Cohort Profile: The European Longitudinal Study of Pregnancy and Childhood (ELSPAC) in the Czech Republic
BACKGROUND: Temporal trends of chemicals in the population are key to identifying changing sources of chemicals and determining the effectiveness of various legislative measures. OBJECTIVE: The present study focused on time comparisons to explore a possible decrease in PAH metabolite levels in the Czech population. Legislative measures occurred between sampling periods, including restricting smoking and the Air Protection Act. METHODS: Ten metabolites of PAHs were measured in urine samples collected in 2011-2012 from mothers and children from DEMOCOPHES-CZ study (N = 235) and in 2019-2020 from children, teenagers, and young adults from CELSPAC studies (N = 809). Multivariate linear regression, Kruskal-Wallis ANOVA, and Mann-Whitney test (MW) were used to investigate differences in OH-PAHs between periods, age categories, and exposure determinants. RESULTS: Median concentrations significantly decreased between 2011-2020 by 30-35% for 1-OH-NAP, 2-and 3-OH-FLUO, 85% for 1-OH-PHE, and 44% for 2/3-OH-PHE, while 2-OH-NAP increased by 29% in non-smoking adults. In children, median concentrations of all metabolites decreased by 10-51%, with 2-OH-NAP rising by 49%. Smokers showed the largest differences, with significant decreases of 46-59% in the median concentrations of 2-OH-NAP, 2/3-OH-PHE, 9-OH-PHE, and 1-OH-PYR, and 76-91% in OH-FLUOs, 1-OH-NAP, and 1-OH-PHE. Fish and offal consumption, season, locality, and type of cooking were significant factors associated with levels of OH-PAHs, explaining 4-9% of the variability. Smoking was the main contributor in 2011, explaining up to 45% variability; no difference was found between smokers and non-smokers in 2019. New reference values of OH-PAHs in urine were calculated for the Czech population. IMPACT: This study analyses the temporal trends of OH-PAHs in the population in the context of introduced legislative measures. In addition, it examines OH-PAH exposure in children, adolescents, and young adults in relation to lifestyle factors and establishes new reference values for polycyclic aromatic hydrocarbons that are important for public health decision-making. BACKGROUND: The joint impact of exposure to multiple urban environmental factors on asthma remains unclear. METHODS: We analysed data from 14 European cohorts to assess the impact of the urban exposome on asthma incidence across the life course. We linked three external exposome domains (air pollution, built environment, ambient temperature) to the participants' home addresses at baseline. We performed k-means clustering within each domain and assessed associations of clusters with asthma adjusting for potentially relevant covariates in cohort-specific analyses, with subsequent separate meta-analyses for birth and adult cohorts. An environmental risk score using a coefficient-weighted sum approach was used to assess the impact of combining the three domains. FINDINGS: A total of 7428 incident asthma cases were identified among 349,037 participants (from birth up to age 70+). Overall, we observed higher risks of asthma for clusters characterized by high particulate matter and nitrogen dioxide exposure in adults (ORmeta = 1.13, 95%CI:1.01-1.25), and clusters characterized by high built-up area and low levels of greenness in both children and adults (ORmeta = 1.36, 95%CI: 1.14-1.64 for birth cohorts and ORmeta = 1.15, 95%CI: 1.03-1.28 for adult cohorts, respectively). The joint exposure using the environment risk score combining the three domains was consistently associated with higher risks of incident asthma (ORmeta = 1.13, 95%CI: 1.07-1.20 for birth cohorts, ORmeta = 1.15, 95%CI: 1.10-1.20 for adult cohorts per 20% increase). On average 11.6% of the incident asthma cases could be attributed to environmental risk score above cohort-specific median levels. INTERPRETATION: Multiple environmental exposures jointly contribute to incident asthma risk across the life course. Urban planning accounting for these factors may help mitigate asthma development. Accelerated epigenetic aging has been associated with changes in cognition. However, due to the lack of neuroimaging epigenetics studies, it is still unclear whether accelerated epigenetic. Aging in young adulthood might underlie the relationship between altered brain dynamics and cognitive functioning. We conducted neuroimaging epigenetics follow-up of the European Longitudinal Study of Pregnancy and Childhood (ELSPAC) prenatal birth cohort in young adulthood and tested the possible mediatory role of accelerated epigenetic aging in the relationship between dynamic functional connectivity (DFC) and worse cognition. A total of 240 young adults (51% men; 28-30 years, all of European ancestry) participated in the neuroimaging epigenetics follow-up. Buccal swabs were collected to assess DNA methylation and calculate epigenetic aging using Horvath's epigenetic clock. Full-scale IQ was assessed using the Wechsler adult intelligence scale (WAIS). Resting-state functional magnetic resonance imaging (rs-fMRI) was acquired using a 3T Siemens Prisma MRI scanner, and DFC was assessed using mixture factor analysis, revealing information about the coverage of different DFC states. In women (but not men), lower coverage of DFC state 4 and thus lower frequency of epochs with high connectivity within the default mode network and between default mode, fronto-parietal, and visual networks was associated with lower full-scale IQ (AdjR2 = 0.05, std. beta = 0.245, p = 0.008). This relationship was mediated by accelerated epigenetic aging (ab = 7.660, SE = 4.829, 95% CI [0.473, 19.264]). In women, accelerated epigenetic aging in young adulthood mediates the relationship between altered brain dynamics and cognitive functioning. Prevention of cognitive decline should target women already in young adulthood.

BACKGROUND: Maternal alcohol consumption can adversely affect children's development, but the impact of paternal drinking is less understood. We aimed to investigate whether maternal or paternal alcohol consumption during pregnancy affected children's mental health and behavior. METHODS: A total of 2,013 parent-child triads from the European Longitudinal Study of Pregnancy and Childhood cohort were used. Data on alcohol consumption was obtained from questionnaires during pregnancy and after the child's birth. Mental health and behavior of children were assessed with Strength and Difficulties Questionnaire (SDQ). The associations were tested using linear regression, adjusting for socio-demographic and psychosocial covariates. RESULTS: Increased maternal alcohol consumption was associated with higher total SDQ scores at ages 7, 11, and 18 years old when the outcomes were reported by mothers, but only at 11 years when reported by children. We did not observe any dose-response relationship, and the effect size did not change during the follow-up. The effects were observed across various domains of SDQ: in the emotional symptoms subscale at age 11, in the conduct problems subscale at ages 7 and 11, and in the hyperactivity/inattention subscale at age 18. Paternal alcohol consumption was not associated with SDQ. CONCLUSIONS: Maternal alcohol consumption during pregnancy is associated with long-term effects on children's mental health and behavior, particularly when reported by mothers. No association was found between paternal alcohol consumption, suggesting that the results may stem from biological effects of alcohol or other factors beyond the direct exposure, potentially encompassing broader maternal psychosocial or behavioral characteristics.

Aims. Parental postpartum depressive symptoms have been extensively studied, but the combined longitudinal depression trajectories of parents and their long-term development beyond the postpartum period remain largely underexplored. We identified dyadic longitudinal depressive symptom trajectories in new parents, followed over an 11-year period, and compared parental characteristics, as well as child temperament and mental health factors, across different parental trajectory classes. Methods. A prenatal cohort of 5,518 couples was studied. Depressive symptoms were measured using the Edinburgh Postnatal Depression Scale at eight time points: in the prenatal stage, in the newborn stage, and at 6 months, 18 months, 3 years, 5 years, 7 years and 11 years after the birth of the child. Results. Dyadic Latent Class Growth Modelling identified five classes of couples: (1) mother has elevated depressive symptoms, father is non-depressed (24%); (2) both mother and father have elevated depressive symptoms (20%); (3) both mother and father are constantly non-depressed (42%); (4) both mother and father are constantly depressed (5%); and (5) mother is constantly depressed, father has elevated depressive symptoms (9%). Relationship maintenance (particularly being married or separated) was the most strongly associated with the classes. Socio-economic resources, emotional well-being, health, obstetric history and parental background also served as meaningful covariates. Child temperament and mental health showed weak correlations with parental trajectory classes. Conclusions. Parents with postpartum depressive symptoms often experience depressive symptoms long-term. Separated parents are particularly vulnerable to adverse depressive trajectories. Our findings underscore the importance of dyadic methods in estimating unique combinations of parental depression trajectories.

BACKGROUND: Early life socioeconomic disadvantage and adverse experiences may lead to overeating, which is in turn associated with increased body mass index (BMI). However, recent evidence indicated that the association between childhood BMI and overeating might be bidirectional. This bidirectionality prompts the need for further investigation of early life predictors of BMI in childhood. OBJECTIVES: To longitudinally assess the directionality of the association between childhood BMI and perceived overeating and to investigate their antecedent early life predictors. METHODS: The sample included data from 5151 children from the ELSPAC study, collected between 18 months and 11 years of child age. The outcomes were child BMI and mother-reported overeating, assessed at the age of 3, 5, 7 and 11 years. Predictors included maternal BMI, maternal education, single parenthood, financial difficulties and adverse childhood experiences (ACEs) reported by parents and paediatricians. The random intercept cross-lagged panel model was applied. RESULTS: The mean child's BMI at age 3 was 15.59 kg/m2 and increased to 17.86 kg/m2 at age 11. The percentage of parent-reported overeating increased in the following period, from about 12% at age 3 to 17% at age 11. The results showed temporal stability in perceived overeating and BMI, with a bidirectional relationship strengthening over time. The child's BMI was associated with maternal BMI. Maternal BMI was positively associated with child-perceived overeating, but a stronger effect was found for ACEs. ACEs mediated the impact of maternal education, financial difficulties and single parenthood on overeating. CONCLUSIONS: We observed stable bidirectional associations between BMI and perceived overeating. The results indicated two main pathways: one linked to maternal BMI and early childhood BMI increase followed by perceived overeating and the second associated with ACEs mediating the effect of early childhood social factors on perceived overeating, leading to gradual BMI gain.

BACKGROUND: Nutrient deficiencies during pregnancy may affect offspring development. We aim to examine the association between prenatal vitamin B12 intake and children's cognitive development. METHODS: A total of 5151 mother-child pairs from the Czech part of ELSPAC study were included in the analysis. Dietary information was obtained during pregnancy using food frequency questionnaire. Parents reported on their child's speech and language development at 18 months, 3, 5 and 7 years. Intelligence quotient (IQ) was measured at 8 years in subcohort of 854 children. RESULTS: Children of mothers with higher vitamin B12 intake demonstrated higher scores in language (B = 0.20, 95% CI 0.06, 0.34) and talking and understanding (B = 2.39, 95% CI 0.97, 3.80) in a fully adjusted model at 18 months. Additionally, they were more likely to get maximum points in the intelligibility test at age 3 (OR = 1.05, 95% CI 1.01, 1.09) in unadjusted model, however, not in fully adjusted model. We found a positive effect of higher vitamin B12 intake on verbal IQ (B = 1.08, 95% CI 0.09, 2.08). CONCLUSIONS: We identified consistent associations between prenatal vitamin B12 intake and children's cognitive development. The results suggest that inadequate vitamin B12 during pregnancy may negatively affect children's cognitive development, particularly in speech and language.

BACKGROUND: Maternal perinatal mental health is essential for optimal brain development and mental health of the offspring. We evaluated whether maternal depression during the perinatal period and early life of the offspring might be selectively associated with altered brain function during emotion regulation and whether those may further correlate with physiological responses and the typical use of emotion regulation strategies. METHODS: Participants included 163 young adults (49% female, 28-30 years) from the ELSPAC prenatal birth cohort who took part in its neuroimaging follow-up and had complete mental health data from the perinatal period and early life. Maternal depressive symptoms were measured mid-pregnancy, 2 weeks, 6 months, and 18 months after birth. Regulation of negative affect was studied using functional magnetic resonance imaging, concurrent skin conductance response (SCR) and heart rate variability (HRV), and assessment of typical emotion regulation strategy. RESULTS: Maternal depression 2 weeks after birth interacted with sex and showed a relationship with greater brain response during emotion regulation in a right frontal cluster in women. Moreover, this brain response mediated the relationship between greater maternal depression 2 weeks after birth and greater suppression of emotions in young adult women (ab = 0.11, SE = 0.05, 95% CI [0.016; 0.226]). The altered brain response during emotion regulation and the typical emotion regulation strategy were also as sociated with SCR and HRV. CONCLUSIONS: These findings suggest that maternal depression 2 weeks after birth predisposes female offspring to maladaptive emotion regulation skills and particularly to emotion suppression in young adulthood.

The pace of biological aging varies between people independently of chronological age and mitochondria dysfunction is a key hallmark of biological aging. We hypothesized that higher functional impact (FI) score of mitochondrial DNA (mtDNA) variants might contribute to premature aging and tested the relationships between a novel FI score of mtDNA variants and epigenetic and biological aging in young adulthood. A total of 81 participants from the European Longitudinal Study of Pregnancy and Childhood (ELSPAC) prenatal birth cohort had good quality genetic data as well as blood-based markers to estimate biological aging in the late 20. A subset of these participants (n = 69) also had epigenetic data to estimate epigenetic aging in the early 20s using Horvath's epigenetic clock. The novel FI score was calculated based on 7 potentially pathogenic mtDNA variants. Greater FI score of mtDNA variants was associated with older epigenetic age in the early 20s and older biological age in the late 20s. These medium to large effects were independent of sex, current BMI, cigarette smoking, cannabis, and alcohol use. These findings suggest that elevated FI score of mtDNA variants might contribute to premature aging in young adulthood.

OBJECTIVES: The objective of this study was to identify typologies of activity-related behaviours during adolescence and to explore transitions between the identified typologies. Additionally, we aimed to identify demographic indicators associated with the transitions and typology membership. DESIGN: Prospective cohort study. SETTING: Czech Republic. PARTICIPANTS: Individuals involved in the Czech part of the European Longitudinal Study of Pregnancy and Childhood study, aged 11 to 18 years. The study involved over 563 individuals, of whom 380 provided complete data for the analysis. PRIMARY OUTCOME MEASURES: Time spent outdoors, participation in organised physical activity (PA) and sport activities, time spent watching television and using a personal computer, and total sleep duration at ages 11, 15 and 18 years. Typologies were identified using Latent Transition Analysis. RESULTS: Four typologies of activity-related behaviours were identified and labelled to reflect their behavioural profiles: (1) Actives (high outdoor time and organised PA and sport participation, low screen time and optimal sleep duration); (2) Active screeners (median outdoor time, high organised PA and sport participation, high screen time, and optimal sleep duration); (3) Poor sleepers (average outdoor time and organised PA and sport participation, low screen time and not meeting sleep guidelines) and (4) Averages (average duration of all behaviours and optimal sleep duration). A major shift in typology membership from 11 to 18 years was observed, with a decreasing proportion of individuals in typologies characterised by a high proportion of outdoor time and participation in organised PA and sport activities (ie, Actives; Active screeners). A high proportion of individuals also transitioned to the typology with poor sleeping habits (ie, Poor sleepers). Sex and maternal education were associated with the typology membership and transition probabilities (p<0.05). CONCLUSIONS: Targeting lifestyle interventions to those with specific lifestyle patterns in early adolescence may be beneficial for reducing the risk of poor sleep and promoting healthy lifestyle patterns later in life.
