Úvod: Infekce cévních rekonstrukcí je vzácná, ale závažná komplikace v cévní chirurgii, spojená s vysokou morbiditou a mortalitou. Včasná diagnostika infekce cévní rekonstrukce je důležitá pro správnou a včasnou chirurgickou a antibiotickou léčbu, která zlepšuje výsledek. Taktika a technika operačního řešení infekcí cévních rekonstrukcí se za poslední dvě dekády změnily a tento trend lze pozorovat i v naší retrospektivní studii. Metody: Retrospektivní analýzou jsme vyhodnotili skupinu nemocných s protetickými cévními rekonstrukcemi založenými na chirurgické klinice FN v Plzni v období 2003–2021. V analyzovaném 19letém období jsme z celkového souboru 2090 založených periferních bypassů řešili 23 infikovaných cévních rekonstrukcí a z celkového souboru 1940 centrálních rekonstrukcí na břišní aortě a v pánevním řečišti řešili 27 infikovaných rekonstrukcí. Výsledky: Incidence infekcí periferních cévních rekonstrukcí v období 2003–2021 na našem pracovišti dosáhla 1,1 % s časnou mortalitou 8,7 % a centrálních cévních rekonstrukcí za stejné časové období 1,4 % s časnou mortalitou 33 %. Závěr: Výsledky naší retrospektivní studie jsou srovnatelné se zkušenostmi jiných pracovišť prakticky ve všech parametrech. Shodně naše pracoviště přešlo na in situ náhrady explantovaných rekonstrukcí a potvrzujeme dobrou zkušenost se stříbrem impregnovanými protézami.
Introduction: Vascular graft infection is a rare but serious complication in vascular surgery, associated with high morbidity and mortality. Early diagnosis of vascular graft infection is important for proper and timely surgical and antibiotic treatment that improves the outcome. The tactic and techniques of surgical treatment of vascular graft infection have changed over the last two decades, and this trend can also be observed in our retrospective study. Methods: We evaluated a group of patients with prosthetic vascular reconstructions performed at the Department of Surgery, University Hospital in Pilsen in the period of 2003–2021 using retrospective analysis. In the analyzed 19-year period, 23 infected vascular grafts were managed out of a total of 2090 performed peripheral bypasses, and 27 infected vascular grafts were managed out of a total set of 1940 central reconstructions. Results: The incidence of peripheral vascular graft infections at our Department of Surgery in the period of 2003–2021 reached 1.1% with the early mortality rate of 8.7%; 1.4% central vascular graft infections occurred in the same time period with 33% early mortality. Conclusion: The results of our retrospective study are comparable virtually in all parameters with the experience of other departments. Consistently, our department switched to in situ replacements for explanted vascular grafts and we can confirm good experience with silver impregnated grafts.
OBJECTIVE: Changes in the N20/P25 amplitude of somatosensory evoked potentials (SEP) of the median nerve have been found to correlate with those in cortical regional cerebral blood flow (rCBF). Our study presents the use of median nerve SEP amplitude in predicting the clinical outcome of urgent surgical internal carotid artery (ICA) recanalization. METHODS: A total of 27 patients suffering an acute ischemic stroke (AIS) with extracranial ICA occlusion within 24 h were prospectively recruited. The primary preoperative endpoints included the SEP amplitude absolute value (SEP-amp) and the SEP amplitude side-to-side ratio (SEP-ratio). Clinical outcome at 3 months postoperatively was assessed using the modified Rankin scale (mRS-3M). RESULTS: The positive predictive values (PPVs) for SEP-amp and SEP-ratio were 95.5% and 100%, respectively, with the negative predictive values (NPVs) being 60.0% and 100%, respectively. The SEP-ratio correlated fully with mRS-3M. CONCLUSION: The median SEP side-to-side N20/P25 amplitude ratio seems to be a very strong positive and negative predictor of the clinical outcome of urgent recanalization of an extracranial ICA occlusion. SIGNIFICANCE: The results suggest that cortical evoked activity may help in selection patient for surgical recanalization and predict clinical recovery after an acute ischemic stroke.

The field of vascular surgery is constantly evolving and is unsurpassed in its innovation and adoption of new technologies. Endovascular therapy has fundamentally changed the treatment paradigms for aneurysm and occlusive disease. As we continue to make advances in not only endovascular therapy, but also robotic surgery, artificial intelligence, and minimally invasive surgery, it is important that the vascular community stay at the forefront. Topics include the advantages of laparoscopic and robotic surgery over open surgery for aortic procedures, robotic versus laparoscopic aortic surgery, patient candidacy for robotic-assisted aortic surgery, and how to increase training and adoption of robotic-assisted laparoscopic aortic surgery. Future growth includes the development of new platforms and technologies, creation and validation of curriculum and virtual simulators, and conduction of randomized clinical trials to determine the best applications of robotics in vascular surgery.
OBJECTIVE: To evaluate the diagnostic performance of 18F-FDG PET/CT in the detection of stent graft infection (SGI). METHODS: In a retrospective study, two nuclear medicine physicians have independently analyzed 17 18F-FDG PET/CT examinations performed for clinical suspicion of SGI. The images were evaluated for the uptake pattern and intensity, and by the maximum standard uptake value (SUVmax), the target-to-background ratio with blood pool (TBRBP) and liver uptake (TBRhep) as a reference. The SGI was defined as the presence of focal hyperactivity with an intensity exceeding hepatic uptake. CT images were independently assessed for signs of SGI. Clinical review of all further patients' data served as the standard of reference. RESULTS: Nine cases were established as SGI by the clinical review. PET/CT correctly diagnosed SGI in eight and yielded a sensitivity of 89% and specificity of 100%. The mean SUVmax, TBRBP, and TBRhep values were 9.8 ± 4.0, 6.9 ± 2.6, and 4.6 ± 1.7 in the group of patients with true SGI, and 4.0 ± 1.1, 2.5 ± 0.4 (p < 0.001) and 1.9 ± 0.2 (p < 0.001) in true negative cases, respectively. CT alone showed a sensitivity of 78% and specificity of 100% and was concordant with PET/CT in 14 cases. The best performing threshold values of SUVmax, TBRBP, and TBRhep were 5.6, 3.5, and 2.2, respectively. CONCLUSION: 18F-FDG PET/CT with expert evaluation, semiquantitative and quantitative image analysis with the proposed threshold values for SUVmax, TBRBP, and TBRhep has good diagnostic accuracy in the detection of SGI. We propose that visual grading scale for SGI should use hepatic uptake as a visual reference.

BACKGROUND: Posterior inferior cerebellar artery (PICA) aneurysms are an uncommon, heterogeneous group of aneurysms with poorer clinical outcomes compared to other intracranial aneurysms. We performed a multicenter retrospective study to analyze the outcome in a large series of patients treated with modern microsurgical and endovascular techniques. METHODS: Records of 94 patients treated for PICA aneurysms between 2000 and 2015 at three large tertiary referral centers were retrospectively reviewed. RESULTS: Eighty-three patients met inclusion criteria and of these, two died before treatment, leaving 81 treated patients (43 underwent endovascular and 38 surgical treatment). Among patients treated endovascularly, procedure-related complications occurred in four cases (11.8%). Six patients (19.4%) suffered from complications directly associated with surgery. Recurrences occurred in 0% of surgical and in 16.3% of endovascularly treated patients, requiring treatment. Patients with unruptured asymptomatic aneurysms had good outcomes. In the group of 67 ruptured aneurysms, 16 endovascularly (47.1%) and 15 surgically (48.4%) treated patients had modified Rankin scale (mRS) scores of 3-6. Of patients in poor neurological condition (Hunt & Hess (H&H) IV-V at admission), 84.6% suffered a poor clinical outcome. Fifty percent of patients with distal and 31.9% patients with proximal ruptured PICA aneurysms suffered a poor neurological outcome. CONCLUSIONS: This study of PICA aneurysms demonstrates that results of both treatment modalities are comparable. However, endovascular treatment is associated with higher risks of recurrence, requiring additional treatment. Outcomes were mostly impacted by clinical state at admission.

Úvod: Cílem práce je zhodnocení našich zkušeností a výsledků s použitím hybridních rekonstrukcí v terapii multietážového postižení tepenného systému dolních končetin a vyhodnocení vlivu indikace a typu hybridní rekonstrukce na jejich průchodnost. Metoda: Retrospektivní analýza souboru pacientů s prospektivním sběrem dat. Na základě typu hybridní rekonstrukce rozdělení pacientů do 5 skupin. Skupina pacientů „1“ podstoupila transluminální angioplastiku (TA) ? implantaci stentu do pánevního řečiště v kombinaci s plastikou tepen v třísle (35 pacientů; 27,6 %). Skupina pacientů „2“ podstoupila TA ? implantaci stentu do pánevního řečiště v kombinaci s infrainguinálním bypassem (15 pacientů; 11,8 %). Skupina pacientů „3“ podstoupila plastiku tepen v třísle v kombinaci s TA výtokového traktu končetiny: AFS ? AP ? bércové tepny (52 pacientů; 40,9 %). Skupina pacientů „4“ podstoupila provedení infrainguinálního bypassu v kombinaci s TA distálně od místa distální anastomózy rekonstrukce (3 pacienti; 2,4 %). Skupina pacientů „5“ představuje heterogenní skupinu hybridních rekonstrukcí, které nespadaly do některé ze 4 výše uvedených skupin (22 pacientů; 17,3 %). Na základě indikace k výkonu rozdělení souboru do 3 skupin: skupina „ALI“ pacienti s akutní končetinovou ischemií, skupina „CLI“ pacienti s kritickou končetinovou ischemií, skupina „II“ pacienti s klaudikačními obtížemi. Analýza průchodností provedena s použitím Kaplan-Meier life tables. Porovnání průchodnosti mezi skupinami provedeno pomocí log-rank testu. Statistická analýza provedena s použitím softwaru IBM SPSS Statistics 21.0 . Výsledky: Soubor prezentuje 127 pacientů, kteří podstoupili 127 hybridních intervencí. Technický a klinický úspěch hybridní rekonstrukce byl 96,9 % a 98,4 %. 30denní perioperační mortalita byla 3,1 %. Výsledky primární (PP), primární asistované (PAP) a sekundární (SP) průchodnosti ve 2 letech byly nejlepší u pacientů s hybridním výkonem typu „1“ a „3“, tedy kombinací plastiky tepen v třísle a transluminální angioplastiky vtokového nebo výtokového traktu končetiny. Výsledky skupiny „1“ v pořadí PP, PAP a SP ve 2 letech: 80,6 %, 84,3 % a 84,3 %. Výsledky skupiny „3“ ve 2 letech ve stejném pořadí: 81,1 %, 82,8 % a 86,3 %. O poznání horších výsledků bylo dosaženo u pacientů ze skupiny „2“ a „5“. Výsledky PP, PAP a SP ve 2 letech u pacientů ze skupiny „2“ byly: 39,4 %, 59,1 % a 59,1 %. Výsledky průchodnosti pacientů z heterogenní skupiny „5“ byly ve 2 letech: 30,4 %, 49,2 % a 70,7 %. Při zohlednění vlivu indikace na průchodnost rekonstrukce ve 2 letech byly nejlepší výsledky primární asistované a sekundární průchodnosti dosaženy u pacientů ze skupiny „CLI“: 81,6 % a 86,2 %. Jenom primární průchodnost ve 2 letech byla nejlepší u pacientů s klaudikacemi (skupina „II“), a to 71,1 %. Pacienti indikováni k hybridnímu výkonu pro akutní končetinovou ischemií (ALI) dosahovali nejhorších výsledků z uvedených 3 skupin, a to PP, PAP a SP ve 2 letech: 33,7 %, 46,2 % a 45,5 %. Amputation-free survival ve 2 letech měl nejlepší hodnotu pro pacienty ze skupiny „CLI“, a to 89,1 % („II“ 81,9 %; „ALI“ 61,4 %). Overall-survival a amputation-free survival ve 2 letech za celý soubor dosahovaly hodnot 93,2 % a 82,3 %. Závěr: Hybridní revaskularizace je kombinovaný výkon, který umožňuje řešení multietážového postižení tepenného systému dolních končetin v rámci jednoho sezení. Je zatížen nízkým rizikem periprocedurální mortality a morbidity pro pacienta. Lze jím dosáhnout dobrých výsledků průchodnosti tepenného systému a záchrany končetiny bez ohledu na typ chronické ischemie (klaudikace, kritická končetinová ischemie). Výsledky u pacientů s akutní končetinovou ischemií jsou inferiorní. Jako výkon s nejlepšími výsledky nám vychází kombinace plastiky tepen v třísle spojená s transluminální angioplastikou vtokového nebo výtokového traktu končetiny.
Introduction: The goal of this study was to evaluate our experience and results of single staged hybrid procedures for revascularization of lower extremities affected by multilevel arterial occlusive disease. We analysed the effect of the indication and type of reconstruction on patency. Method: Patients were retrospectively reviewed. Data collection was conducted prospectively. Patients were divided into 5 groups based on the type of hybrid reconstruction. Group “1” included patients who underwent transluminal angioplasty (TA) ? stenting of iliac arteries with endarterectomy (EA) and patch arterioplasty of the femoral bifurcation (35 patients; 27.6 %). Group “2” included patients who underwent TA ? stenting of iliac arteries with infrainguinal bypass (15 patients; 11.8 %). Group “3” consisted of patients who underwent TA ? stenting of outflow lower extremity arteries: superficial femoral artery ? popliteal artery ? crural arteries in combination with EA and patch arterioplasty of the femoral bifurcation (52 patients; 40.9 %). Group “4” represented patients who underwent infrainguinal bypass surgery in combination with TA distal to the site of open reconstruction (3 patients; 2.4%). Group “5” represented a heterogenous population of patients who underwent a hybrid reconstruction which did not belong to any of the 4 previously mentioned groups (22 patients; 17.3 %). The patients were divided into 3 groups based on the indication criteria of the intervention: Patients with acute limb ischemia (ALI) were put into group “ALI”. Patients with critical limb ischemia (CLI) were put into group “CLI”. Patients with claudications were put into group “II”. Patency analyses were performed using Kaplan-Meier life tables. Differences in patency rates between the different groups of patients were determined using the log-rank test. Statistical analysis was performed using the software IBM SPSS Statistics 21.0. Results: The study included 127 patients who underwent 127 hybrid arterial procedures. Technical and clinical success rates were 96.9% and 98.4%. 30-day perioperative mortality rate was 3.1%. The primary (PP), assisted-primary (APP) and secondary patency (SP) results at 2 years were the best amongst the patients from groups “1” and “3”. These patients underwent patch arterioplasty ? EA of the femoral bifurcation in combination with transluminal angioplasty of inflow or outflow arteries of the lower extremity. The resulting PP, APP and SP rates at 2 years in group “1” were 80.6%, 84.3% and 84.3%. Patency rates in group “3“ in the same order at 2 years were 81.1 %, 82.8% and 86.3%. Significantly lower patency rates were achieved in patients from groups “2“ and “5“. The resulting PP, APP and SP rates in group “2” at 2 years were 39.4%, 59.1% and 59.1%; in group “5“ at 2 years they were 30.4%, 49.2% and 70.7%. Taking into account the effect of the indication on patency rates, we found that patients from group “CLI” had the best APP and SP rates at 2 years: 81.6% and 86.2%. Only the PP rate at 2 years was the best in group “II” (patients with claudications): 71.1%. Patients who underwent their operations because of acute limb ischemia (group “ALI”) achieved the worst results among these 3 groups. Their PP, APP and SP rates at 2 years were 33.7%, 46.2% and 45.5%. Patients from the group „CLI“ had the best amputation-free survival at 2 years: 89.1% amongst the 3 groups subdivided according to the indication for the intervention (“II“ 81.9%; “ALI“ 61.4%). Overall survival and amputation-free survival at 2 years were 93.2% and 82.3%, respectively, for the whole studied population. Conclusion: The hybrid procedure is an intervention which combines endovascular and open reconstruction performed at the same time. Hybrid procedures provide effective therapy for patients with multilevel lower extremity arterial disease. They have low complication rates, periprocedural morbidity and mortality. Using these procedures, we are able to achieve good limb salvage rates and patency rates irrespective of the form of chronic limb ischemia (claudications, critical limb ischemia). Results for patients with acute limb ischemia are inferior. The combination of patch arterioplasty ? endarterectomy of the femoral bifurcation with transluminal angioplasty of inflow or outflow limb arteries is our hybrid procedure of choice with the most favourable results.

AIM: The aim of the paper was to determine the causes of varicose vein recurrence and, when neovascularisation is suspected, to confirm or exclude its presence and to establish its contribution to the recurrence of varicose veins. METHODS: A retrospective analysis of a set of 217 legs reoperated in our department over a period of 17 years with a two-year prospective histological and histochemical (nestin) analysis of resected veins. RESULTS: Reflux as a cause of varicose vein recurrence was identified in 93% of the limbs. It was most commonly found in the area of the saphenofemoral junction, followed by the area of the saphenopopliteal junction. Reflux in the perforating veins was almost invariably linked to that in the saphenofemoral junction or saphenopopliteal junction; an isolated damage to the perforating veins was only present in three limbs. Histological and histochemical analysis was performed for the samples of eleven veins in which neovascularisation was suspected based on preoperative duplex ultrasonography evaluation. Neovascularisation was confirmed in none of these veins and none of them was the cause of recurrence. CONCLUSION: The major cause of varicose vein recurrence is a reflux left unresolved during the primary surgery, particularly in the area of the saphenofemoral junction. A less common cause is progression of the disease and the occurrence of a new reflux. Neovascularisation is of no importance for the recurrence of varicose veins.

More than 50% of all end-stage renal disease (ESRD) patients die from cardiovascular complications. Among them, heart failure and pulmonary hypertension play a major role, and published studies document significantly higher mortality rates in patients with these two states. Arteriovenous fistulas (AVF) and arteriovenous grafts (AVG) are the preferred types of vascular access (VA). However, both AVF and AVG increase cardiac output and in turn could contribute to (the decompensation of) heart failure or pulmonary hypertension. No really safe access flow volume exists, and the ESRD patients' reactions to it vary considerably. We review the mechanisms involved in the cardiovascular consequences of increased cardiac output and available literary data. The link between access flow volume and increased mortality due to pulmonary hypertension or heart failure probably exists, but still has not been directly evidenced. Regular echocardiography is advisable especially in patients with symptoms or with high VA flow (>1,500 ml/min).

The 25th World Congress of the International Union of Angiology was held in Prague from July 1st to 5th, 2010.There were a total of 586 reports (of these, there were 430 oral presentations and 255 poster presentations, highlighting many problems in arterial pathology: optimal methods of visualization and treatment of carotid arterial atherosclerosis,endovascular recanalization of intracranial arteries and hybrid interventions on carotid arteries in acute period of ischaemic stroke, management of patients with asymptomatic atherosclerosis of carotid arteries, main problems in treatment of coronary artery atherosclerosis, complications of endovascular techniques, management of patients with aneurysms of the thoracic and abdominal portions of the aorta, as well as atherosclerosis of lower limb arteries. Special attention was paid to treatment of lower limb critical ischaemia and diabetic foot syndrome, problems concerning venous thromboembolism and methods of anticoagulant therapy, as well as genetic predictors of the development of venous diseases. Other problems discussed at the Congress were as follows: methods of endovascular treatment of chronic venous insufficiency, prevention and microsurgical interventions in treatment of lymphedema, the role of endovascular techniques in correction of arteriovenous malformations, and robot-assisted surgical interventions.

Cieľ: Autori v práci analyzujú pacientov v geriatrickom veku s komplikáciami po chirurgickom infrainguinálnom revaskularizačnom výkone. Hodnotia skúsenosti pracoviska s liečbou týchto komplikácií. Metóda: Retrospektívna analýza pacientov hospitalizovaných v rokoch 2003–2008 na Oddelení cievnej chirurgie FNsP J. A. Reimana s komplikáciou po chirurgickom infrainguinálnom revaskularizačnom výkone. V sledovanom období sme vykonali 251 infrainguinálnych revaskularizácií u 209 pacientov s vekom nad 70 rokov. Pooperačné komplikácie u týchto pacientov rozdelili na celkové, lokálne cievne a lokálne necievne. Výsledky: Z celkových komplikácií zaznamenali kardiálne komplikácie u 21 pacientov (8,37 %), respiračné komplikácie u 8 pacientov (3,19 %), neurologické komplikácie u 2 pacientov (0,80 %), hemokoagulačné komplikácie u 21 pacientov (8,37 %) a akútne renálne zlyhanie u 2 pacientov (0,80 %). Z lokálnych cievnych komplikácií v sledovanom období sa vyskytla trombóza rekonštrukcie v 33 prípadoch (13,15 %), infekcia rekonštrukcie v 7 prípadoch (2,79 %), pseudoaneuryzma v anastomóze v 1 prípade (0,40 %), krvácanie v 12 prípadoch (4,78 %), zlyhávajúca rekonštrukcia v 6 prípadoch (2,39 %) a hĺbková žilová trombóza v 1 prípade (0,40 %). Z lokálnych necievnych komplikácií infrainguinálnych revaskularizačných výkonov sa vyskytli lymfokély a lymfatické fistuly v 4 prípadoch (1,59 %), pooperačný edém končatiny v 22 prípadoch (8,76 %), kompartment syndróm v 3 prípadoch (1,20 %) a ischemická neuropatia v 2 prípadoch (0,80 %). Záver: Revaskularizačné výkony u geriatrických pacientov s akútnymi aj chronickými uzávermi tepien dolných končatín sú plne indikované. Infrainguinálne revaskularizácie majú uspokojivú dlhodobú priechodnosť, záchranu končatiny a funkčné výsledky. Vysoký vek pacienta nie je kontraindikáciou revaskularizačného výkonu.
Aim: The authors analyse complications of surgical lower extremity arterial reconstructions in elderly patients. They evaluate experiences and treatment results in patients with this complications. Methods: It is a retrospective analysis of the patients hospitalized during years 2003–2008 at the Department of Vascular Surgery of the Teaching Hospital J. A. Reimana with complication after surgical lower extremity arterial reconstruction. During study period they executed 251 lower extremity arterial reconstruction in 209 patients 70 years of age or older. They divide postoperative complications into general, local vascular and local non-vascular. Results: From general complications cardiac complications were recorded in 21 patients (8.37%), respiratory complications in 8 patients (3.19%), neurologic complications in 2 patients (0.80%), hematological complications in 21 patients (8.37%) and acute renal insufficiency in 2 patients (0.80%). During study period from local vascular complications 33 cases of reconstruction trombosis occured (13.15%), reconstruction infection occured in 7 cases (2.79%), anastomotic pseudoaneurysm in 1 case (0.40%), hemorrhage in 12 cases (4.78%), failing reconstruction in 6 cases (2.39%) and deep vein trombosis in 1 case (0.40%). From local non-vascular complications of lower extremity arterial reconstructions lymphoceles and lymphatic fistulas occured in 4 cases (1.59%), postoperative extremity edema in 22 cases (8.76%), compartment syndrome in 3 cases (1.20%) and ischemic neuropathy in 2 cases (0.80%). Conclusion: Revascularization procedures in elderly patients with acute and chronic lower extremity ischemia are absolutely indicated. Infrainguinal reconstructions have good long term patency rate, limb salvage and functional outcomes. Old age patients aren’t contraindicated for lower revascularization procedures.
