Srdeční resynchronizační terapie se stala nedílnou součástí léčebného spektra u pacientů s pokročilým srdečním selháním. Indikační doporučení České kardiologické společnosti z roku 2008 vycházela z předcházejících Evropských doporučení pro léčbu srdečního selhání a srdeční resynchronizační terapii. Po zveřejnění výsledků velkých randomizovaných studií u pacientů s mírným srdečním selháním byla Evropská doporučení v roce 2010 inovována. V našem přehledném článku porovnáváme jednotlivé verze doporučení a rozebíráme indikační kritéria pro srdeční resynchronizační terapii v některých specifických skupinách pacientů, např. u nemocných s permanentní fibrilací síní, se štíhlým komplexem QRS a u pacientů ve vyšším věku s významnými komorbiditami.

Cardiac resynchronization therapy has become an integral part of the therapy spectrum in patients with advanced heart failure. Guidelines for cardiac resynchronization therapy issued by the Czech Society of Cardiology in 2008 were based on the preceding version of European guidelines for the treatment of heart failure and cardiac resynchronization therapy. European guidelines have been updated in 2010 after results of major randomized trials in patients with mild heart failure became available. In this article we compare individual versions of the guidelines and discuss indications for cardiac resynchronization therapy in specific subgroups, such as patients with permanent atrial fibrillation, narrow QRS, or elderly population with significant comorbidities.
