V příspivku je uvedena stručná kazuistika diarea-pozitivního hemolyticko-uremického syndromu u dvouleté dívky, který skoneil úmrtím pacientky. Puvodcem bylo enterohemoragické Escherichia coli (EHEC) O26 s produkcí Shiga toxinu 2. V elánku je zduraznino, že toto vážné onemocniní mohou, kromi nejeastiji uvádi- ného sérotypu E. coli O157:H7, vyvolat i kmeny dalších séroskupin: O26, O111, O103 a O145. V NRL pro E. coli a shigely SZÚ jsme za ideálních okolností schopni zjistit poítomnost EHEC do 24, resp. do 48 hodin.

We report a fatal case of diarrhea-associated hemolytic uremic syndrome (HUS) in a 2 year-old girl, caused by enterohemorrhagic Escherichia coli (EHEC) O26 producing Shiga toxin (Stx) 2. This case demonstrates that in addition to E. coli O157:H7 which is the most common cause of HUS, strains of other serogroups (O26, O111, O103, O145) can also cause this severe disease. Stool specimens from patients with suspected EHEC infections should be sent to NRL for E. coli and Shigellae at NIPH for molecular and phenotypic detection of stx/Stx that is feasible within 24-48 h.
