Cíl studie: Shrnout novější poznatky o mechanismech, ovlivňujících implantaci / nidaci embrya. Typ studie: Souhrnná studie založená na literárním přehledu. Název a sídlo pracoviště: Ústav pro péči o matku a dítě, Praha. Předmět a metoda studie: Faktory, ovlivňující implantaci-nidaci embrya v děloze, se dostávají do popředí zájmu v souvislosti s podrobným studiem příčin neobjasněných sterilit a opakovaného potrácení a s rozvojem techniky in vitro fertilizace s následným přenosem embrya. Práce má upozornit na některé novější poznatky o mechanismech regulujících nidaci a o jejich hlavních poruchách. Jde zejména o imunitní faktory (protilátky, buňky, cytokiny a další humorální působky), jejichž přesná regulace na fetomaternálním rozhraní je rozhodující podmínkou úspěšné nidace. Nové poznatky se objevily v genetice časného embrya, nové pohledy přinášejí moderní endoskopické operační techniky. Závěr: Doporučení, vyplývající z uvedených poznatků pro diagnostiku a léčbu poruch nidace embrya, jsou uvedena v závěru článku.

Objective: To summarize recent knowledge concerning mechanisms which influence the implantation of embryo. Design: Literature-based overview. Setting: Institute for the Care of Mother and Child, Prague. Subject of the study: Factors influencing implantation of embryo in the uterus elicit increased interest due to study of unexplained failures of embryotransfer following the successful in vitro fertilization. Our article points to recent information about physiology and pathology of mechanisms controlling implantation, namely the factors of immunity (antibodies, cells, cytokines and other mediators) whose exact regulation on the feto-maternal interface is a crucial precondition of successful implantation. Also the genetics of early embryo, as well as the possibilities of modern endoscopic techniques offer new insight onto mechanisms of implantation. Recommendations for diagnostics and treatment of implantation failure are given in the end of the article.
