This document summarizes current WHO guidance for public health surveillance of coronavirus disease 2019 (COVID-19) in humans caused by infection with severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). The global goals to end the COVID-19 emergency are to reduce SARS-CoV-2 transmission and the impact of COVID-19 disease. Surveillance remains fundamental to understanding the evolution of the virus, the risk factors for severe disease and the impact of vaccination and public health and social measures. WHO continues to recommend maintaining and strengthening surveillance to achieve the core surveillance objectives for COVID-19 such as early warning for changes in epidemiological patterns, monitoring trends in morbidity and mortality, monitoring burden of disease on health care capacity and incorporating strategic and geographically representative genomic surveillance.

Routine diagnostic surveillance for COVID-19 has been augmented with environmental surveillance (ES) in wastewater in many locations. ES provides additional evidence on virus in circulation at population level including presence or absence, early warning of increasing or decreasing trends, and information on variants of concern or interest. This guidance is targeted at public health officials who want to understand and integrate complementary ES into COVID-19 control strategies.

More than two years since the first SARS-CoV-2 infections were reported, the COVID-19 pandemic remains an acute global emergency. In this Strategic Preparedness, Readiness and Response plan for 2022, WHO sets out a number of key strategic adjustments that, if implemented rapidly and consistently at national, regional, and global levels, will enable the world to end the acute phase of the pandemic.

All countries should increase their level of preparedness, alert and response to identify, manage and care for new cases of COVID-19. Representative, quality, timely and continuous genetic surveillance of SARS-CoV-2 is critical to the COVID-19 outbreak response. This document provides practical guidance to Global Influenza Surveillance and Response System (GISRS) laboratories and other relevant national laboratories to move beyond virus detection to genomic sequencing of SARS-CoV-2 PCR positive materials obtained from sentinel surveillance of influenza-like illness (ILI), acute respiratory infection (ARI) and severe acute respiratory infection (SARI). It contains considerations on sample selection for sequencing, numbers of viruses to be sequenced, metadata and timeliness for sharing genetic sequence data (GSD) and opportunities for technical support.

The falling cost and complexity of generating GSD provides opportunities for expanding sequencing capacity; however, challenges to widespread implementation remain. This document provides policy-makers and stakeholders with guidance on how to maximize the public health benefit of SARS-CoV-2 genomic sequencing activities in the short and long term as the pandemic continues to unfold. Practical considerations for the implementation of a virus genomic sequencing programme and an overview of the public health objectives of genomic sequencing are covered. This guidance focuses on SARS-CoV-2 but is applicable to other pathogens of public health concern.
