Krátký přehled zvládání stabilizované CHOPN vyplývající z dokumentu Světové strategie diagnostiky, prevence a léčby CHOPN, která byla publikována v listopadu 2006. Popsána současná doporučení zvládání CHOPN ve srovnání s poslední Světovou strategií CHOPN.
A short review of management of stable COPD based on the Global Strategy for the Diagnosis, Management, and Prevention of COPD document published in November 2006. The current recommendations for COPD management are compared with the latest global strategy document.
Exacerbace CHOPN je definována jako příhoda v průběhu nemoci, která je charakterizována změnou obvyklé dušnosti, kašlem a/nebo vykašláváním nad obvyklé každodenní kolísání těchto potíží, která začíná náhle a může zdůvodňovat změnu v zavedené léčbě u nemocného s CHOPN. Nejčastější příčiny exacerbace jsou infekce tracheobronchiálního stromu. Znečištění ovzduší je příčinou asi deseti procent exacerbací. Příčinu asi jedné třetiny těžkých exacerbací nelze zjistit. Při exacerbaci dochází k bronchospazmu, k edému sliznice bronchů a ke stagnaci hlenu. Vzhledem k tomu, že stěna bronchů je již většinou primárně zesílena, dochází k výraznému zvýšení rezistence. Vznikají oblasti s nízkým poměrem ventilace-perfuze. Značně se zvyšuje hyperinflace i iPEEP (elastické napětí, které je přítomno na úrovni funkční reziduální kapacity- FRC). Hlavním příznakem exacerbace je pocit ztíženého dýchání, který je často doprovázen sípáním a pocitem tíhy na hrudníku, zvýšením kašle a tvorby sputa, změnou barvy nebo viskozity sputa a teplotou. Exacerbace se mohou také projevovat množstvím necharakteristických obtíží, jako je nevolnost, nespavost nebo naopak spavost, únavnost, deprese a zmatenost. Léčba exacerbace, jak ambulantní, tak nemocniční, je symptomatická. Příznaky obstrukce léčíme podáním bronchodilatancií a kortikosteroidů, hypoxémii podáním kyslíku, známky bakteriální infekce antibiotiky.
Lxacerbation of COPD is defined as an event in the course of the disease which is characterized by changes of usual dyspnoea, cough and/or expectoration exceeding the normal daily fluctuations. Its onset is sudden and it may justify alterations in established treatment of COPD patients. Most commonly, exacerbations are due to infections of the tracheobronchial tree. Approximately 10 percent of exacerbations are caused by air pollution. In about one third of severe exacerbations, the cause cannot be determined. Exacerbations give rise to bronchospasm, oedema of the bronchial mucosa and stagnation of mucus. As a result of the usual primary thickening of the bronchial wall, resistance increases significantly. Areas with a low ventilation/perfusion ratio develop. Both hyperinflation and intrinsic positive end-expiratory pressure (iPEEP) increase. The main symptom of exacerbation is breathlessness, often accompanied by wheezing and chest pressure, increased cough and sputum production, changes in sputum colour or viscosity, and a temperature. Exacerbations may also be manifested by numerous uncharacteristic difficulties such as nausea, sleeplessness or drowsiness, fatigue, depression or confusion. Both outpatient and inpatient treatment of exacerbation is symptomatic. The symptoms of obstruction are treated with bronchodflators and corticosteroid, hypoxemia with oxygen therapy and signs of bacterial infection with antibiotics.
Prezentujeme kazuistiku pacienta po ezofagoileokoloplastike, u ktorého RTG nález hrudníka budil podozrenie z prítomnosti pleurálneho výpotku. Z tohoto dôvodu bol opakovane hospitalizovaný na našom oddelení za účelom diferenciálnej diagnostiky.
The author describes the case of a patient who has undergone oesophagoileocoloplasty and whose post-operalive X-ray of the thorax presented suspicious signs of pleural effusion. As a result the patient was repeatedly admitted to our department for differential diagnosis.

Tvorba hlenového sekretu a jeho odstraňování z dýchacích cest je velmi důležitou součástí nespecifického obranného systému respiračního traktu. Hlenovou retenci obvykle zapříčiňuje kombinace zvýšené hlenové tvorby (tzv.hlenová hypersekrece) a poškozené mukociliární clearance (MC). Během posledních 30-40 let došlo k vytvoření značného počtu (významově se částečně překrývajících) termínů popisujících léky působící na odstraňování hlenového sekretu. V tomto stručném sdělení bychom vás rádi seznámili s jedním ze současných klasifikačních schémat léků ovlivňujících hlen. Všechna farmaka působící na hlen můžeme nazývat mukoaktivní medikací. Mukoaktivní léky pak můžeme rozdělovat na expektorancia, modifikátory iontového membránového transportu, mukolytika, mukokinetika a mukoregulátory. Lepší pochopení mechanismu jejich účinku by nás mohlo vést k cílenější a efektivnější léčbě našich nemocných, event. k rozvoji dalších léčiv příznivě ovlivňujících mukociliární clearance.
The production and clearance of respiratory mucus is a critical defence mechanism of the upper and lower respiratory tracts. Mucus retention is usually due to a combination of hypersecretion and impaired mucociliary clearance. Over the years, a diverse terminology has been used to describe „mucoactive medication". Some of these terms include expectorants, mucolytics, mucus mobilizers, mucus regulators, mucus modifiers or mucociliary enhancers. We present recent classification system of mucoactive medication. Mucoactive drug is the general term for medication used to increase secretion removal or reduce mucus hypersecretion. Expectorants enhance the volume or hydratation of respiratory secretions. Ion transport modifiers induce ion and water transmembrane transport. Mucolytics degrade polymers in respiratory mucus. Mucokinetics increase cough efficiency and mucoregulatory drugs reduce the volume of airway mucus. This classification should help us to better develop and evaluate new types of treatment and to direct therapy toward patients.

Autoři předkládají „doporučený ošetřovatelský postup“ dekontaminace, čištění a vyššího stupně dezinfekce bronchoskopu, který schválila Bronchologická komise ČPFS a byl prodiskutován s pracovní skupinou nemocničních hygieniků ČR. Konkrétní postupy jsou uvedeny v souladu s platnou legislativou a upřesňují jednotlivé dílčí kroky při ručním mytí i dezinfekci a při automatickém mytí a dezinfekci, za které je zodpovědná sestra endoskopického pracoviště. V komentáři je doporučeno vybavování pneumologických pracovišť moderními technologiemi odpovídajícími úrovni současné medicíny. Předložený ošetřovatelský postup je závazný, může být ale modifikován po domluvě s ústavním hygienikem příslušného zdravotnického zařízení.
The authors submit a ,,recommended method to be used by nurses" for the decontamination, cleaning and higher degree of disinfection of bronchoscopes, approved by the Bronchological Commission of CPFS. The method was also discussed with the working team of hospital health officers in the Czech Republic. Specific steps of the method were adjusted in line with existing legislation. The proposal specifies individual partial steps during manual washing and disinfection and automatic washing and disinfection, which are the resposibility of the head nurse at an endoscopic department. In their comments the authors recommend that pneumological departments be provided with up-to-date technologies in line with the standards of contemporary medicine. The submitted method to be used by nurses is mandatory - modifications are, however, possible if and when approved by the health officer of the hospital or clinic concerned.

Racionálna farmakoterapia respiračných ochorení predstavuje dôležitú súčasť skvalitnenia starostlivosti o pacientov trpiacich najmä ich chronickou formou. Väčšina liečiv, ktoré sa používajú v liečbe najmä zápalových ochorení, pôsobí prostredníctvom rôznych receptorových a enzýmových systémov. Ich ovplyvnenie má za následok zmiernenie, resp. odstránenie prejavov ochorenia, čím sa zlepší kvalita života týchto pacientov. Autori článku charakterizujú mechanizmy účinkov najdôležitejších skupín liečiv (bronchodilatanciá, protizápalové látky) a uvádzajú perspektívne možnosti a novšie alternatívy terapie ochorení dýchacieho systému.
Rational pharmacotherapy of respiratory diseases represents an important factor in improving the care of patients suffering especially from their chronic forms. Most drugs used in the therapy of mainly inflammatory diseases act via various receptor and enzyme systems. If these are affected, disease manifestations decrease or disappear and the patients' quality of life improves. In the text, the authors characterize mechanisms of action of the most important groups of drugs (bronchodilators, antiinflammatory agents) and present perspective options and new alternatives for treating respiratory diseases.
Hepatopulmonální syndrom je kombinace chronické hepatopatie a respirační insuficience I. typu. Hypoxemie je způsobena abnormální dilatací intrapulmonálních cév nedostatečnou inhibicí vazodilatačních mediátorů při omezeném metabolismu jaterního parenchymu. Portopulmonální hypertenze je méně obvyklou asociací portální a plicní arteriální hypertenze. Příčinou je dysfunkce endotelu v plicním řečišti s nadměrnou tvorbou vazokonstrikčních mediátorů. Pro obě onemocnění je společná alterace plicní vaskulatury s rozdílnou klinickou symptomatologií.
Hepatopulmonary syndrome is a combination of chronic hepatopathy and type I respiratory insufficiency. Hypoxemia is caused by abnormal dilation of intrapulmonary vessels by insufficient inhibition of vasodilatory mediators in limited metabolism of the hepatic parenchyma. Portopulmonary hypertension is less common association of portal and pulmonary arterial hypertension caused by dysfunctional endothelium in the pulmonary bed with excessive production of vasoconstrictive mediators. Both diseases are characterized by common changes in the pulmonary vasculature with various symptomatology.
ÚVOD: Príčina bronchiálnej hyperreaktivity (BHR) je v poslednom čase predmetom intenzívneho výskumu. S BHR sa dáva do spojitosti aj gastroezofágový reflux (GER). CIELE: Zistiť prevalenciu BHR u detí s GER a hodnotiť možnú koreláciu medzi stupňom závažnosti GER a BHR. METÓDY: 34 detí s GER a bez klinických prejavov bronchiálnej astmy (vek 6-18 rokov; priemerný vek 12,4 rokov) sa podrobilo funkčnému vyšetreniu pľúc a bronchiálnej provokácii histamínom (bronchoprovokačný test). Na potvrdenie bronchiálnej hyperreaktivity bola použitá náležitá koncentrácia histamínu spôsobujúca pokles FEV1 o 20 % (PC20). VÝSLEDKY: U 19 detí (55 %) vo vyšetrovanej skupine sme zistili, že potrebná provokačná koncentrácia PC20 histamínu je < 5mg/ml. To u nich potvrdzuje ľahkú alebo strednú BHR. Nebola zistená korelácia medzi stupňom závažnosti GER a BHR. ZÁVER: Deti s GER majú zvýšenú bronchiálnu reaktivitu na inhaláciu histamínu v porovnaní so zdravými deťmi.
Background: The aetiology of bronchial hyperresponsiveness (BHR) has been widely studied in the last years. Gastrooesophageal reflux disease (GORD) may contribute to BHR. Aims: To assess the prevalence of BHR in children with GORD and potential correlation between the severity of GORD and BHR. Method: 34 children with GORD and no clinical evidence of asthma (age: 6-18 years; mean age: 12.4 years) underwent pulmonary function and histamine bronchial provocation tests. Histamine concentration causing a 20 % decrease of FEVl (PC20) was used to prove bronchial hyperresponsiveness. Results: In 19 children (55 %) from the studied group, the provocation concentration of histamine (PC20) was lower than 5 mg/ml, confirming their mild or moderate BHR. BHR was more prevalent in boys. No correlation was found between the severity of GORD and BHR. Conclusions: Children with GORD have increased bronchial responsiveness to inhaled histamine when compared to normal subjects.
