Multicentrická epidemiologická studie Identifikace původců časných a pozdních nozokomiálních pneumonií u pacientů v intenzivní péči prostřednictvím genetické analýzy bakteriální DNA a určení jejich šíření

Předmětem řešení tohoto multicentrického projektu je identifikace a šíření etiologických agens nozokomiálních pneumonií u kriticky nemocných pacientů. Projekt také sleduje zastoupení multirezistentních kmenů a vliv iniciální ATB léčby na mortalitu a morbiditu pacientů s ohledem na adekvátnost a včasnost jejího nasazení. Metodika řešení spočívá v izolaci a identifikaci patogenů ze vzorků endosekretu biochemickými identifikačními testy a automatizovanými systémy. Citlivost na ATB je testována standardní diluční mikrometodou dle kritérií EUCAST. Produkce širokospektrých beta-laktamáz typu ESBL a AmpC je detekována modifikovanými testy. Pozitivní fenotypové výsledky jsou ověřeny polymerázovou řetězovou reakcí, detekující nejčastěji se vyskytující geny. U vybraných enterobakterií je identita klinických izolátů analyzována srovnáním restrikčních fragmentů genomové DNA separovaných pomocí pulzní gelové elektroforézy.; The subject of this multicentric project is the identification and dissemination of nosocomial pneumonia etiologic agents. Monitoring the incidence of multidrug-resistant strains and the effect of initial antibiotic therapy on mortality in patients with regard to the adequacy and timeliness of deployment. The methodology of the solution lies in isolation and identification of pathogens from microbiological samples with biochemical identification tests and automated systems. Sensitivity to antibiotics will be tested according to standard dilution micromethod EUCAST criteria. Positive phenotypic results will be verified by polymerase chain reaction detecting the most frequently occurring genes. The identity of selected Enterobacteriaceae clinical isolates will be analyzed by comparing restriction fragment of genomic DNA separated by pulsed field gel electrophoresis.
