Komplikace po totální endoprotéze kolenního kloubu u pacientů s revmatoidní artritidou

Náhrada kolenního kloubu totální endoprotézou je součástí operační činnosti na všech větších ortopedických pracovištích. Implantace u nemocných s revmatoidní artritidou patří z hlediska možného výskytu komplikací k nejrizikovějším výkonům. Na ortopedické klinice bylo v letech 1990 – 2004 operováno 67 klientů s revmatoidní artritidou, jimž byla aplikována náhrada v 86 případech. Sdělení je zaměřeno na počty komplikací, jejichž výčet a řešení je dokládán. Zcela bez komplikací byl peroperační a pooperační průběh u 57 endoprotéz (66,3 %), v 15 případech (17,4 %) se autoři setkali s minimálními obtížemi a u 14 aloplastik kolenního kloubu (16,3 %) s komplikacemi s důsledky. Tyto výsledky jsou konfrontovány s odbornou literaturou. Závěrem je konstatováno, že operativa endoprotéz kolenního kloubu u nemocných s revmatoidní artritidou je vždy rizikovým úkonem. Z komplikací, které se v souboru vyskytly, by se autoři rádi poučili pro další praxi.
The knee joint replacement by total endoprosthesis is performed in all larger orthopedic hospitals. With respect to possible complications in patients with rheumatoid arthritis, it is one of the most dangerous procedures. Sixty-seven patients with rheumatoid had arthritis had 86 knee joint replacements in orthopedic clinic between 1990 and 2004. The authors describe many complications and their treatment in this paper. No per-operative and post-operative complications in 57 endoprostheses (66.3%), minimal troubles in 15 cases (17.4 %), and complications in 14 knee joint endoprostheses (16.3 %) were observed. These results are reviewed with respect to current literature data. It is summarized that surgery of the knee joint endoprostheses in patients with rheumatoid arthritis is always somewhat risky procedure. The authors would like to learn for the future practice on the basis of the complications occurring in the set of those patients.
