru metastatického rozsevu. V případě karcinomu prostaty bývá nejčastějším místem vzdálených metastáz samotný skelet či skelet v kombinaci s metastatickým postižením ilických případně paraaortálních uzlin. V literatuře bylo zatím popsáno jen velmi malé množství plicních metastáz karcinomu prostaty, které se navíc většinou vyskytovaly synchronně s kostními metastázami. Jejich výskyt navíc koreloval s agresivním histologickým subtypem tumoru (Gleason score 8 a víc) a elevací PSA. Tento případ je raritní skutečností, že byla nalezena solitární plicní metastáza karcinomu prostaty u pacienta s neměřitelnými hodnotami PSA a Gleason score 6 s vyloučením generalizace do jiných míst.
Currently, prostate cancer is the third most common cancer disease in men in the Czech Republic and the second most common tumour found in men worldwide. Its incidence increases annually. Every year, diagnostic and treatment algorithms for prostate cancer are updated which take into account the rate of local tumour progression, its grade of histological differentiation, and the degree of metastatic spread. In the case of prostate cancer, the skeleton alone or the skeleton in combination with metastatic involvement of the iliac and/or para-aortic nodes are the most common sites of distant metastases. So far, only a small number of lung metastases of prostate cancer have been reported in the literature; in addition, they typically presented synchronously with bone metastases. Moreover, their occurrence correlated with an aggressive histological tumour subtype (a Gleason score of 8 and higher) and PSA elevation. This case is rare due to the fact that a solitary lung metastasis of prostate cancer was found in a patient with undetectable PSA levels and a Gleason score of 6 with exclusion of generalization to other sites.

PURPOSE: This paper compares individual radiation therapy techniques used for prostate cancer and their benefits in clinical practice. METHODS: We retrospectively analyzed 921 patients with localized prostate tumors treated between 1997 and 2012. We divided the patients into four groups according to the selected treatment technique (conformal radiation therapy [3DCRT], intensity-modulated radiation therapy [IMRT], image-guided radiation therapy [IGRT], and volumetric-modulated arc therapy [VMAT]) and evaluated the incidence of acute and chronic gastrointestinal (GI) and genitourinary (GU) toxicity. RESULTS: The incidence of grade 2 or greater acute GU and GI toxicity was significantly higher among techniques other than IGRT (p˂0.001). We found the same results in the case of grade 3 or greater acute GU toxicity (p˂0.001). Grade 3 or higher acute GI toxicity occurred only in one patient treated by 3DCRT. Cumulative late GI toxicity of grade 2 or higher and grade 3 or higher was recorded over 3 years significantly more frequently among non-IGRT techniques as compared to IGRT (p˂0.001). As regards GU toxicity, we found significantly higher incidence only for grade 2 or higher (p˂0.001), not for grade 3 or higher. No occurrence of grade 4 toxicity was recorded. The greatest incidence of patients without acute and chronic GI/GU toxicity was recorded in connection with VMAT. CONCLUSION: IGRT demonstrated a pronounced reduction in acute and chronic GU and GI toxicity as compared to non-IGRT techniques in the treatment of localized prostate cancer.

OBJECTIVES: Metabolic syndrome (MetS) and its components (high blood pressure, BMI≥30, altered fasting glucose, low HDL cholesterol and high triglycerides) may undermine early perioperative outcomes after radical prostatectomy (RP). We tested this hypothesis. MATERIALS & METHODS: Within the National Inpatient Sample database (2008-2015) we identified RP patients. The effect of MetS was tested in four separate univariable analyses, as well as in multivariable regression models predicting: 1) overall complications, 2) length of stay, 3) total hospital charges and 4) non-home based discharge. All models were weighted and adjusted for clustering, as well as all available patient and hospital characteristics. RESULTS: Of 91,618 patients: 1) 50.2% had high blood pressure, 2) 8.0% had BMI≥30, 3) 13.0% had altered fasting glucose, 4) 22.8% had high triglycerides and 5) 0.03% had low HDL cholesterol. Respectively, one vs. two vs. three vs. four MetS components were recorded in 36.2% vs. 19.0% vs. 5.5% vs. 0.8% patients. Of all patients, 6.3% exhibited ≥3 components and qualified for MetS diagnosis. The rates of MetS increased over time (EAPC:+9.8%; p < 0.001). All four tested MetS components (high blood pressure, BMI≥30, altered fasting glucose and high triglycerides) achieved independent predictor status in all four examined endpoints. Moreover, a highly statistically significant dose-response was also confirmed for all four tested endpoints. CONCLUSION: MetS and its components consistently and strongly predict early adverse outcomes after RP. Moreover, the strength of the effect was directly proportional to the number of MetS components exhibited by each individual patient, even if formal MetS diagnosis of ≥3 components has not been met.
