In our expericne the minimally invasive techniques have the very similar results as compared with clasical technique. But the incidence of postoperative pain are very limited / in classical approach 60% incidence of pillar pain/ We shall evaluate the pressure in carpal tunnel during surgery. We will measure pressure in different levels of hand a foream and in different position of hadn and fingers before and after carpal ligament dissection. We will able to change our approach when the pressure descendus after release will be low. We shall correlate results of pressure measurement with electrophysiological findings and clinical results. We assume, that with establishment of pressure measurement we further improve the function of the hand and in positive way we influence the working ability.
Na základě našich zkušeností mají minimálně invazivní techniky chirurgické léčby sy karpálního tunelu obdobné výsledky při srovnání s technikou klasickou. Významně snižují vznik pooperačních bolestí, které se vyskytují asi u 60% případů řešených klasickyVe studii budeme zjišťovat hodnoty tlaku v karpálním tunelu v průběhu operace. Zjišťovat budeme hodnoty v různých úrovních ruky a předloktí a v různých polohách ruky a prstů před a po protětí karpálního ligamenta. Budeme moci prohloubit disekci při nedostatečném poklesu tlaku po transcici vazu. Srovnáme 2 skupiny nemocných - s měřením a bez měření tlaku. Provedeme korelaci elekrofyziologických testů s hodnotami zjištěných tlaků, dále s výsledky operační léčby Předpokládáme, že zavedením tohoto měření zlepšíme výsledky léčby syndromu karpálního tunelu. Urychlíme tím návrat plné funkce ruky a tím pozitivně ovlivníme délku pracovní neschopnosti.
